Theyinform patients about treatment and rehabilitationoptions, about health careproviders, and about other TK services. Ina key feature, the model coordinates careacross different health care sectors.Through the CCM approach, TK not onlyassures the best possible care for itsinsurees, but also safeguards its long-termfinancial sustainability.
