[영문]In December 2004, the BertelsmannStiftung, The Commonwealth Fund, andAcademyHealth jointly sponsored a conferencein Berlin, Germany on “Case Studies inChronic Care Management.” This conferenceassembled representatives from the U.S.and German Governments, several organizationsthat had developed chronic caremanagement initiatives in the two countries,and other health care and policy experts todiscuss clinical, organizational, and financingissues. The case studies discussed at theconference are presented in this issue of theReview; this article describes some relevantconsiderations that are common to the U.S.and German health systems.
