[영문]Phase I of the voluntary chronic careimprovement (CCI-I) under traditional feefor-service Medicare initiative seeks toextend the benefits of disease managementto an elderly population with comorbidchronic medical conditions. Active, sustainedinvolvement of treating physicians, ahistorical deficit of disease managementprograms, is a CCI-I program goal. Duringthe last decade, Kaiser Permanente, anintegrated health care delivery system withmore than 60 years of experience in managingthe care of individuals and populations,has applied the chronic care model(CCM) to develop care management strategiesfor populations of patients with chronicmedical conditions. Physician leadershipand involvement have been key to successfullyincorporating these practices intocare. The scope of physician involvement inleading, developing, and delivering chronicillness care management at KaiserPermanente is described as a basis for identifyingopportunities to involve practicingphysicians in the CCI-I.
