[영문]Chronic illness combined with functionalimpairment often results in an increasedneed for medical care and supportive longtermcare (LTC) services. Navigating thehealth care system is challenging and complex,and even more so for patients withcomplex needs. Traditional fee-for-servicecare does not support and facilitate coordinationand collaboration between providersand service settings. In New York State,managed LTC, a model of coordinated carefor the chronically ill, endeavors to providea bridge between primary, acute, home andcommunity-based, and institutional LTCservices for a medically complex and functionallyfrail nursing home eligible population.
