[영문]Hospital admissions among patients withcongestive heart failure (CHF) are a majorcontributor to health care costs. A comprehensivedisease management program forCHF was developed for private and statutoryhealth insurance companies in order toimprove health outcomes and reduce rehospitalizationrates and costs. The programcomprises care calls, written training material,telemetric monitoring, and healthreports. Currently, 909 members from sixinsurance companies are enrolled. Routineevaluation, based on medical data warehousesoftware, demonstrates benefits interms of improved health outcomes andprocesses of care. Economical evaluation ofclaims data indicates significant cost savingsin a pre/post study design.
