[영문]Physicians in a region south of Bremen,Germany, created PRO DEM (for patientswith dementia), a service coordinating medicalcare and social intervention for patientswith dementia. The program now servesabout 125 outpatients and their families,along with 360 patients in nursing homes.Two nurse consultants coordinate patientcare. Treatment strategies are decided duringa multidisciplinary case conference amongphysicians, nurses, and other professionals.Social intervention modules include variouscare groups for patients and respite servicesfor caregivers. PRO DEM aims to providefully coordinated care for a better quality oflife for patients and caregivers, delaying nursinghome admission as long as possible.
