Abstract

초록

본 연구는 우리나라 실업가구를 대상으로 실업가구의 특성에 따라 다르게 나타나는 실업후의 의료이용 중단을 파악하고 이에 영향을 미치는 요인을 살펴봄으로써 실업가구 유형별로 대응방안을 강구하는 것을 목적으로 하고 있다. 지금까지 수행된 의료서비스 이용을 결정하는 요인에 관한 연구는 주로 일반인을 대상으로만 이루어져 저소득층 및 실업자 등 특수계층에 관한 연구는 거의 이루어지지 못한 실정이다. 최근 경제위기를 경험한 국내에서도 실업이 건강에 미치는 영향에 관한 연구는 심리적 측면인 정신적 건강 및 실업가구의 의료이용 감소에 관한 실증적인 연구는 이루어졌으나, 의료기관이용 감소가 어떤 요인에 의해 영향을 받는지를 규명한 연구는 거의 없는 실정으로 이에 대한 연구가 절실히 요구되고 있다. 본 연구는 Andersen의 의료이용 모형을 가구주의 특성에 적용한 배상수 모델에 가구유형 및 실업후의 경제상태를 나타내는 변수를 추가하여 새로운 모형을 설정하여 사용하였다. 연구결과, 실업가구의 의료기관이용 중단에 가장 큰 영향을 미치는 요인은 생계유지의 어려움이었고, 그 다음으로 영향력 있는 요인은 가구주 연령, 가구재산, 실업자수, 노인가구 여부, 의료보장수혜 여부 순으로 나타났다. 따라서 이러한 연구결과에 근거하여 의료중단이 높은 실업가구부터 우선순위를 정하여 기본적인 의료서비스를 제공함으로써 의료안전망으로부터 소외되는 계층이 발생하지 않도록 이에 대한 예방대책이 마련되어야 한다. 우선적으로 실업가구의 의료비 부담을 낮추어 의료기관을 지속적으로 이용할 수 있도록 실업자 및 그 가족에게 공공기관 이용 등의 대체의료자원의 공급 및 의료비 보조를 위한 지원이 이루어져야 할 것이다. 또한 의료보장권 안에 있는 실업가구에는 의료보험료 지원비율을 한시적으로 확대하고, 의료보장권 밖에 있는 실업가구는 의료보장의 수혜범위를 확대하도록 한다. 이러한 수혜기준은 의료이용 중단이 가장 높은 실업가구인 생계유지가 어려운 저소득층 실업가구, 60세 이상의 노인가구주 실업가구, 재산이 적은 실업가구, 실업자수가 많은 실업가구, 급성질환 발생 가능성이 높은 비노인층 실업가구, 의료보장의 혜택을 받지 못하는 계층의 실업가구를 우선적인 관리대상자로 지정하여 제공하여야 할 것이다.;The purposes of this study are to find the reason why unemployed households stop the use of medical institution after being dismissed from their job, to find the factors influencing their use of medical institution, and to suggest the policy measures which can remove these factors. Many of the previous studies conducted in Korea have not been paying sufficient attention to needy people and unemployed people's households. Also, even the studies conducted on the health status of unemployed people's households after the 1997 economic crisis were focused only on the psychological aspects of those households. The model of this study is newly developed by adding the variable which express the factors of unemployed households type and post-unemployment economic conditions to Baesangsoo's model of using the medical institution. The main factor that prevents unemployed people's households from utilizing medical institutions is their difficulty of maintaining living standards. Other factors include age of the head of household, property of the household, number of the unemployed, the number of aged family members in the household, and medical security. This study suggests that the government should make a network of medical security with reference to the factors and should provide basic medical services in order for unemployed people's households to have proper medical services. In other words, the government should provide financial support for unemployed people's households so that they can continuously receive their medical services. The government should temporarily strengthen the medical insurance support for these households to increase their medical security. The government should also extend the national medical security to cover the currently excluded.
