알코올 의존 환자들은 퇴원 후 평생 동안 재발 위험에 직면하므로 만성질환 관점에서 지속적인 관리가 요구된다. 퇴원 후 지속적인 외래 방문은 알코올 의존 환자의 결과에 긍정적인 영향을 주지만, 지속적인 외래 방문 수준은 낮다고 보고되고 있다. 따라서 우리나라의 알코올 의존 환자의 지속적인 외래 방문 현황을 파악하고, 이에 영향을 주는 환자 특성을 파악하는 것은 환자 퇴원 계획 수립, 재발 방지 프로그램 개발 등에 활용될 수 있다. 본 연구에서는 건강보험심사평가원의 진료비청구자료를 사용하여 알코올 의존 입원 환자를 선정하고, 퇴원 후 지속적 외래 방문율을 분석하였다. 2010년 알코올 의존으로 입원한 환자는 18,835명이었고, 평균 연령은 50.4세, 여성은 11.6%였다. 알코올 의존으로 퇴원한 환자 중에서 6개월 동안 한 달에 최소한 한번 이상 지속적으로 외래를 방문한 환자는 8.3%에 불과하였고, 특히 3개월 내에 지속적 외래 방문율이 크게 감소하였다. 환자 특성별로는 연령이 젊을수록, 남성, 의료급여 환자일수록 지속적인 외래 방문율이 낮았고, 내과적, 정신적 질환이 동반된 환자, 이전 입원 경험이 있는 환자는 지속적인 외래 방문율이 높았다. 따라서 퇴원 후 지속적 외래 방문율이 낮은집단에 초점을 둔 퇴원 교육과 지역사회 서비스 연계를 강화하는 노력이 요구된다.;Alcohol-dependent patients face a substantial risk of relapse after inpatient care in their lives. The continuing outpatient care after intensive inpatient care is an important role to reduce the risk of relapse. However, there has been relatively little research on the affecting factors of continuing care. This study aimed to identify the continuous outpatient visit rate and determine factors to affect the continuous outpatient visit after discharge. A continuous outpatient visit is defined as an outpatient visit that is made at least once every 6 months. A total of 18,835 alcohol-dependent patients were followed up for 6 months after discharge. We performed multiple logistic regression to analyze the association between continuous outpatient visit and patient’s characteristics. Only 8.3% of patients received continuous outpatient care within 6 months of their discharge. Patients who were younger, male, and under medicaid were less likely to make post-discharge outpatient visits on a continuous basis. Effort needs to be exterted to provide adequate discharge education programs and to strengthen community services for these patients.
