의료자원배분의 효율성이나 의료비용 억제 측면에서 문제점이 있는 제도로써 비판을 받고 있는 행위별수가제의 대안으로 진료비를 사전적(prospective)으로 예측 가능한 총액으로 지불하는 총액예산제의 도입방안을 검토하였다. 선진국들은 1970~80년대의 진료비증가에 대처하고 의료보장성을 확보하기 위하여 예산제를 대부분 채택하였다. 그런데 총액 예산제를 단기적으로 도입하기에는 우리 의료시장의 특성상 많은 장애요인을 안고 있으며, 장애요인을 극복하기 위한 세심한 준비와 여건조성을 다져나가야 한다. 진료비총액을 계약하더라도 비 보험 진료부문으로 비용을 전가하는 왜곡을 방지하기 위하여 보험급여를 확대하되 재원조달의 한계를 감안하여 본인 부담률을 조정하여야 할 것이다. 혹은 비 보험 진료의 투명성을 확보하기 위하여‘전자진료카드’의 도입이 필요하다. 의료서비스의 질(성과)과 연계한 총액예산을 계약하되, 배분을 의료공급자단체에 맡기고, 의료기관 및 병상, 의료인력 등 의료자원에 대해 의료계에서 스스로 조절할 수 있는 자율을 부여하는 방안을 검토할 수 있다. 그러나 서비스의 질 확보를 위한 의료자원 투입기준을 마련하거나, 서비스 질 평가에 근거한 성과중심의 예산제가 도입되어야 한다. 총액 진료비를 통제하는 대신에 의료공급에 대한 규제를 완화하기 위해서 요양기관 계약제로 전환하는 방안을 검토한다. 한편 총액예산과 의료서비스의 질(성과)만 계약하는 방식을 취하지 않는 한, 진료영역별 지불보상방식(즉 행위별수가 혹은 DRG, 일당제, 인두제 등)을 비롯한 보험약가, 재료 대를 포함하는 포괄적인 계약이 되어야 할 것이다.;Despite its positive effect on increasing the quality of healthcare services and practitioner’s self-regulation, the current fee-for-service reimbursement has been often criticized as an inefficient means of curbing expenditures and allocating resources. Most advanced countries have been adopting global budgeting system to induce cost-effective treatment behaviors from healthcare providers and to increase equity in the National Health Insurance. In this context, this study aims to examine the appropriateness of introducing global budgeting into Korea. To this end, this study: i) theoretically reviews previous studies and the pros and cons of global budgeting; ii) examine reimbursement systems in advanced countries, and draws implications for Korea; and iii) propose ways to introduce global budgeting to the National Health Insurance. With its in-depth examination of risk factors and proposed solutions associated with the introduction of global budgeting, this study will contribute to further research on improvement of reimbursement system. Also, in proposing ways to make global budgeting beneficial to consumers, providers and the insurer, this study can serve as an effective bridging tool between various interest groups.

아스피린은 심혈관계 질환의 위험이 있는 당뇨병환자들에게 심혈관계 질환의 일차예방 및 이차예방을 위해 필요하다. 본 연구는 미국의 당뇨병 환자들 가운데서 아스피린의 사용자수를 파악하고 아스피린 사용과 관련된 요인을 발견하는 것이 목적이다. 연구방법으로는1999년 Behavioral Risk Factor Surveillance System (BRFSS) 이 연구자료로 사용되었다. 17개 주만이 당뇨병과 심혈관계 질환에 관한 자료를 가지고 있어 이들 17개 주만 표본에 포함되었고, 17개주의 모든 관찰자가 분석에 사용되었다. 건강행태와 관한 앤더슨 모델이 수정되어 분석모형으로 사용되었으며, 가중치를 사용한 로지스틱 분석으로 아스피린의 사용과 관련되어 있는 요인들을 분석하였다. 연구결과 당뇨병 환자들 가운데 14. 75 %만이 심혈관계 질환의 예방목적으로 아스피린을 사용한 반면, 당뇨병을 갖고 있지 않은 사람들의 14. 67%가 심혈관계 질환의 예방목적으로 아스피린을 사용하여 당뇨병 환자들과 차이가 없었다. 당뇨병 환자들 가운데 7 1세 이상 노인, 남자, 대학졸업자, 콜레스테롤이 높은 자, 매일 흡연자, 음주자, 비만자, 심근경색이나 관상동맥질환을 앓은 적이 있는 자, $14, 999 이하의 저소득계층, 무보험자와 같은 특성을 가진 사람들이 심혈관계 질환의 예방을 위한 아스피린 사용이 적었다. 결론적으로 대부분의 당뇨병 환자는 심혈관계 질환을 앓을 위험요인을 가지고 있다. 그럼에도 불구하고 당뇨병 환자들의 아스피린 사용률은 매우 낮았다. 당뇨병 환자들의 심혈관 질환 예방 목적의 아스피린 사용을 촉진하기 위해서는 의사 및 아스피린 사용률이 상대적으로 낮은 당뇨병 환자들을 대상으로 한 교육이 매우 필요하다.;Objective: Aspirin is known to be a good agent for the primary prevention as well as the secondary prevention of cardiovascular diseases. The objective of the study was to estimate the prevalence of aspirin use and to find factors that are related with aspirin use among diabetic patients in the U . S . Methods: The Behavioral Risk Factor Surveillance System (BRFSS) of 1999 was used for the data. Only 17 states had the information on diabetes and cardiovascular diseases and all the observations of these 17 states were included in the analysis. Anderson’s health-behavior model was slightly modified and used as the analytic framework. The problem from using aspirin was added in the Anderson’s model along with predisposing, enabling, and need factors. Logistic regression with the weight option was used to estimate the factors that may be associated with aspirin use. Results: The percentage of aspirin use among diabetic patients was 14.75%, while 14.67% of all the other observations used aspirin. Diabetic patients who had the following characteristics used aspirin less; aged 71 or older, male, college education, high cholesterol, smoking everyday, drinkers, obese people, having had myocardial infarction or coronary heart disease, low income (under $14,999), uninsured. Conclusion: Despite the fact that most of diabetic patients have at least one risk factor of cardiovascular disease, the prevalence of aspirin use was very low. To enhance aspirin use among diabetic patients, there are needs for the physician education for diabetic patients in general, and the targeted education for those diabetic patients whose aspirin use was particularly low.
