Abstract

The purpose of this study was to examine health care expenditure of the age group of 65 years and older and to analyze the factors associated with the expenditure. The study was conducted with data on three programs in the Korean Public Health Care System: Korean National Health Insurance(NHI), Medical Aid and Long-term Care Insurance, 2003-2009. The results are consistent with previous literature that the expenditure has been increased rapidly, accounting for 31.4% of the total treatment expenditure of NHI in 2009. The increase rate reaches at 24% in 2009, which is the highest rate since 2003. The factors include the number of older population, levels of income (eligible program types), health care utilization types (in-patient/ out-patient/ pharmaceutic), and related policy changes. The increasing number of older population contributes 16%~35% to increase the total treatment expenditure for the old age group, depending on the utilization and program types. The result shows that the changes in health care expenditure of older Medical Aid’s beneficiaries, different from that of older NHI beneficiaries, is not relevant to the economic situation. Since older persons are likely to suffer from chronic diseases, it is recommended that the individual’s designated physician(clinic) program be implemented and the continuum of care be improved for effective chronic disease management. Lastly, high increase rate of the days of stay in hospitals should be prevented by reducing oversupply of acute care hospital beds and promoting appropriate patient referral to long term care facilities.

Although many Koreans have purchased private health insurance (PHI), there has few study defining characteristics of those who purchase multiple PHI. This study examined the determinants of purchasing multiple PHI in Korea. The subjects were a national random sample of the aged 45 or older from the Korean Longitudinal Study of Ageing in 2006, and 9,367 samples were used for the analysis. The Heckman’s sample selection model was used to deal with non-random assignment to those who purchase PHI from the whole sample of the study. The model examined the determinants of the purchasing multiple PHI in terms of the demographic and socioeconomic characteristics, health status and health behaviors. Thirty-one hundred and sixty people purchased PHI, and 47.8% of them had multiple PHI. A female, the younger, or the educated was inclined to have multiple PHI. Individuals who have more children were less likely to have multiple PHI. Those who have private transfer or higher total household assets are more likely to purchase multiple PHI. Those who self-assessed their health status as better compared to poor conditions tended to have multiple PHI. Regular exercise was positively related to having multiple PHI. This study has important implications which examined determinants of purchasing multiple PHI to the extent that many Koreans have purchased PHI even with the enrollment in social health insurance. This study also suggests that purchasing PHI needs to be interpreted in the context of playing a role in accordance with social health insurance.

This study examines the scale of occurrence of Catastrophic Health Expenditure (CHE), and identifies the factors influencing CHE among low-income households. Korea Welfare Panel (KOWEP) (2006) were used in the study. CHE is defined by when the households' medical spending out of ability to pay exceeds 10%, 20%, 30%, and 40%. The study examined the frequency of CHE with 6,992 households, Moreover, among 3,233 low-income households whose income amounts to less than 60% of the median income, logistic regression was conducted. The occurrence of CHE in the entire household appeared to be 19.1%, 9.7%, .8%, and 3.7% with the threshold at 10%, 20%, 30%, and 40%. For the low-income households, it accounted for 32.2%, 19.5%, 12.0%, and 7.8%, which showed a significant increase compare to the average households. The following cases showed high chances of incurring CHE: When the householder is female, older than 65, unemployed, less educated, has no spouse, has poor perceived health status, has a low income, has chronic sufferers in the family, has no child under five years old, and has a small number of family members. It also showed that those with national health insurance as a form of medical security rated higher than those with medical aid. The higher incidence of CHE among low-income households implies that South Korea offers a low level of medical security, which can drive its people to poverty.
