예산에 의해 운영되는 의료시스템의 재정적자문제는 시스템 자체의 존폐를 결정하는 중요한 문제이다. 만약보다 더 근본적이고 현실적인 원인 을 알 수 있다면 그 문제의 해결이 좀 더 희망적일 수 있다고 생각된다. 본고는 각 국의 재정문제의 차이를 가져오는 요인 의 설명에 역점을 두는 기존의 연구와는 달리‘ 왜 현대의료시스템을 구현하고 있는 나라들에서는 공통적으로 재정적 감당능력에 비해 의료비의 수준이나 증가가 과도한 현상이 일어나고 있는 가’라는 질문에 답하고자 한다. 그 이유는 만일 그러한 공통적인 원인 이 존재하고 밝혀질 수 있다면, 차이를 설명하는 설명보다 더 문제의 핵심에 접근할 수 있다고 생각하기 때문이다. 즉 그 공통의 부분이 문제의 대부분과 그 발전방향을 결정한다고 생각되기 때문이다. 현대의료시스템의 재정문제는 일반적으로 정보의 비대칭성과 질병발생의 불확실성으로 대표되는 보건의료시장의 특이성과 이에 대한 제도적 대응방식 그리고 의료에 대한 사회적 가치관에 의해야기 되는 것으로 설명되고 있다. 이에 대해 본 연구는 현대사회의 공통적인 기능분화적인 사회기 전을 통해 각국에 공통적으로 적용될 수 있는 현대의료시스템의 개념을 규정하고, 이 현대의료시스템을 형성시키는 의미구조가 재정문제를 일으키는 행위를 발생시키는 의미구조임을 밝힌다. 그리고 기존의 설명 중 중요한 부분이 이 현대의료시스템을 통한설명에 포함될 수 있음을 밝힌다. 현대의료시스템은 역사적으로 형성된 현대 기능분화사회의 한 기능시스템이다. 현대의료시스템을 규정하는 제일 중요한 요소는 전통의료와는 달리 병의 진단과 치료가 그것을 전담하는 닫쳐진 전문화된 시스템 안에서 만행해진다는 것이다. 이러한 닫쳐진 전문화된 시스템의 형성은 양가코드(binary code)의 의미적 특성에 의해 결정된다. 그 특성은 제삼의 가능성을 제외하고, 의미의 규정이 양가코드의 두 값 사이만을 왕복하게 하는 것이다. 현대의료시스템을 형성시키는 양가코드는 “병든/건강한”이다. 이 양가코드의 양의 값 혹은 규정값은“병든”이고, 負의 값 혹은 비규정값은“건강한”이다. 양가코드에서통상양의 값은 사회에서 선호되어 행위의 접속을 가져오는 값이고, 부의 값은 양의 값에 대해 부정의 가능성을 생각하게 하여 반성을 가능하게 하는 것이다. 이러한 일반적 특성에 비교해 볼 때 현대의료시스템의 양가코드는 양의 값에 일반적으로 비선호되는 “병든”이 있고, 부의 값에 현대의료시스템의 목적인 “건강한”이 놓여있다. 따라서 현대의료시스템에서 행위는 “병든” 이라는 값에 반응하여 이루어지고, 이는 진단과 치료라는 행위이다. 문제는 그 행위에 대한의 문이나 반성을 일으키는 부의 값이 진단과 치료의 목적인 “건강한”이어서 진단과 치료에 대한의 문이나 반성이 전혀 일어나지 않게 되어 있다는 것이다. 이러한 현대의료시스템의 행위를 결정하는 양가구조의 특성 때문에 현대의료시스템에서는 진단과 치료에 소요되는 재정이 정말가치가 있는 가에 대한반성이 근본적으로 결여되게 되어있다. 이에 현대의료시스템의 재정문제의 근원이 놓여있다고 본다. 그리고 이러한 행위가 특히 전문화되고 닫쳐진 현대의료시스템에서의 공급자의 독점적지위에 의해 더욱 쉽게 관철될 수 있는 것이다. 이러한 현상의 근본적인 변경은 현대의료시스템의 행위를 방향 지워주는 양가코드의 변경에 의해서 바뀔 수 있다. 그 한 가지 가능성으로 유전자 연구와 관련해서중요해지고 있는 유전적으로 “치료가능한/치료불가능한”과 같은 양가코드가“병든/건강한”을 대체하여 현대의료시스템의 행위를 결정하는 양가코드가 되는 것이다.;The continued deficit problem of the modern medical systems can threat the survival of the medical systems that depend on the budget. The root of this financial problem seems to lie in the constitution way of the action in the modern medical systems. The modern medical system is the product of the functional social d i fferentiation in the modern society. The major functional systems in the modern society are constructed by the binary codes. Binary codes are composed of contrasted two values: the positive or designation value and the negative or designationfree value. This structure of the binary codes lead to the two consequences that are important for our study. 1. The positive value can find the connective action or reaction in the society. The negative value raise a doubt about the action constituted by the positive value. This reflection controls the action in a system that is constructed by the binary code. 2. The binary codes that exclude the third possible value lead to the closed system. That is, the meaning that guides a action in the system oscillates only between the positive and the negative value. There are no other possibilities in the system constructed by the binary code. This corresponds to the specialization in the modern society. The special feature of the modern medical system is that the diagnosis and therapy are practiced only in the medical system. The modern medical system is closed with the functions of the diagnosis and therapy. These functions are fully differentiated by the binary code “sick/healthy”. This binary code of the modern medical system has a distinctive feature. The positive value of a binary code leading to the full differentiation of a modern functional system as economy, politic, law, education etc is usually the preferred value in a society. But the positive value of the modern medical system is “sick” and leads to the action of the diagnosis and the therapy. And the negative value that should raise the doubt about the action of the diagnosis and therapy in the medical system is “healthy”. But this negative value is the goal of the action in the modern medical system. Therefore it cannot play the usual role of the negative value of a binary code, i.e. the raise of the doubt about the action in the system. This results in the lack of the reflection on the value of the diagnosis and therapy in the modern medical system. The value of the diagnosis and the therapy is too natural for the system to lead to the doubt about its value. This seems to be the root of the financial problem in the modern medical system. The financial problem is amplified by the fact that the specialization or the full differentiation of the medical system give doctors the monopoly power that can realize the action of the diagnosis and therapy as the system dictates. Man can find the fundamental solution about the financial problem of the modern medical system from the above diagnosis of the problem, although it is a very long-range solution. If man can find a new binary code that can raise the reflection on the value of the diagnosis and the therapy in the modern medical system and if the new code can succeed in becoming the binary code that constitute the action of the modern medical system, then the financial problem can be solved fundamentally. Such a possibility can be found in the field of the genetics, where the binary code “curable/incurable” from the genetical perspective play the important role.
