청소년 비만의 사회경제적 비용

초록

청소년 비만은 성인병의 원인이 될 수 있으며, 생리적 기능을 저하시키고, 정서적 스트레스를 주는 등 개인의 신체적·정서적 건강에 부정적인 영향을 미치게 된다. 본 연구에서는 12~18세의 청소년 비만을 예방하고 조기진단 및 치료의 중요성에 대한 근거를 마련하기 위해 청소년의 과체중 및 비만으로 발생하게 되는 사회경제적 비용을 추계하였다. 다만, 청소년이 비만하더라도 질환이환율이 낮으며, 대사와 내분비 이상과 관련한 질환이 임상적으로 발현하기까지는 장기간의 시간이 소요되므로, 청소년의 과체중 및 비만이 장기적으로 성인에 이르기까지 발생하게 되는 사회경제적 비용을 중심으로 분석하고자 하였다. 비만의 사회경제적 비용을 도출하기 위해서 비용을 직접비용과 간접비용으로 구분하고, 연구 대상 집단의 비만유병률과 비만관련 질병 발생에 영향을 미치는 비만의 인구기여분(population attributable fraction: PAF)을 적용하였다. 분석결과, 비만과 관련된 질환으로 당뇨, 암, 뇌혈관질환, 고혈압성질환, 관절증 등이 있었으며, 이로 인해 약 1조 3,638억원의 사회경제적 비용이 유발되는 것으로 나타났다. 이는 다시 말해서, 적정한 정책개입을 통해 과체중 청소년 또는 비만 청소년을 감소시킬 수 있다면 본 연구에서 추계한 1조 3,638억원의 사회경제적 손실 중 상당부분을 절감할 수 있는 효과를 달성할 수 있다는 것이다. 우리나라의 경우 청소년 비만과 이환율, 조기사망에 대한 영향에 관한 기초자료를 구축하고 그 영향을 밝혀 예방과 관리에 활용되기 위한 연구가 필요할 것으로 여겨진다.;Adolescent obesity, the period between 12 and 18 years of age, is expected to increase the rates of many chronic diseases. Thus, adolescent obesity is associated with significant economic burden on both individual and society, resulting in considerable healthcare costs and loss of productivity. The aim of the study is to estimate the socioeconomic cost of adolescent obesity. This study takes long-term perspective and employs a prevalence-based approach. First, we select obesity-related diseases from reviewing the existing literature, then estimate both direct and indirect costs of the selected diseases. Next, we compute population attributable fractions (PAFs). Finally, the estimated socioeconomic costs of illness related obesity are derived by multiplying the costs of obesity-related diseases by PAFs. Direct costs include medical care expenditures, caregiver's costs, and traffic costs. Indirect costs representing productivity loss due to premature death and lost workdays. The results show that the socioeconomic cost of adolescent obesity is about 1,363,800 million Won. We provide the estimates by sex: 867,783 million Won for male 496,023 million Won for female.
