This study shows that increases in long-term care applications were caused by the influence of supply characteristics, and we analyzed that applications increased due to supply characteristics. In this study, an empirical analysis was conducted to examine the effect of long-term care supply on induced demand. Linear panel analysis was used to control each regional and periodical error as endogeneity. A close relationship was observed between the supply and care applications. Over the years, suppliers and applications increased together and every year suppliers led the increase in applications. Our linear panel analysis found that the effect on the application varied depending on the supply characteristics. Home-based supply had a significant impact on the increase in applications. On the other hand, residential facility had no significant effect on applications. The number of social workers had the greatest influence on the increase in applications. As demand inducement occurred due to supply by region, it is necessary to strengthen the role of insurer and public to identify elderly individuals in need of care services.
본 연구는 장기요양 공급 특성이라는 제도적 측면이 인정신청의 증가에 미치는 영향을 분석하는 것을 목적으로 한다. 지금까지 급여이용 특성을 바탕으로 수요를 정의하는 공급자 유인수요(SID) 관련 연구가 활발히 이뤄졌지만, 국내외 선행연구에서는 공급자가 수요자를 발굴하는 현상에 관해서는 다루어진 바가 없었다. 이에 본 연구는 65세 이상 고령인구의 증가에 따른 인구 구조적 수요 증가 요인과는 별개로, 공급 특성에 따른 인정신청의 증가를 제도 수요의 유발로 보고, 실증분석을 시도했다. 이를 위해 지난 10년 간 전국 시군구를 분석단위로 설정하여 내생성을 통해 지역별/시점별 특성을 통제하고, 선형패널분석 방법을 활용하여 장기요양 공급 특성이 수요 유발인 인정신청에 미친 영향을 검증하였다. 분석 결과, 시군구별 공급 특성과 인정신청 간 밀접한 관계가 관찰되었다. 매년 지역별 공급 기관 총량에 따라 인정신청 건수가 동일한 양상으로 조절되었다. 선형패널분석 결과, 공급 특성에 따라 인정신청에 미치는 영향이 달랐다. 65세 인구와 별개로, 시군구별 재가 기관 공급량이 인정신청 증가에 유의미한 영향을 미쳤지만, 입소 시설은 인정신청 증가에 유의미한 영향이 관찰되지 않았다. 또한 기관 종사자 중 사회복지사 인원이 인정신청 증가에 가장 높은 영향을 미쳤다. 시군구별 장기요양 공급이 수요 발굴이 이뤄지기에, 보험자 및 공공 영역의 지역사회 사례발굴에 대한 역할을 강화해 서비스가 필요한 노인 발굴이 필요하다.
