Abstract

초록

본 연구는 1995 Survey of Consumer Finances (SCF) 데이터를 이용하여 미국의 계층별 가계소득의 구성과 분포를 비교하였다. 소득수준, 교육수준, 연령, 인종에 따른 하위 집단별로 평균 가계소득과 이를 구성하는 요소와 각 요소가 총 가계소득에서 차지하는 비중을 비교분석 함으로서 계층별 소득격차에 기여하고 있는 소득의 주된 구성요소를 파악하는 것을 연구의 목적으로 하고 있으며 더 나아가 소득의 측정방법에 따라 계층별 소득불균형의 정도가 달라 질 수 있음을 시사하고 있다. 일반적으로 가계소득을 경제적 지위의 척도로 삼을 경우 모든 측정 가능한 소득이 가계소득에 포함되지만 현실적으로 조사되는 소득은 실질적인 총소득보다는 작게 측정이 될 것이다. 또한 계층별로 주요 소득원과 분포가 다를 것이며 어떠한 소득이 포함되느냐에 따라서 조사되는 총소득은 달라질 수 있으며 경제적 지위 또한 다르게 측정될 것이다. 본 연구에서는 SCF 의 세부적인 소득 데이터를 가지고 money income 뿐만 아니라 정부 복지 정책을 통해서 지원되는 식료품 보조 쿠폰(food stamps) 등과 같은 non-money income 도 총소득에 포함시켜 한층 폭 넓은 개념으로 총소득을 측정하였다. 소득구성요소에 대하여 weighted multi-regression 을 이용하여 계층별로 소득구성요소의 상대적 중요성을 비교하였다. 연구의 결과를 살펴보면 임금, 자영업에 의한 소득, 자본소득이 상대적으로 계층 간에 불균등하게 분포되어 있었으며 이와 같은 소득은 고소득층에 집중적으로 편중되어 계층 간 소득불균형을 심화 시키고 있는 것으로 조사되었다. 반면에 사회보장이나 퇴직연금 소득은 상대적으로 계층 간에 균등하게 분포되어 있었으며 이러한 소득과 정부복지정책 프로그램을 통한 소득은 저소득층의 경우에 주요 소득원으로 나타났으며 이러한 소득을 제외하여 총소득을 측정하는 경우 저소득층의 경제적 지위가 실제보다 낮게 측정될 우려가 있음을 보여 주었다.;This paper addressed income distribution in the United States by comparing each income component among subgroups with 1995 Survey of Consumer Finances (SCF) data. The mean of household total income and its sources were compared across income quintiles and demographic sub-groups to estimate the relative importance of each income component. Both household money income and nonmoney income were included in measuring the total household income for the analysis. This paper examines what is the major income source among subgroups and shows the difference in the ratio of income compositions to total household income. It shows which income components are more significant than the others in measuring total income within groups using weighted multi-regression analysis for total income by income quintiles. The result shows that wage and salary income, self-employment income (professional practice, business, or farm) and capital income (bonds, stocks, real estate, and any other investment income) are highly unequally distributed. On the other hand, income from social security, pension, or retirement programs are more equally distributed than the other components.

본 연구는 1999년 장기이식을 전제로 뇌사를 법적으로 인정하는 ‘장기등이식에 관한법률’이 제정되어 장기이식을 위한 법적 기반은 마련되었으나, 법 제정 이후 뇌사자의 장기기증은 오히려 감소되고 있고 있는 시점에서 일반국민의 장기기증에 대한 태도와 이에 영향을 미치는 요인을 분석하여, 그 결과를 바탕으로 향후 장기기증 활성화를 위한 기초 자료를 제공하는 것을 목적으로 하였다. 본 연구는 한국보건사회연구원에서 실시된 ‘뇌사 및 장기이식에 대한 여론조사’ 자료를 이용하였으며, 본 연구의 주요 결과는 다음과 같다. 첫째, 뇌사의 개념에 대한 지식 정도, 뇌사를 사망으로 받아들이는 뇌사 수용도, 조사대상자 본인의 장기기증 의사, 조사대상자 가족의 장기기증 의사에서 성별, 연령, 결혼상태, 교육수준, 직업, 월평균 가구소득 등 인구학적·사회경제적 변수에 따른 집단간 차이를 나타내고 있었다. 둘째, 경제적 능력에 따른 장기이식수술 의사의 경우, 장기이식 수술 의사가 있음을 응답한 사람의 대부분은 장기이식 수술비용 부담 능력이 없었다. 셋째, 조사대상자 본인 및 조사대상자 가족의 장기기증 의사에 대한 로지스틱 회귀분석(logistic regression) 결과, 조사대상자 본인의 장기기증 의사에 영향을 미치는 변수로는 결혼상태, 가족의 장기기증 의사, 사후 사체훼손에 대한 거부감, 장기이식술에 대한 인식으로 나타났고, 조사대상자 가족이 뇌사상태일 경우 가족의 장기기증 의사에 영향을 미치는 요인으로는 가구소득, 뇌사에 대한 지식, 뇌사 수용도, 장기이식술의 필요성, 본인의 장기기증 의사로 나타났다. 우리나라는 뇌사인정에 대한 공감대가 형성되지 않고 있고, 전통적인 유교사상의 영향, 사체훼손에 대한 두려움, 가족간의 장기기증에 대해서는 관대하나 타인에 대한 장기기증에 소극적인 가족 중심적인 사고방식 등 우리나라 고유의 문화적·윤리적 영향을 많이 받고 있는 것으로 생각된다. 때문에 우리나라에서 장기기증을 활성화시키기 위해서는 장기기증에 대한 일반인들의 이해 부족을 줄여 사회적으로 공감대를 형성하기 위한 노력을 해나가야 할 것이며, 특히 우리나라의 고유의 문화적·윤리적 환경 속에서 적절한 장기기증 활성화 방안을 찾아야 할 것이다.;This study aims to analyze public attitudes toward organ donation and several relevant factors in a current situation where, despite the 1999 legislation of Organ Transplantation Law, the number of brain-dead donors is decreasing. This study is based on  “A Survey on Public Opinion on Brain Death and Organ Transplantation” conducted in 2002 by the Korea Institute for Health and Social Affairs. The subject group consists of 1,016 people who have been selected through systematic sampling from a group of 4,000 adult men and women over the age 20 by trained telephone survey workers between July 8, 2002 to July 15, 2002. This study utilizes SAS for data analysis and chi-square test and logistic regression analysis to understand what effects demographic and socioeconomic factors have on peoples attitudes toward organ donation. Some of the main findings of this study are as follows. First, there were differences in people’s knowledge of the concept of brain death, acceptance of brain death, the respondent’s intention of organ donation, and the respondent's intention of family members’ organ donation between demographic and socioeconomic groups. Second, as to the respondent’s-to have an organ transplant operation, most of those who responded as having willingness to receive an organ donation were those without the ability to pay for the cost of organ transplant operation. Third, affecting variables to the respondent's intention of organ donation were marital status, respondent's intention of family members’ organ donation, traditional view of human body, awareness of organ transplantation. Affecting variables to the respondent's intention of family members' organ donation were monthly income level, knowledge of the concept of brain death, reluctance to accept brain death, awareness of organ transplantation, and the respondent's intention of organ donation. There are some cultural differences between Korea and West, which may explain the reluctance to donate organs. So, it is not enough that the system and technology of organ transplantation have been imported, a cultural and ethical background transplant ins also needed in Korea. From these results, this study draws several policy suggestions. First, as seen from the case of such developed countries as Spain and the US, there is a need for government-initiated publicity and education efforts to change people’s attitudes toward brain death and organ transplantation. Second, there is a need for institutional means to prioritize the individual’s consent to organ donation over his or her family members refusal of it. Also, the government should make effort to reduce the patient’s cost of organ transplantation and thus to achieve national equity in the distribution of organs.
