사회경제적 수준에 따라 암 관리 결과의 차이가 드러나고 있으며, 암 발생률과 생존율이 높은 계층에 집중한 암 의료이용 시스템 구축의 필요성이 제기되고 있다. 본 연구에서는 한국형 환자 네비게이터의 역할을 탐색하기 위하여 ‘Q 방법론’을 적용하여 환자와 의료전문가의 주관적 인식을 탐색하고자 하였다. Q표본은 문헌연구를 통해 1차적으로 47개의 표본을 도출하였으며, 최종 23개의 Q 진술문으로 확정하였다. P표본은 암전문가(의사, 간호사, 사회복지사)와 저소득층 암환자를 포함한 27명이며, 분석결과 한국에서 요구되는 환자 네비게이터 역할에 대한 주관적 인식 유형은 4가지로 분류 되었다. 치료 정보를 안내하고 암환자와 의료진 간 의사소통을 돕는 정보 제공자 유형(유형 1), 경제적 ? 물리적 자원을 연계하고 관련 정보를 제공해주는 실질적 자원 연결자 유형(유형2), 환자의 치료 장벽을 해소해주는 암 치료 장애 해소자 유형(유형3), 그리고 치료과정에서 환자 및 보호자의 다양한 욕구에 대해 자문을 제공하는 건강 유지 자문가(유형4)이다. 이 결과를 통해 의료정보제공과 경제적 지원 서비스 중심의 접근이 필요함을 알 수 있었으며, 이는 향후 환자 네비게이터가 한국 암 의료이용 현장에서 중점적으로 수행해야 할 역할과 과제라고 볼 수 있겠다.;Health disparities is one of the major issues for cancer health care, and the patient navigator programs are recognized to overcome barriers to care and in accessing quality care and treatment. To create culturally sensitive patient navigation programs for Korean populations, the authors asked health care professionals and cancer patients how they perceive the roles of patient navigators thru Q methodology. Initially 47 Q samples were drawn from literature reviews, and 23 Q Statements were confirmed thru staff discussion, professional consult, and pilot tests. We provided the finalized 23 Q statements to 15 health care professionals and 12 cancer patients who participated in the survey. We found that Koreans perceive patient navigators for four different roles. First, medical informants (Type 1) who help communicate between patients and health care professionals, and guide treatment related procedures. Secondly, intermediaries (Type 2) who provides necessary resources to low income and under-served patients. Thirdly, a barrier evacuator (Type 3) who help patients to meet their needs. Fourthly, health care advisor (Type 4) who provides services for family members. The study indicates various aspects for culturally sensitive navigation programs for Korean cancer patients.

전 세계적으로 암 발생률과 사망률, 암관리비용이 증가하고 있다. 우리나라 또한 사회경제적 수준에 따라 암 치명률과 사망률의 격차를 보일 뿐 아니라, 오히려 사회경제적 취약계층 일수록 암 치료를 위한 의료이용에서 소외되는 건강불평등 양상이 나타나기 시작했다. 따라서 효과적이고 효율적인 의료재원 활용의 필요성이 제기되고 있다. 외국의 경우 우리보다 앞서 암 치료 소외계층이 의료 서비스 이용 과정에서 겪게 되는 물리적?경제적 장애요소에 적절히 개입하여 의료이용 접근성을 높이기 위한 ‘환자 네비게이션 프로그램’이 개발?적용되고 있으며 지속적으로 여러 연구를 통해 프로그램의 효과가 검증되고 있다. 미국 및 캐나다에서 진행된 200개 이상의 환자 네비게이션 프로그램을 통해 사회경제적 취약계층이 암 의료이용 시 겪게 되는 경제적 어려움, 물리적으로 낮은 이용 접근성, 문화적 차이와 언어적 제약, 분절된 서비스 전달체계, 인종에 따른 차별 등을 줄이고 암 의료이용에의 접근성을 높인 결과 암 생존율이 높아진 것으로 보고되었다. 이러한 검토에 따라 우리나라 암 치료 소외계층의 의료이용 형평성 제고를 위한 환자 네비게이션 프로그램 적용 가능성 탐색을 위해서는 우선 암 환자 및 가족 등 프로그램 대상자의 요구도 조사가 이뤄져야 하며 운영 주체를 통한 원활한 프로그램의 기틀 마련, 그리고 네비게이터의 발굴이 이뤄져야 한다. 한국형 환자 네비게이션 프로그램의 성공적인 적용을 위해서는 암 환자 의료이용 실태에 대한 체계적인 조사와 네비게이터 양성을 위한 교육 프로그램이 과제로 제시되었다.;The rate of cancer and the cost of treatment for cancer have increased exponentially worldwide. Given the social costs attributed to the rising rate of cancer-related mortality and death, medical inequality became an issue, and there is a recognized need for effective medical support for cancer patients. In several countries, the Patient Navigation Program, a systematic cancer prevention and intervention program designed to be accessible to Health care disparities in need of cancer treatment, has been reported to be effective. The focus of the study is to investigate the characteristics of Patient Navigation Program and to examine the feasibility of the program in South Korea. The navigation program train paraprofessionals to provide intervention services for cancer patients who experience barriers to medical services. Presently, there are over 200 Navigation Programs in the United States and Canada. As a result, the program has been found to be effective as increasing number of low-income patients have access to medical service which has reportedly decreased the rate of cancer-related mortality. In order to examine whether such program would be as effective in South Korea, our study examines the background and characteristics of Navigation Program use. There is a need for survey for data base for cancer patient service utility and develop navigator training program. The navigation program will decrease the rate of cancer related mortality and serve for medical inequality.
