초록

인구의 고령화와 핵가족화로 가족 내 세대간부양이 감소하고 있으며, 이는 독거노인 증가와 노인자살 등 제반 사회문제를 야기하고 있다. 65세 이상 노인들을 대상으로 자살생각에 미치는 요인을 분석하기 위해 기술 통계적 분석과 다변량 로지스틱 회귀분석을 실시하였다. 종속변수는 자살생각(suicidal ideation) 유무이며, 독립변수는 인구학적요인(연령, 성, 교육, 거주지역, 배우자유무, 동거 가족 수), 경제적 요인(소득, 직업), 건강요인(신체적 건강, 정신적 건강, 주관적 건강, 기능적 능력), 건강 행위적 요인(알코올, 흡연, 운동), 의료 이용요인(외래방문, 병원입원) 등을 포함하였다. 본 연구는 국민건강영양조사 제1기[1998년], 제2기[2001년], 제3기[2005년] 자료를 이용하여 연도별 회귀분석을 실시하였다. 노인인구 중 지난 1년 간 자살생각을 경험한 비율은 2005년 30.5%, 2001년 26.2%, 1998년 33.6%이었다. 다변량 로지스틱 회귀분석 결과 주관적 건강상태가 나쁠수록, 우울증상이 높을수록, 스트레스가 심할수록, 배우자가 없을수록 노인의 자살생각이 높음을 알 수 있었다. 본 연구는 노인의 자살예방을 위해서는 정책당국자는 무엇보다도 노인들의 육체적 건강증진과 우울증 예방 및 스트레스 감소에 정책의 우선순위를 둘 것을 제안한다.;This study examined the determinants of suicidal ideation among the elderly in Korea. The subjects were national random samples who were 65 years or older from the Korea National Health and Nutrition Examination Survey in 1998, 2001, and 2005. Three Logit models were used to examine the determinants of suicidal ideation in terms of demographic characteristics, socioeconomic characteristics, health status, health-related behaviors, and healthcare utilization. The proportion of the elderly who had suicidal ideation within previous 1 year was 30.5% in 2005, and 26.2% in 2001, and 33.6% in 1998. The elderly who assessed themselves as healthier were less likely to have suicidal ideation than the elderly who did not. and the elderly with a spouse were less likely to have suicidal ideation than the elderly without a spouse. Those who suffered from hypochondria or stress were more likely to have suicidal ideation than their counterparts. This study suggests that government or policy makers should turn their attention to improving the physical health of the elderly and to preventing hypochondria or stress of the elderly in oder to decrease their suicide rate.

많은 사람들이 민간의료보험에 가입하고 있으며 이들 중 다수는 복수 가입을 하고 있으므로 복수 가입에 초점을 맞추어 가입자 특성을 분석한 연구가 필요하다. 이 연구에서는 민간의료보험의 단수 가입자와 복수 가입자의 특성을 비교 분석하고, 복수 가입에 영향을 미치는 요인을 파악하고자 하였다. 고령화연구패널조사(KLoSA)의 1차년도 자료(2006)를 이용하여 9,367명을 분석하였다. Heckman 선택 모형을 이용하여 전체 대상자 중 가입자를 대상으로 민간의료보험 복수 가입에 영향을 미치는 인구·사회 및 경제적 요인과 건강 상태 및 건강 관련 행태 요인을 분석하였다. 3,160명이 민간의료보험에 가입하고 있으며 이 중 47.8%(1,512명)가 복수 가입자였다. 여성, 저연령, 고학력자의 복수 가입 확률이 높았다. 자녀 수가 많으면 복수 가입 확률이 낮았다. 사적 이전을 받거나 가구 총자산이 많으면 복수 가입 확률이 높았다. 주관적 건강 상태가 좋은 사람, 정기적으로 운동을 하는 사람의 복수 가입 확률이 높았다. 민간의료보험의 복수 가입자가 많으며, 복수 가입에 영향을 주는 요인이 가입의 결정 요인과 유사하게 사회경제적 수준과 건강 상태라는 것은 기존 민간의료보험의 취약한 보장성을 시사한다.;Although many Koreans have purchased private health insurance (PHI), there has few study defining characteristics of those who purchase multiple PHI. This study examined the determinants of purchasing multiple PHI in Korea. The subjects were a national random sample of the aged 45 or older from the Korean Longitudinal Study of Ageing in 2006, and 9,367 samples were used for the analysis. The Heckman’s sample selection model was used to deal with non-random assignment to those who purchase PHI from the whole sample of the study. The model examined the determinants of the purchasing multiple PHI in terms of the demographic and socioeconomic characteristics, health status and health behaviors. Thirty-one hundred and sixty people purchased PHI, and 47.8% of them had multiple PHI. A female, the younger, or the educated was inclined to have multiple PHI. Individuals who have more children were less likely to have multiple PHI. Those who have private transfer or higher total household assets are more likely to purchase multiple PHI. Those who self-assessed their health status as better compared to poor conditions tended to have multiple PHI. Regular exercise was positively related to having multiple PHI. This study has important implications which examined determinants of purchasing multiple PHI to the extent that many Koreans have purchased PHI even with the enrollment in social health insurance. This study also suggests that purchasing PHI needs to be interpreted in the context of playing a role in accordance with social health insurance.
