끝나지 않은 고통: 원폭피해자 2세의 건강 수준에 대한 연구
Unending Suffering: A Study on the Health Status of the Second Generation of Atomic Bomb Survivors

Abstract

In this study, we surveyed 748 second-generation atomic bomb survivors nationwide to explore the health problems of second-generation atomic bomb survivors. In addition, by calculating the sex and age-standardized prevalence rates using data from the National Health and Nutrition Examination Survey (2019), we examined whether there was an excess health risk in the second -generation atomic bomb survivors compared to the general population. The health status of second-generation atomic bomb survivors was generally poor compared to the general population group, and this was consistently confirmed in various indicators such as self-reported morbidity, subjective health status, mental health status, and prevalence of limited activity. Their unmet medical experience was more than double that of the general population, and in particular, the unmet medical utilization due to economic reasons was higher than that of the general population. The above findings suggest that not only first-generation atomic bomb survivors but also their descendants have significant health problems, and that such health problems may not be properly addressed through appropriate care. Considering the high health anxiety of second-generation victims of the atomic bomb, support such as free health checkups and psychological counseling is required.

이 연구에서는 원폭피해자 2세들이 안고 있는 건강 측면에서의 문제를 탐색적으로 파악하고자 전국 원폭피해자 2세 748명에 대한 설문조사를 통해 2세의 건강 수준을 다각적으로 분석하였다. 또한 국민건강영양조사(2019년) 자료를 이용해 성·연령 표준화 유병비를 산출함으로써 일반 인구집단 대비 원폭피해자 2세에서 초과건강위험이 있는지 살펴보았다. 분석 결과, 원폭피해자 2세들의 건강 수준은 전반적으로 일반 인구집단에 비해 좋지 못한 것으로 나타났으며, 이는 자가보고 질환 이환 상태, 주관적 건강 수준, 정신건강 수준, 활동제한, 와병경험 등의 다양한 지표에서 일관되게 확인되었다. 한편, 원폭피해자 자녀들의 미충족 의료경험은 일반 인구집단보다 2배 이상 높았으며, 흡연율이나 음주율로 살펴본 건강위해 행태에서는 원폭피해자 2세들이 일반인구집단에 비해 유의하게 낮았다. 이상의 연구 결과는 원폭피해자 1세뿐 아니라 그 후손 역시 상당한 건강 문제를 안고 있음을 시사하며 그럼에도 그러한 문제들이 적절한 의료이용을 통해 제대로 해소되지 못하고 있을 가능성을 보여준다. 원폭피해자 2세들의 높은 건강불안을 해소할 수 있도록 건강검진, 심리상담 등의 지원이 필요하며, 나아가 원폭피해에 대한 사회적 관심과 인식개선이 필요함을 제언한다.

초록

본 연구는 한국의 비정규직 고용과 건강의 연관성을 다룬 기존 논문을 체계적으로 정리하고 향후 연구방향을 제시하고자 하였다. 이를 위해 국내외 웹 기반 학술데이터베이스를 활용해 비정규직 고용과 건강의 연관성을 검토한 문헌을 검색하여 2014년 12월 31일까지 출판된 학술논문 37편을 검토하였다. 본 연구의 결과는 다음과 같다. 첫째, 관련하여 총 37편의 논문이 출판되었으며 모두 양적 연구였고, 가장 많이 이용된 데이터는 한국노동패널(N=9)이었다. 둘째, 건강변수는 정신건강(N=17)과 자가평가건강수준(N=13)에 집중되어 있었다. 셋째, 전체 37편 중 31편의 연구에서 인구학적 혹은 사회경제적 정보를 잠재적 교란인자로 통제하여 다변량분석을 진행했다. 넷째, 전체 37편 중 35편의 연구결과에서 정규직보다 비정규직의 건강이 유의미하게 나빴음을 보여주었다. 이에 기반하여, 향후 연구는 다음과 같은 고려가 필요하다. 비정규직을 정의하고 범주화하는 구체적, 현실적 기준이 제시되어야 한다. 행정데이터나 의료기록 등의 자료를 활용하여 보다 다양한 건강관련 변수를 고려하고, 고용형태와 건강과의 연관성 분석에서 교란인자와 매개인자를 구분해서 분석해야 한다. 나아가, 고용형태와 건강에 대한 질적 연구나 정책적 개입 연구, 성별, 종사산업별, 연령별 차이에 주목한 연구가 필요하다.;The goal of this study is to systematically review the academic literature on the association between non-standard employment and health in South Korea. We searched online academic databases to find relevant papers in domestic and international journal by 31 December 2014 and this study reviewed a total of 37 articles. The followings are the findings of this study. First, all 37 studies have adopted quantitative methods and the most frequently used dataset was the Korean Labor & Income Panel Study (N=9). Second, the studies focused mainly on mental health (N=17) or self-rated health (N=13). Third, 31 studies controlled demographic or socio-economic indicators as potential confounders. Fourth, 35 studies reported that non-standard workers had poorer health compared to standard workers. This study suggests recommendations for future research. First, it is necessary to define non-standard employment with concrete criteria, reflecting labor market conditions in Korean society. Second, various health outcome should be examined using administrative data and medical records. Third, it is important to take account of relevant confounders in the data analysis. Finally, future research needs to consider how the health effect of non-standard employment could differ by worker’s gender, age, and industry division.
