초록

본 연구는 영국 가족수발자들의 시설이용의향을 심층적으로 이해하는데 그 목적을 두고 있다. 따라서 수발자들의 의향에 어떠한 요인들이 영향을 미치는지 살펴보고자 하였다. 본 연구는 수발자들의 의향을 심층적으로 이해하기 위해 질적 연구방법을 사용하였다. 인터뷰 대상은 14명의 영국 가족수발자들로 75세 이상 중등증 이상의 치매진단을 받은 노인을 수발하고 있는 경우로 한정하였다. 인터뷰 자료는 반복적 비교분석법을 사용하여 분석하였다. 연구결과 수발자의 시설의향 태도는 ‘불가형’, ‘유보형’, ‘가능형’으로 구분되었다. ‘불가형’의 결정적 요인은 가족주의와 시설에 대한 불신이었다. ‘유보형’은 충분한 재가서비스와 경제적 요인이 주요 요인으로 나타났다. ‘가능형’은 약한 가족주의와 치매노인과 수발자의 나쁜 건강상태, 동거하지 않는 경우가 결정적인 요인이었다. 대부분의 영국 가족수발자들은 국가가 권리로써 장기요양에 대한 보호서비스를 제공해야 한다고 인식하고 있었다. 그럼에도 불구하고 여전히 가족수발을 지속하고자 했다. 본 논문은 수발의향을 결정함에 있어 개인주의가 가족주의와 대립되는 않는다는 사실을 발견하였다.;This study aims to provide deeper understanding of family caregivers’ attitudes towards utilization of residential care in the UK. Therefore, this study purposes to explores determinants of family caregivers’ attitudes. This study uses a qualitative method to provide a deeper understanding of the attitudes. This study interviewed 14 family caregivers in Edinburgh in the UK. The caregivers are family members who care for over 75years old, diagnosed with moderate or severe dementia by a GP. Interview data analyzed according to the ‘constant comparison method’ based on the grounded theory. As a result, the attitudes divided into three different groups of ‘never’, ‘reserve’ and ‘possible’. The influential factors of a group of ‘never’ were strong familism and distrust of institutional care. The ‘reserve’ group said that they postponed using residential care due to enough community care at home and economic burden. The ‘possible’ group has determinants which are week familism, bad health of caregiver and older people with dementia, and lives apart from older people. Most of caregivers in this study believe that the state should provides free long-term care for older people as their right. Nevertheless, they want to continue to care their relatives. This study found the fact that the individualism is not contrary notion of the familism. It means that familism and individualism is thus can complement each other. This study has based on small groups of people, thus it is difficult to generalize. However, it tries to provide in-deepth understanding of caregivers. It also shows the new perspective on the attitudes of caregivers in the Western society.

본 연구는 상이한 문화와 제도가 치매노인 가족수발에 미치는 영향을 분석하고자 하였다. 이를 위해 한국의 수원시와 스코틀랜드의 에딘버러시를 중심으로 문화와 제도의 차이가 가족수발자의 수발동기, 수발형태, 수발부담 등에 어떠한 영향을 미치는지 살펴 보았다. 본 연구는 연구물음에 답하기 위해 에딘버러시와 수원시의 75세 이상 중등도 이상의 치매노인을 6개월 이상 수발하고 있는 가족수발자를 각각 14명씩 심층인터뷰하였다. 연구 결과 한국과 스코틀랜드는 수발자의 수발동기와 일상생활 보호의 측면에서 비슷한 것으로 나타났다. 그러나 경제적 보호, 인식적 보호, 가족 수발원조, 주거 상태, 수발부담 등의 경우 두 나라의 수발자들 간에 차이가 나타났다. 이 차이는 어디에서 비롯되는가. 기존 연구에서는 이를 유교주의 문화로 해석해왔다. 그러나 본 연구는 그것이 탈가족화된 제도주의적 사회복지에 기인한 것으로 본다. 스코틀랜드의 수발자들은 또한 수발에 적극적이고 조직화된 가족의 수발원조로 가족 내에서 치매노인수발을 지속적으로 해 나가고 있었다. 따라서 효는 단순히 유교주의 문화에 국한 된 것만은 아니다. 이것은 동서고금할 것없이 모든 인간사회에 나타나는 현상이라고 본다. 즉 차이는 문화가 아니라 제도에서 비롯되고 있었다. 이런 점에서 진정한 효를 실천하기 위한 제도적 토대, 즉 탈가족화 된 사회복지제도를 마련하는 것이 중요하다.;The objective of this study is to explore differences of family care between Scotland and Korea focusing on social welfare institutions. The majority of previous studies have focused on cultural aspects of family care such as Confucianism and familialism in East Asia. However, this study focuses on institutional differences in two countries in the area of family care for older people with dementia. This study had a semi-structured interview with fourteen family carers in Suwon and Edinburgh to better understand the impact of culture and institution. There were similarities and differences in the family care. With regard to similarities, type, responsibility and motivation of caregiving were very similar in both areas. In fact, this illustrates that the motivation of Koreans may not necessarily be stronger filial piety than that of Scottish people. On the other hand, differences were found in living arrangement, financial care, cognition care, care burden and family networks. The result of this study shows that the institutional differences create different attitudes towards caring for older people with dementia.
