Abstract

초록

본 연구는 국내외 문헌 및 관련 통계 등을 이용하여 우리나라 결핵 발생과 사망, 그리고 국가결핵관리 현황과 문제점을 분석하여 향후 정책 방향을 제시하였다. 연구결과, 우리나라의 결핵 발생과 사망은 감소되고 있는 추세이나, 2015년 결핵 발생률과 사망률은 각각 인구 10만 명당 80명 및 5.1명으로 OECD 국가 중에서 가장 높은 수준이다. 우리나라 결핵발생과 사망에 있어서 그간 신속한 결핵 진단과 치료율 제고를 통한 환자관리 부분이 많은 기여를 하였던 것으로 판단되며, 결핵 발생의 40%와 결핵 사망의 78%를 차지하는 노인에 대한 결핵관리와 외국인, 노숙인 등 취약계층에 대한 결핵관리는 미흡한 실정이다. 한편, 지난 10년 전에 비해 10대 및 20대의 결핵 발생률이 급격히 감소하였는데, 이는 학교에서의 결핵 접촉자조사가 기여한 바가 큰 것으로 판단된다. 향후 결핵 발생과 사망을 낮추기 위해 제2기 결핵관리종합계획(2018~2022)을 충실히 이행해 나가면서 우선순위에 따라 관련 예산 투입을 확대해 나가야 할 것이다. 먼저, 결핵 환자관리에 우선순위에 두고 결핵 조기 진단과 치료율 제고와 더불어, 비순응환자 관리나 다제내성 결핵환자 관리 강화, 그리고 그간 미진했던 노인, 외국인, 취약계층에 대한 체계적인 결핵관리 대책을 마련할 필요가 있다. 둘째, 집단시설별로 최적화된 접촉자조사 체계를 갖추고, 중앙정부·지자체·의료기관간 협력 및 역할을 강화하며, 일상접촉자보다 밀접접촉자에 우선순위를 두고 검진율과 치료율을 높이기 위한 노력을 기울일 필요가 있다. 셋째, ‘2주 이상 기침하면 결핵검진’ 등 핵심 메시지 전달 및 홍보를 강화하고, 의료인 등 고위험군을 대상으로 잠복결핵감염 검진 및 치료를 실시하는 등 결핵 예방을 강화할 필요가 있다.

수학적 모델링을 활용한 북한의 결핵 환자 발견율 및 질병부담 추계
Estimation of North Korea’s Tuberculosis Case Detection Rate and Disease Burden: Using Mathematical Modelling

Abstract

To identify why tuberculosis still remains an unresolved issue in North Korea, this study focuses on the importance of case detection. The objectives of this research are to estimate tuberculosis case detection rate of North Korea by using mathematical modelling as well as empirical data and to examine the effect of case detection rate on the disease burden in the future. Dynamic transmission model was developed and the parameters, such as case detection rate and effective contact rate, were estimated based on model fitting to notified case data of North Korea. The model fitting was conducted as follows: first, the scenarios were developed according to the proportion of latent tuberculosis patients in 2000 and time dependent function of case detection rate, including constant, linear, and non-linear function; second, the candidate scenarios were selected depending on whether the predefined calibration target was satisfied; third, the best goodness-of-fit scenario was chosen as the final model. The result indicates that, in 2018, the case detection rate is estimated at 0.64 per year and the proportion of undetected patients in terms of new patients at 35%, and in terms of total patients at 70%. To reduce the basic reproduction number below the threshold of 1, the case detection rate should be a minimum of 1.38 per year. Assuming the case detection rate from year 2019 to 2070 is maintained at the 2018 level, the projected 2070 prevalence would be 470 per 100,000 population; 2070 prevalence is predicted to increase to 694, decrease to 322, and reduce to 71 per 100,000 population, provided that the case detection rate is 0.50, 0.80, 2.00 per year respectively. In conclusion, this study shows that the improvement of case detection is required to tackle tuberculosis in North Korea. In order to increase case detection rate, it is necessary to develop alternatives leading to improved speed on diagnosis as well as the expansion of diagnosis subjects.

이 연구는 오랫동안 풀리지 않았던 북한 결핵 문제를 이해하기 위해 환자 발견의 중요성에 주목했으며, 수학적 모형을 활용해 결핵 환자 발견율을 추정하고 향후 환자 발견율이 변화될 경우 기대할 수 있는 질병부담 증감 효과를 확인하는 것을 목적으로 한다. 결핵의 수학적 모형은 상미분 방정식을 활용해 동적전염모형으로 설계했으며, 북한의 신고 결핵 감염자 수를 모형에 적합해 환자 발견율과 전염률을 추정했다. 모형 적합은 초기 잠복 결핵 감염자 비율과 시간에 따른 환자 발견율 함수 가정(상수, 선형, 비선형)에 따라 시나리오를 세분화하고, 모형 적합 기준을 충족하는 후보 시나리오 가운데 모형 적합도가 가장 높다고 판단되는 시나리오를 최종 선정했다. 분석 결과 2018년 결핵 신규 감염자의 35%, 총 감염자의 70%가 발견되지 못하고 누락된 상태인 것으로 추정되며, 기초감염재생산수를 역치인 1이하로 낮추기 위해서는 2018년 0.64/년으로 추정된 환자 발견율을 최소 1.38/년 수준으로 높여야 하는 것으로 나타났다. 2019-2070년 환자 발견율이 2018년과 동일한 0.64/년으로 지속될 경우 2070년 결핵 유병률은 470/10만 명, 환자 발견율이 0.50/년, 0.80/년, 2.00/년으로 변화될 경우에는 결핵 유병률이 각각 694/10만 명, 322/10만 명, 71/10만 명이 될 것으로 추산되었다. 결론적으로, 향후 북한의 결핵 질병부담을 큰 폭으로 낮추기 위해서는 ‘진단 대상 확대 및 진단 기간 단축’을 목표로 하는 다양한 환자 발견율 증진 방안들이 개발되어야 할 것이다.
