The current government is promoting the introduction of the Personal Budgets Scheme for disabled people as a national task. Individual needs assessment and the subsequent development of self-directed support plans are essential elements of Personal Budgets. However, the current official assessment and support planning elements are inadequate to realize these objectives. This study aims to provide tools applicable to the development of individualized support plans to achieve the principles of Personal Budgets. To do so, we examined the tools used in determining eligibility for public disability support services in Korea and compared them with tools employed in countries that have implemented the Personal Budget Schemes. Furthermore, we presented a case example using the ICF framework, which can encompass needs assessment, support plan development, and service support monitoring. This showed that the framework of case studies could be represented as an evaluation table to quantify interdisciplinary interventions, changes in case individuals, and the establishment of local community networks. As future research tasks, we proposed the development of elements and procedures that can enhance self-direction and the objectification of assessment tools to ensure fair resource allocation.
현 정부는 장애인 개인예산제를 국정과제로 추진하여 제6차 장애인정책종합계획에 따라 2023년 모의적용을 추진하였고, 2024~2025년 시범사업, 2026년 본사업 도입을 앞두고 있다. 개인별 욕구 평가와 자기주도적 지원계획 수립은 개인예산제의 핵심 요소 중 하나이나, 현재 이를 실현하기 위한 평가와 수립 도구에 관한 준비는 미흡한 편이다. 이 연구의 목적은 개인예산제가 취지에 맞게 운영될 수 있도록 지원계획 수립에 적용할 수 있는 도구를 제안하는 것이다. 이를 위해 현재 국내 공적 장애인지원서비스의 수급권 판정과 지원계획 수립에 활용하는 도구를 살펴보고, 영미권의 개인예산제 시행 국가에서 활용하는 평가 및 지원계획 수립 도구와 비교하여 함의를 도출하였다. 또한 욕구 평가와 지원계획 수립, 서비스 지원의 모니터링까지 포괄할 수 있는 틀로 ICF 연구 지사(research branch)에서 발표한 사례 연구의 틀을 수정, 가상의 사례에 적용하여 다학제적 개입, 사례자 변화의 계량화, 지역사회 네트워크의 구축을 평가표로 나타낼 수 있음을 확인하였다. 향후 연구를 위한 과제로 이용자 주도성을 강화할 수 있는 요소와 절차의 개발과 공정한 자원 할당을 위한 평가 도구의 객관화를 제안하였다.
본 연구는 ICF 2단계 분류 항목을 한국 장애인복지법에 따른 15개 장애유형에 적용하여 관련항목을 탐색한 것이다. 건강관련 구성요소(신체기능, 신체구조, 활동과 참여, 및 환경요인)에 해당하는 각 항목이 국내 등록 장애인의 건강상태를 어떻게 설명하고 있는지 확인해 보는 것을 그 목적으로 한다. 이를 위해 해당 분야의 전문가와 장애당사자들에게 관련항목을 묻는 델파이 연구를 진행하였다. 신체기능과 구조의 경우, 장애유형에 따라 델파이 연구가 방법적으로 적절하지 않을 경우 자문위원의 자문을 활용하기도 했다. 패널의 수는 신체기능과 구조의 경우 50명이었고, 활동과 참여 및 환경요인의 경우는 66명이었다. 연구결과 15개 장애유형에 해당하는 관련 항목은 장애의 특성에 따라 다양하게 나타났고, 특히 뇌병변 장애인과 발달장애인, 정신장애인의 경우 해당항목이 매우 포괄적이었다. 본 연구는 한국 장애인의 건강관련정보를 얻기 위한 통합적이고 학제 간 접근을 시도했다는 의미가 있다.;This study is to explore relevant ICF 2 level codes for persons with disabilities in Korea are based on the Registration System. The purpose of study attempts to confirm how Each code and category in the health and health-related components(body function, body structure, activity and participation, environmental factor) describes the health status of persons with disabilities registered in Korea. The Delphi research is performed as study method to deprived relevant codes by 50 medical doctors on body functions and body structures, and 66 rehabilitation or social work field experts and persons with disability themselves on activities and participations and environmental factors. According to disability types and impairment character, the experts’ advisory is added in making decision of relevant codes. The results are as followed. The relevant ICF 2 level codes take different distribution in components in accordance with 15 disability types. Especially, persons with Brain lesion, Autism, Mental disabilities have relevance to 2 level codes comprehensively. This study have the implications of integrated and interdisciplinary approach to achieve informations about health related states.
