청소년 비만의 사회경제적 비용

초록

청소년 비만은 성인병의 원인이 될 수 있으며, 생리적 기능을 저하시키고, 정서적 스트레스를 주는 등 개인의 신체적·정서적 건강에 부정적인 영향을 미치게 된다. 본 연구에서는 12~18세의 청소년 비만을 예방하고 조기진단 및 치료의 중요성에 대한 근거를 마련하기 위해 청소년의 과체중 및 비만으로 발생하게 되는 사회경제적 비용을 추계하였다. 다만, 청소년이 비만하더라도 질환이환율이 낮으며, 대사와 내분비 이상과 관련한 질환이 임상적으로 발현하기까지는 장기간의 시간이 소요되므로, 청소년의 과체중 및 비만이 장기적으로 성인에 이르기까지 발생하게 되는 사회경제적 비용을 중심으로 분석하고자 하였다. 비만의 사회경제적 비용을 도출하기 위해서 비용을 직접비용과 간접비용으로 구분하고, 연구 대상 집단의 비만유병률과 비만관련 질병 발생에 영향을 미치는 비만의 인구기여분(population attributable fraction: PAF)을 적용하였다. 분석결과, 비만과 관련된 질환으로 당뇨, 암, 뇌혈관질환, 고혈압성질환, 관절증 등이 있었으며, 이로 인해 약 1조 3,638억원의 사회경제적 비용이 유발되는 것으로 나타났다. 이는 다시 말해서, 적정한 정책개입을 통해 과체중 청소년 또는 비만 청소년을 감소시킬 수 있다면 본 연구에서 추계한 1조 3,638억원의 사회경제적 손실 중 상당부분을 절감할 수 있는 효과를 달성할 수 있다는 것이다. 우리나라의 경우 청소년 비만과 이환율, 조기사망에 대한 영향에 관한 기초자료를 구축하고 그 영향을 밝혀 예방과 관리에 활용되기 위한 연구가 필요할 것으로 여겨진다.;Adolescent obesity, the period between 12 and 18 years of age, is expected to increase the rates of many chronic diseases. Thus, adolescent obesity is associated with significant economic burden on both individual and society, resulting in considerable healthcare costs and loss of productivity. The aim of the study is to estimate the socioeconomic cost of adolescent obesity. This study takes long-term perspective and employs a prevalence-based approach. First, we select obesity-related diseases from reviewing the existing literature, then estimate both direct and indirect costs of the selected diseases. Next, we compute population attributable fractions (PAFs). Finally, the estimated socioeconomic costs of illness related obesity are derived by multiplying the costs of obesity-related diseases by PAFs. Direct costs include medical care expenditures, caregiver's costs, and traffic costs. Indirect costs representing productivity loss due to premature death and lost workdays. The results show that the socioeconomic cost of adolescent obesity is about 1,363,800 million Won. We provide the estimates by sex: 867,783 million Won for male 496,023 million Won for female.

본 연구는 한국의 복지현황 및 발전 수준을 보여줄 수 있는 복지지표체계를 개발하고자 하는 목적에서 출발한다. 기존의 단편적이고 비이론적인 복지지표들을 넘어 체계적이고 견고한 이론적 토대를 가진 복지지표체계를 개발함으로써, 현재의 복지 상황을 이해하고 복지발전 수준을 통합적으로 평가할 수 있는 준거틀을 제시하는 것이 본 연구의 주요한 목적이다. 이를 위해 본 연구에서는 OECD의 사회지표 분석틀을 기반으로 하되 한국 상황에의 적합성을 고려, 수정한 새로운 복지지표 분석틀을 제안하였다. 한국의 복지지표 분석틀은 3가지 분류기준을 포함하는데, 첫째, ‘PSR(Pressure - State - Response)’모형에 따른 사회적 맥락(social context), 사회적 상황(social status), 복지노력(welfare efforts)이다. 둘째, 사회정책의 목표기준으로 OECD의 사회지표 분석틀에서 제안한 자립, 형평성, 건강, 사회응집에 더하여 지속가능성을 포함, 총 5가지 정책목표를 제시하였다. 셋째, 생애주기별 기준을 사용하여 아동·청소년기(0-24세), 성인기(25-64세), 그리고 노년기(65세 이상) 세 가지 생애주기로 구분하였다. 기존의 복지지표를 분류하는 기준들이 일정한 기준이나 이론적 근거 없이 인구집단, 제도 혹은 사업, 정책목표 등의 기준들이 혼재되어 있는 상황에서, 논리적이고 이론적인 근거를 가진 복지지표체계를 구성하고자 시도한 이 연구는 한국의 복지지표체계 구축에 중요한 방향성을 제시할 뿐 아니라 추후 OECD 등의 사회지표체계 논의에도 기여할 것이다.;This study aims to develop a new conceptual framework of welfare indicators in Korea. By developing a system of welfare indicators which is a theoretically well-grounded as well as methodologically sound, this research aims to propose a new framework to be used to monitor and analyse the development of welfare and quality of life as well as general trends of social change in Korea. This research proposed a new system of welfare indicators on the basis of the OECD social indicator framework. This framework has three dimensions. The first dimension considers the nature of these indicators, grouping them in three areas - social context, social status, societal response - which is similar to “Pressure-State-Response(PSR)”approach. Social context indicators are crucial for understanding the broader social policy context. Social status indicators describe the social outcomes that policies try to influence. Social response indicators provide information about what society is doing to affect social status indicators. In our framework, we used the ‘welfare effort’instead of ‘societal response’to put its emphasis on the government’s efforts. The second dimension groups indicators according to the broad objectives of social policy - self sufficiency, equity, health status, social cohesion. In our framework, we added sustainability to policy objectives list. The third dimension groups indicators according to life-cycle - children under 24 years old, adults from 25 to 64 years old, the aged over 65 years old. This framework for welfare indicators is systemic and coherent, so it will be helpful for the development of welfare statistics.
