초록

본 연구는 한국에서 차별경험과 건강의 관계에 대한 기존 연구들을 체계적으로 분석하고 향후 필요한 연구방향을 제시한다. 이를 위해 국내외 웹 기반 학술데이터베이스를 활용하여 2014년 8월 31일까지 출판된, 차별경험과 건강과의 관계를 연구한 학술논문 52편을 분석하였다. 체계적 문헌분석 결과는 다음과 같다. 첫째, 관련 연구들은 2003년에 시작되어 2010년 이후 활발히 진행되었다. 둘째, 연구방법에서 양적 연구(N=46)가 가장 많았고 질적 연구(N=5)는 상대적으로 적었다. 양적 연구는 1편을 제외하고 모두 단면연구(N=45)였다. 셋째, 한국의 차별과 건강에 대한 연구들은 정신건강(N=42)에 집중되어 있으며, 넷째, 인구집단 별로 살펴보았을 때 이주민(N=21), 노인(N=12), 장애인(N=7), 그리고 노동자(N=7) 순으로 연구가 이루어졌다. 체계적인 문헌고찰에 기반하여, 향후 한국에서는 다음과 같은 연구가 필요하다. 첫째, 한국사회에 존재하는 학력, 계급, 고용형태 등에 따른 차별에 대한 심층적인 연구가 필요하다. 둘째, 차별측정도구의 정확성과 타당성에 대한 비판적 논의가 요청된다. 셋째, 정신건강뿐만 아니라 다양한 건강관련변수를 고려해야 한다. 넷째, 종단데이터를 이용한 양적 연구가 요구된다. 마지막으로, 다중차별에 대한 보다 많은 연구가 필요하다.;Although the association between perceived discrimination and health has been a main issue in public health, there has been little systematic discussion in a South Korean context. This study sought to systematically review the existing literature on discrimination and health in South Korea and present several suggestions for future research. Selected through online academic databases in English and Korean, 52 empirical journal articles are reviewed for this study. A systemic review identifies four main characteristics among the studies on perceived discrimination and health in South Korea. First, the studies have been published since 2003 and dramatically increased since 2010. Second, most studies have utilized quantitative methods (N=46) with most of them analyzed cross-sectional data (N=45), while five qualitative methods (N=5). Third, the majority of studies focused on mental health (N=42). Forth, the studies dealt with a variety of population groups including foreigners/immigrants (N=21), the elderly (N=12), and the disabled (N=7). Our critical review suggests several recommendations for future research. First, the overlooked forms of discrimination, for instance, based on education, employment status, and appearance, need to be examined in South Korea. Second, a more critical approach toward perceived discrimination scales is required for future research. Third, researchers need to explore other health outcomes beyond mental health outcomes. Forth, more studies with longitudinal dataset are required given that there was only one cohort study. Fifth, future studies should explore an intersectional nature of discrimination and its impact on health.

본 연구는 한국의 비정규직 고용과 건강의 연관성을 다룬 기존 논문을 체계적으로 정리하고 향후 연구방향을 제시하고자 하였다. 이를 위해 국내외 웹 기반 학술데이터베이스를 활용해 비정규직 고용과 건강의 연관성을 검토한 문헌을 검색하여 2014년 12월 31일까지 출판된 학술논문 37편을 검토하였다. 본 연구의 결과는 다음과 같다. 첫째, 관련하여 총 37편의 논문이 출판되었으며 모두 양적 연구였고, 가장 많이 이용된 데이터는 한국노동패널(N=9)이었다. 둘째, 건강변수는 정신건강(N=17)과 자가평가건강수준(N=13)에 집중되어 있었다. 셋째, 전체 37편 중 31편의 연구에서 인구학적 혹은 사회경제적 정보를 잠재적 교란인자로 통제하여 다변량분석을 진행했다. 넷째, 전체 37편 중 35편의 연구결과에서 정규직보다 비정규직의 건강이 유의미하게 나빴음을 보여주었다. 이에 기반하여, 향후 연구는 다음과 같은 고려가 필요하다. 비정규직을 정의하고 범주화하는 구체적, 현실적 기준이 제시되어야 한다. 행정데이터나 의료기록 등의 자료를 활용하여 보다 다양한 건강관련 변수를 고려하고, 고용형태와 건강과의 연관성 분석에서 교란인자와 매개인자를 구분해서 분석해야 한다. 나아가, 고용형태와 건강에 대한 질적 연구나 정책적 개입 연구, 성별, 종사산업별, 연령별 차이에 주목한 연구가 필요하다.;The goal of this study is to systematically review the academic literature on the association between non-standard employment and health in South Korea. We searched online academic databases to find relevant papers in domestic and international journal by 31 December 2014 and this study reviewed a total of 37 articles. The followings are the findings of this study. First, all 37 studies have adopted quantitative methods and the most frequently used dataset was the Korean Labor & Income Panel Study (N=9). Second, the studies focused mainly on mental health (N=17) or self-rated health (N=13). Third, 31 studies controlled demographic or socio-economic indicators as potential confounders. Fourth, 35 studies reported that non-standard workers had poorer health compared to standard workers. This study suggests recommendations for future research. First, it is necessary to define non-standard employment with concrete criteria, reflecting labor market conditions in Korean society. Second, various health outcome should be examined using administrative data and medical records. Third, it is important to take account of relevant confounders in the data analysis. Finally, future research needs to consider how the health effect of non-standard employment could differ by worker’s gender, age, and industry division.
