초록

본 연구는 주요 건강지표인 평균수명, 사망률, 영아사망률 및 모성사망비에 대하여 통일 전후의 동서독 간, 그리고 남북한 간 비교 분석함으로써 향후 남북한 건강수준의 차이를 전망해 보고자 하였다. 이를 위해 우리나라 통계청과 독일 통계관련 기관, 유엔 등의 원시자료와 국내외 보고서 및 논문 등의 이차자료를 활용하여 분석하였다. 남북한 평균수명의 차이(남 12세, 여자 11세)는 통일 전 동서독 평균수명의 차이(남자 2.4세, 여자 2.9세)보다 훨씬 높은 수준이다. 통일 후 10여 년 동안 여자의 동서독 평균수명의 차이는 거의 해소된 반면, 통일 후 동독 남자의 사망률은 증가하여 동독 남자의 평균수명이 감소하는 등 남자의 동서독 평균수명의 격차는 여전히 해소되지 못하였다. 북한의 영아사망률과 모성사망비는 남한에 비해 각각 8.7배, 8.8배 높은 수준인데, 동서독 간 영아사망률 및 모성사망비의 격차가 통일 이후 빠르게 극복된 점은 우리에게 시사하는 바가 있다 하겠다. 결론적으로, 북한의 건강수준을 향상시키고 남북한의 건강수준의 격차를 줄이는데 필요한 시간과 비용은 통일 독일의 건강수준의 격차를 줄이는데 들어간 시간과 비용보다 훨씬 더 클 것이다. 이에 통일 전부터 남북한 간 건강수준의 차이를 감소시킬 수 있는 전략을 마련하여 국내외적인 노력을 기울여야 할 것이다.;This study analyzed data from Statistics Korea, the German Federal Institute for Population Research, the Human Mortality Database, and the United Nations (UN) to examine changes in life expectancy at birth, mortality, infant mortality rate, and maternal mortality ratio between South and North Korea and between West and East Germany. The results were as follows. First, the difference in life expectancy at birth between South and North Korea (men 12 years, women 11 years) was higher than that between West and East Germany before unification (men 2.4 years, women 2.9 years). There is no difference in life expectancy among women in West and East Germany. Second, the mortality rate of North Korea was 60% higher than that of South Korea. After unification, the mortality rate of men in East Germany increased suddenly and life expectancy among East German men decreased over the subsequent few years. Third, compared to South Korea, the North Korean infant mortality rate and maternal mortality ratio was 8.7 and 8.8 times higher, respectively. The pre-unification differences in infant mortality rate and maternal mortality ratio between West and East Germany reduced shortly after reunification. The findings imply that the cost involved and the time needed to eliminate health inequality between South and North Korean individuals after unification may both be higher than that required to eliminate differences between individuals from East and West Germany. Thus, efforts are needed to reduce the differences in health between individuals from North and South Korea before unification.

본 연구에서는 국내외 문헌을 이용하여 남북한의 영아 및 아동 사망률과 관련 요인을 비교 분석하고, 연구자가 구축한 30년간(1985~2014)의 북한의 영아 및 아동 사망률 관련 데이터베이스를 이용하여 상관분석 및 다중 회귀분석을 실시하였다. 연구결과, 북한의 영아 사망률은 26.4명으로 남한의 3.0명에 비해 8.8배 높았고, 북한의 아동 사망률은 33.4명으로 남한의 3.6명에 비해 9.3배나 높았다. 북한의 국민총소득, 곡물생산량, 예방접종률, 영양상태가 아동 및 영아 사망률과 높은 관련성이 있었고, 다중 회귀분석 결과에서는 곡물생산량과 예방접종률이 북한의 영아 및 아동 사망률에 영향을 미치는 주요 요인이었다. 또한 남한의 아동 사망의 주요 원인은 조산(33%) 및 선천성 이상(22%)이었으나, 북한의 경우는 조산(22%) 및 선천성이상(13%)과 더불어 감염성 질환(30%)이 주요 원인이었다. 특히, 북한의 신생아 사망의 주요 원인은 조산(36%), 선천성 이상(19%) 등인 반면, 신생아기 이후 아동 사망의 주요 원인은 폐렴(43%), 설사(15%) 등 감염성 질환이 절반 이상을 차지하였다. 결론적으로 북한의 영아 및 아동사망률을 낮추기 위해서는 예방접종을 우선순위에 둘 필요가 있으며, 영아 및 아동의 영양개선과 더불어 영아 및 아동 치료를 위한 원활한 의약품 조달 및 의료시설 개선과 의료서비스의 질을 제고하는 방안을 마련해야 할 것이다. 이에 따른 우선순위를 마련하여 체계적이고 장기적인 영유아 부문 대북 인도적 지원이 필요하다 하겠다.;This study analyzed data, from a data set established by the author, with variables from Statics Korea and UNICEF from the period 1985 - 2014. A correlation analysis and a multiple regression model were used as well as a literature review. The results were as follows. First, compared to South Korea, the North Korean infant mortality rate (26.4) and child mortality rate (33.4) were 8.8 and 9.3 times higher, respectively. Second, the findings showed that the gross national income, agricultural crop yield, vaccination rates, and nutrition were significantly related to both the infant and child mortality rate in North Korea. Further, multiple logistic regression analysis showed that agricultural crop yield and vaccination rates were significant factors influencing both the rates in North Korea. Third, the leading causes of death among North Korean children were infectious disease (30%), prematurity (22%), and congenital anomalies (13%) while prematurity (33%) and congenital anomalies (22%) among South Korean children. Especially, almost 60% of the causes of death from postneonatal to under 5 years old was infectious disease including pneumonia (43%) and diarrhoea (15%) while the major causes of neonatal death were prematurity (36%) and congenital anomalies (19%) in North Korea. These findings suggest that policy makers should prioritize childhood vaccination programs and should improve nutrition for infants and children. Also efforts are needed to not only provide essential drugs but also improve quality of medical care and facilities for North Korean infants and children through humanitarian aids to North Korea.
