Abstract

The purpose of this study was to examine the effects of organizational communication and team work on perception of importance and activities on patient safety in general hospital employee. The survey was performed with 450 hospital employees working in 2 hospitals in B City. The data was collected by questionnaires and were analyzed with the SPSS/Win 23.0 program. The results of the survey were as follows. The levels of organizational communication, team work, perception of importance on patient safety management and patient safety assurance activities were 3.26, 3.65, 3.74 and 4.07 respectively out of a 5-point scale. There was a statistically significant positive relationship between variables. The significant factors that influence perception of importance on patient safety management were age (>40years), occupation(administration), contact department, work experience (>10years), guidebook (have), organizational communication and team work which explained 24.2% (F=21.481, p<.001) of perception of importance on patient safety management. The significant factors that influence patient safety assurance activities were occupation (administration), contact department, work experience (3-5 years), team work which explained 22.8% (F=34.234, p<.001) of patient safety assurance activities. Therefore, it is necessary to develop and apply effective organizational communication and teamwork programs to improve patient safety assurance activities for hospital employees.

본 연구는 병원종사자가 지각하는 조직 의사소통, 팀워크, 환자안전관리 중요성 인식 및 환자안전보장활동 정도를 파악하고 환자안전관리 중요성 인식과 환자안전보장활동에 영향을 미치는 요인을 확인하기 위한 서술적 조사연구이다. B광역시 2개 종합 병원종사자 450명을 대상으로 자료를 수집하였고, 수집된 자료는 SPSS WIN 23.0을 이용하여 분석하였다. 분석결과는 다음과 같다. 병원종사자들의 조직 의사소통은 평균 3.26±0.42점(5점 만점), 팀 워크는 3.65±0.50점(5점 만점), 환자안전관리 중요성 인식은 3.74±0.50점(5점 만점), 환자안전보장활동은 4.07±0.59점(5점)이었으며, 모든 변수들 간에는 정적 상관관계가 있는 것으로 나타났다. 병원종사자의 환자안전관리 중요성 인식에 영향을 미치는 요인은 연령대(40세 이상), 직종(행정), 부서(접점부서), 10년 이상의 근무경력, 지침서 구비(있다), 조직 의사소통, 팀워크로 나타났으며 24.2% (F=21.481, p<.001)의 설명력이 있는 것으로 나타났다. 병원종사자의 환자안전보장활동에 영향을 미치는 요인은 팀워크, 직종(행정), 근무경력(3~5년), 부서(접점부서)로 22.8%(F=34.234, p<.001)의 설명력이 있는 것으로 나타났다. 따라서 병원종사자의 환자안전관리 중요성의 인식과 환자안전보장활동을 향상시키기 위해서는 직종과 부서를 고려한 효율적인 조직 의사소통과 팀워크에 대한 프로그램의 개발 및 적용이 필요하다.

본 논문의 목적은 과다경쟁에 노출되어 있는 도심형 요양병원이 환자확보를 하기 위한 전략적 관리요소를 도출함에 있다. 분석 방법으로 도심형 요양병원을 대상으로 환자가 고려하는 선택속성의 중요도와 성취도를 활용한 IP(Importance-Performance)분석을 수행하였고, 각 선택요인의 중요도와 성취도간 차이를 검증하여 핵심적 관리요소를 밝혀내고자 하였다. IP분석 수행 결과, 의사의 친절함, 의견수렴도, 행정직원들의 친절함, 내부시설 청결여부, 의사의 실력과 의술의 신뢰도, 적정한 인력확보, 안전시설관리, 노인진료에 대한 의사의 경력은 우선적인 개선항목으로 분석되었다. 반면 의사의 상세한 진료상담, 간호사의 친절함, 간병인의 친절함, 물리치료사의 친절함, 침구류의 청결여부, 시설의 청결상태, 영양식 요인은 강점항목으로 분석되었다. 또한, 이동구조의 편리함, 최신 의료장비, 편의시설 마련여부, 병원의 체계적인 재활 및 진료 프로그램, 행정절차의 간편성, 저렴한 비용, 공신력 있는 기관의 평가, 병원의 명성, 협력병원과의 유대관계에 대하여 크게 중요성을 인식하고 있지 않은 것으로 나타났다. 불필요하게 성취도가 높게 나타나고 있는 항목으로는 편리한 교통 및 주차시설, 다양한 재활시설, 짧은 입원 대기기간, 야간진료의 편의성으로 분석되었으며 이에 대한 자원배분을 제고할 필요가 있겠다. 더불어 중요도와 성취도간의 차이를 분석한 결과, 시설의 편의성에 비해 간호사 및 간병인의 친절성을 포함한 인적서비스의 질적 개선이 중요함을 제시할 수 있었다.;The present study aims to derive strategic management factors for urban geriatric hospitals exposed to excessive competition to attract patients. As for the analysis method, this study performed an IP (Importance-Performance) analysis using the importance and performance of choice factors considered by patients towards urban geriatric hospitals, and verified the differences between the importance and performance of each choice factor to identify integral management factors. According to the IP analysis, doctors’ kindness, administration staff’s kindness, doctors’ competence, reliability of medical practice, acceptance of patients’ opinions, proper staffing, maintenance of safety facilities and doctors’ career in geriatric medicine were found as the factors that should be given priority for improvement. By contrast, doctors’ scrupulous consultation and treatment, nurses’ kindness, caregivers’ kindness, physiotherapists’ kindness, overall sanitation of hospitals and choice of nourishing food were found as strengths. Also, it was found that convenience of movement structure, up-to-date medical equipment, convenient facilities, systematic rehabilitation and consultation programs in hospitals, streamlined administrative procedures, low expenses, ratings from reputable agencies, reputation of hospitals and ties with partner hospitals were not considered important. The factors of unnecessarily high performance included convenient transportation and parking facilities, diverse rehabilitation facilities, short queues for admission and convenient night-time medical treatment. Thus, allocation of resources to these factors need be reconsidered. In addition, the analysis of the difference between importance and performance suggests that the qualitative improvement in terms of personal services including the kindness of nurses and caregivers is more important than the convenience of facilities.
