노년기 우울에 관한 연구: 다층모형 적용을 통한 개인 및 지역자원 효과 검증
A Study of Depression in the Elderly by Individual and Community Effects

Abstract

The purpose of this study was to identify individual and regional level factors that affect depression among the elderly. It also verified the interaction effect between the two levels. To obtain this, multilevel analysis was conducted on 1,987 elderly people aged 65 and over in 16 cities and provinces nationwide. The main results were as follows. First, as a result of the unconditional means model test, the depression of the elderly appeared differently according to the region, which made it reasonable to apply the multilevel analysis. Second, social resources of the elderly showed differences according to the region. Third, social resources such as, trust, social cohesion, and citizen participation among elderly have a significant effect on depression of the elderly. Finally, the interaction effects between social cohesion and community poverty rate, residential welfare facilities and health care resources on the depression were verified. Based on these results, the policy implications and practical intervention plans for improving mental health of the elderly were discussed.

본 연구의 목적은 노인의 우울에 영향을 미치는 개인 요인과 지역 수준 요인을 확인하고, 두 수준 간 상호작용 효과를 검증하는 것이다. 이를 위해 개인 및 지역 수준의 다층적 자료를 구성하였다. 개인 자료는 비례층화할당을 통해 추출된 만 65세 이상 노인의 설문자료로 구성되었고, 이를 시(도)별 국가통계포털 지역수준 자료와 혼합 자료로 구성하였다. 전국 16개 시(도), 만 65세 이상 노인 1,987명을 대상으로 다층분석(Multilevel analysis)을 실시하였다. 주요 분석결과는 다음과 같다. 첫째, 기초모형 검증결과, 노인의 우울은 지역에 따라 차이가 존재하는 것으로 나타났다. 둘째, 노인의 사회자원이 지역에 따라 차이가 발생하는 것으로 나타나, 다층모형 적용은 적절한 것으로 나타났다. 셋째, 노인의 사회자원 중 신뢰, 사회응집력, 시민참여가 노인의 우울에 유의한 영향을 미치는 것으로 나타났다. 마지막으로, 사회응집력과 지역사회 빈곤율, 주거복지시설, 보건의료자원의 상호작용 효과가 검증되었다. 이러한 연구결과를 토대로 노인의 정신건강 증진을 위한 정책적, 실천적 개입 방안을 논의하였다.

초록

본 연구는 서울시 노인의 노후준비 수준이 노인의 주관적 건강에 미치는 영향을 분석함으로써 노인의 건강을 증진시키기 위한 실증적 기초자료를 제공하기 위해 수행되었다. 이에 따라 본 연구의 목적은 서울시 노인의 노후준비형태 및 지역사회 특성이 노인의 주관적 건강수준에 미치는 영향을 분석함으로써 서울시 노인의 건강수준 향상을 위한 개입방안 및 정책적 함의를 도출하는 것이다. 이를 위해 서울시복지재단의 2010서울시복지패널데이터를 활용하였으며, 서울시 25개 구, 65세 이상 남녀 노인 1,429명을 대상으로 변인 간의 관계를 분석하였다. 분석방법으로는 개인 및 지역 수준의 자료분석에 적합한 다층분석(Multi-level Analysis)을 실시하였다. 분석결과, 노후준비 수준이 높은 노인일수록, 경제활동을 할수록, 현재 앓고 있는 질환이 적을수록, 지역사회 내 보건의료자원 수가 많을수록 주관적 건강 수준이 높은 것으로 나타났다. 또한 사회참여를 통한 노후준비와 보건의료자원 수 및 지역사회 신뢰수준의 상호작용 효과가 검증되었고, 신체적 건강관리를 통한 노후준비 역시 보건의료자원 수 및 지역사회 65세 이상 노인 인구수와 상호작용 효과가 나타났다. 이러한 연구 결과를 토대로 서울시 거주 노인의 건강증진을 위한 사회복지적 개입 방안 및 정책적 함의를 제언하였다.;This study aims to provide practical information for improving the elderly’s health. For this purpose, this study analyzed effects of the preparation for old age on the Self-rated health of the elderly in Seoul. This study was analyzed relationships among variables of 1,429 cases of the elderly, 25 districts in the Seoul City Welfare Panel. The statistical analysis was performed multi-level analysis using the HLM(Hierarchical Linear and Nonlinear Modeling) 6.02 version. Multi-level analysis is fit for analysis of multidimensional data like personal and community level data. The result of the analysis of fixed effects indicate that the Self-rated health of the elderly would be greater for people who have a higher level of preparation for old age, a economical activity, a lower diseases, and a more health and medical resources. In addition, the Psycho-social preparation for aging which is a individual factor showed a significantly interactive effect on health and medical resources and Community confidence which are community factors. Health and medical resources and the number of 65 years of age or older have an interactive effect on the relationship between physical preparation for aging and self-rated health of the elderly in Seoul. Based on these results, this study suggested practical and political implications for improving health of the elderly in Seoul.
