Abstract

초록

본 연구의 목적은 린 6시그마를 통한 병원 응급실 프로세스 개선이 유효한지를 살펴 보고자 함에 있다. 현 시스템의 실제 데이터를 이용한 시뮬레이션으로 사례 연구를 수행하였다. 연구 결과 요일별 및 도착 시간대 따른 환자 수는 차이가 있었으나 총 리드타임에 대한 차이가 없는 것으로 측정되어 선행 연구들과 차이를 보였다. 총 리드타임에 영향을 주는 잠재적 원인 변수들 중, 실제진료 프로세스가 가장 높은 상관관계를 보였다. 이러한 실제진료 프로세스는 실제진료시간과 호출대기시간으로 분류되었으며, 실제진료시간은 실제진료과수, 응급관리료, 중증도, 연령대 변수에 따라 시간의 차이를 보였다. 대기시간 중에서 입/퇴원 결정-퇴실시간은 진료결과에 따른 시간의 차이가 측정되었다. 이를 바탕으로 총 6가지의 대안을 설정하였다. 이러한 6가지 대안의 시뮬레이션 수행 결과 대안 Ⅳ은 현재 총 리드타임 대비 22.6분이 감소하였으며, 총 대기시간은 22.2분이 감소하는 시뮬레이션 측정값을 산출하였다. 그리고 프로세스의 효율을 측정하는 PCE에서는 78.9%에서 93.4%로 14.5%가 증가하는 결과가 측정되었다. 이상의 결과들을 종합한 다음과 같은 결론은 얻었다. 지방의 H대학병원 응급의료센터를 신속하고 효율적인 운영을 위하여 실제진료과수에 대한 통제와 입/퇴원 결정-퇴실 대기시간의 제거 및 통합을 통하여 응급의료센터의 프로세스 효율을 증진시키고 효과적인 병원경영의 대안을 제시하였다는 점에서 의의가 있다. 특히, 린 6시그마의 활용되어지는 도구 및 통계적 지표 그리고 시뮬레이션을 통하여 병원경영 성과에 높이기 위한 전략 수행에서 따른 리스크 감소를 줄일 것으로 보인다.;The purpose of this research was to examine the improvement in hospital emergency process using Lean Six Sigma. Case studies were performed with a simulation using real data in the current system. The result showed that the number of patients differed depending on the day of the week and the time of day, while suggesting, unlike previous studies, no difference in total lead time. Among potential variables affecting total lead time, actual treatment process was found to be most highly correlated with it. The actual medical treatment process was classified into the actual medical treatment hour and the call waiting hour. The actual medical treatment hour appeared to be different according to variables such as the number of the actual medical department, the fee for the emergent treatment, and age. In addition, out of the waiting hour, the lead time for the decision of Admission and Discharge-the check-out appeared to be different according to the result of medical treatment. Base on these findings, total 6 alternatives were set. And as a result of simulation using these alternatives, alternative Ⅳ showed the decrease of 22.6 minutes compared to the current total lead time, whereas total waiting hour appeared to be decreased by 22.2 minutes too. Furthermore, the efficiency of the process measured by PCE appeared to be increased by 14.5% from 78.9% to 93.4%. Conclusions obtained from these findings were as follows. This research was meaningful in that for the fast and efficient management of the emergency medical center in H university hospital in province, the number of the actual medical department was controlled at the same time the lead time for the decision of admission and discharge and the check-out was eliminated. And also the process efficiency of the emergency medical center was improved through the integration as well as the alternative for the efficient hospital management was suggested.
