남자의 흡연율이 특히 세계적으로 높은 수준에 있는 우리나라에서는 흡연이 건강에 미치는 영향은 매우 크다. 흡연율은 건강행태 중에서도 사회계층별 격차가 큰 대표적인 건강행태이다. 본 연구는 1998년, 2001년, 2005년 국민건강영양조사를 비교하여 흡연율의 교육수준별, 직업유형별로 연령표준화 흡연율의 차이와 변화추이를 연령표준화 흡연율 및 로지스틱 회귀분석과 상대불평등지수(RII)를 통하여 분석하여 사회경제적 계층별 흡연수준의 시계열적 변화양상을 분석하고자 하였다. 3개 연도에서 모두 25∼64세에서는 교육수준과 소득수준이 낮을수록, 그리고 육체적 노동자에서 흡연율이 높은 양상이 뚜렷하게 나타났다. 또한 1998년, 2001년, 2005년를 비교한 결과, 25∼64세에서 남녀 모두 흡연율에서의 교육, 직업 및 소득수준 간 불평등이 심화되는 경향을 뚜렷하게 볼 수 있었다. 특히, 25∼64세 남자에서 교육수준별, 소득수준별 흡연에서의 절대적, 상대적 불평등의 크기가 커지고 있는 경향을 보였다. 상대불평등지수(RII)를 산출할 수 있는 교육수준별 불평등 양상을 보면, 25∼64세 남자의 흡연율에서 통계적으로 유의한 불평등의 심화가 제시되었다. 25∼64세 여자 및 노인에서는 사회경제적 위치에 따른 흡연율 불평등의 심화는 볼 수 없었다. 이러한 흡연율의 변화 추이와 사회경제적 계층에 따른 추이는 건강증진법 제정 이후 금연정책과 캠페인, 금연클리닉 운영, 담배가격 정책 등 담배소비와 관련된 정책적, 사회환경적 변화가 시도되고 상황에서 의미 있는 정책적 함의를 제시하고 있다. 향후 흡연 감소를 위한 금연사업이나 캠페인, 담배가격 정책은 사회계층에 따라 그 효과가 다르게 나타날 수 있다는 점을 고려하여, 흡연율이 높은 교육수준과 소득수준이 낮은 인구집단에게 보다 효과적으로 전달될 수 있도록 사회경제적 흡연율의 불평등 감소를 고려한 전략과 목표가 추구되어야 할 것이다.;Over the past several decades, smoking as one of crucial lifestyle factors has been known as the major determinants of premature and preventable mortality and morbidity. In addition, it is well documented that people of lower socioeconomic position are significantly more likely to smoke cigarettes. The purpose of this study is to examine the trends of socioeconomic differentials in smoking rates by gender and age, and socioeconomic position in Korea. Korea National Health and Nutrition Survey from 1998, 2001, 2005 was used. This study included 21,417 men and women aged 25 and over. Socioeconomic position indicators were education, occupation and household equivalent income. Age-standardized smoking rates decreased among men aged 25-64 and 65+, and among women aged 65+ between 1988 and 2005, while smoking rates among women aged 25-64 increased between 1988 and 2005. Education and income was inversely associated with smoking in both genders. Those with manual occupations had greater smoking rates than those who had non-manual labor in both genders. Based on the relative index of inequality(RII), unfavorable inequality trends toward low education were found in men aged 25-64. However, these were not found in men aged 65 and over. Continuous anti-smoking policy measures should be directed toward men whose smoking rates are still high. In addition, women's smoking pattern needs to be further investigated in Korea. This study found that the Korean government should effort to develope anti-smoking policies and programs that are attuned to SES-related smoking pattern, with a view to reducing socioeconomic differentials in smoking rates.
