초록

본 연구는 상이한 문화와 제도가 치매노인 가족수발에 미치는 영향을 분석하고자 하였다. 이를 위해 한국의 수원시와 스코틀랜드의 에딘버러시를 중심으로 문화와 제도의 차이가 가족수발자의 수발동기, 수발형태, 수발부담 등에 어떠한 영향을 미치는지 살펴 보았다. 본 연구는 연구물음에 답하기 위해 에딘버러시와 수원시의 75세 이상 중등도 이상의 치매노인을 6개월 이상 수발하고 있는 가족수발자를 각각 14명씩 심층인터뷰하였다. 연구 결과 한국과 스코틀랜드는 수발자의 수발동기와 일상생활 보호의 측면에서 비슷한 것으로 나타났다. 그러나 경제적 보호, 인식적 보호, 가족 수발원조, 주거 상태, 수발부담 등의 경우 두 나라의 수발자들 간에 차이가 나타났다. 이 차이는 어디에서 비롯되는가. 기존 연구에서는 이를 유교주의 문화로 해석해왔다. 그러나 본 연구는 그것이 탈가족화된 제도주의적 사회복지에 기인한 것으로 본다. 스코틀랜드의 수발자들은 또한 수발에 적극적이고 조직화된 가족의 수발원조로 가족 내에서 치매노인수발을 지속적으로 해 나가고 있었다. 따라서 효는 단순히 유교주의 문화에 국한 된 것만은 아니다. 이것은 동서고금할 것없이 모든 인간사회에 나타나는 현상이라고 본다. 즉 차이는 문화가 아니라 제도에서 비롯되고 있었다. 이런 점에서 진정한 효를 실천하기 위한 제도적 토대, 즉 탈가족화 된 사회복지제도를 마련하는 것이 중요하다.;The objective of this study is to explore differences of family care between Scotland and Korea focusing on social welfare institutions. The majority of previous studies have focused on cultural aspects of family care such as Confucianism and familialism in East Asia. However, this study focuses on institutional differences in two countries in the area of family care for older people with dementia. This study had a semi-structured interview with fourteen family carers in Suwon and Edinburgh to better understand the impact of culture and institution. There were similarities and differences in the family care. With regard to similarities, type, responsibility and motivation of caregiving were very similar in both areas. In fact, this illustrates that the motivation of Koreans may not necessarily be stronger filial piety than that of Scottish people. On the other hand, differences were found in living arrangement, financial care, cognition care, care burden and family networks. The result of this study shows that the institutional differences create different attitudes towards caring for older people with dementia.

본 연구는 노인요양시설에서 전문적으로 케어를 담당하는 인력의 역할을 규명하기 위해 요양시설의 주된 케어인력인 요양보호사의 역할을 살펴보고, 보다 전문성을 갖춘 케어인력에게 요구되는 역할과, 이러한 역할수행을 위해 필요한 업무역량이 무엇인지를 규명하는 것을 목적으로 질적 연구방법을 채택하여 6명의 요양보호사 및 6명의 시설운영관계자를 대상으로 개별면접 및 초점집단면접을 실시하였다. 연구 결과, 요양시설의 요양보호사들이 인식하고 있는 케어인력의 기본역할은 일상생활지원, 여가생활지원, 정보전달자인 것으로 나타났으며, 기본역할 이외의 사례관리(조력)자, 기록자, 프로그램 기획가 등의 역할은 요양호사의 개인적 특성에 따라 인식에 차이가 있는 것으로 나타났다. 그리고, 요양시설에서 케어를 전문적으로 수행하는 요양보호사의 전문성 향상을 위해 케어인력이 필수적으로 갖추어야 할 요소를 지식적 측면(knowledge, K), 가치적 측면(value, V), 그리고 기술적인 측면으로(skill, S) 범주화하여 질문한 결과, 지식적 측면에서(K) 요구되는 전문역량은 치매 및 노인성 질환에 대한 보건 및 의료적 지식, 노년기 및 노인 가족에 대한 이해로, 기술적 측면에서(S) 요구되는 전문역량은 전문 상담, 욕구파악, 신변 케어, 행정실무능력이, 가치적 측면에서(V) 요구되는 전문역량은 이용자 및 가족과의 정서적 공감능력 및 이용자의 불안감 해소를 위한 정서안정 유도 능력, 이용자를 복지서비스 고객으로 보는 서비스 마인드인 것으로 나타났다. 이 외에도, 요양시설에서 케어인력의 역할에 부합한 역량이 발휘되고 그것이 요양시설에서 제공하는 서비스의 질 향상으로 이어지도록 하기 위해서는 시설의 인사운영관리측면에서도 구체적 방안 마련이 필요함이 제기되었다.;The purpose of this study is to define what specific skills and knowledge care workers can be required to be recognized as care professionals. This paper, using 12(six are care workers and the other 6 are CEOs or employers of long-term care homes) in-depth interview cases, aimed to analyze care workers’ actual job performance and competency in the fields. The results were as follows: Firstly, the majority of care workers perceive their job performance is mainly supporting daily living activity, leisure activity, and providing necessary information of health and social services. However, their actual scope of work is often enlarged towards case management, recording documents, service programme planning. Some care workers showed the positive attitudes towards the enlargement of work scope but some considered it as “out of their duty”. The different attitudes and perceptions of care workers’ job performance is related to the recognition of care professionalism. Secondly, there are three crucial elements of improving care workers’ job competency. First, in terms of care knowledge, care workers need to be understood aging process, Alzheimer’s dementia and other health and medical conditions of the elderly. Second, in terms of care skills, care workers can perform counselling, understanding needs, daily health care and recording documents. Third, in terms of care value professional care workers are able to empathize with the elderly users and their family. Moreover, care workers, as service providers can accept to see the elderly users as their customers. Finally, human resource development and management in long-term care homes is key to the improvement of the better quality of service.
