Abstract

초록

본 연구는 국내외 문헌을 토대로 클래식 연주자 및 전공자의 신체 및 정신 건강 문제에 관한 고찰을 실시하고, 서울소재 한 사립대학교 음악대학생 및 대학원생을 대상으로 설문조사를 실시(2016.4.4.~4.15.)하여 198명을 분석하였다. 연구결과, 클래식 연주자의 3분의 2 이상은 연주 관련 통증 내지 근골격계질환과 같은 신체적 건강 문제를 경험한 적이 있으며, 설문조사 결과에서는 음대생의 평생 연주통증 경험 비율은 34%이었고, 현재 연주통증 경험 비율은 24%이었다. 클래식 연주자 및 전공자의 주요 정신 건강 문제인 연주불안의 경우는 연구에 따라 16%에서 55%로 나타났고, 설문조사 결과에서는 음대생의 평생 연주불안 경험 비율은 39%이었고, 지난 1년간 연주불안 경험 비율은 34%였다. 또한 음대생의 47%가 지난 1년간 우울을 경험하였고, 11%가 지난 1년간 자살생각을 한 적이 있다고 응답하였다. 한편, 음대생의 연주불안과 우울 및 자살생각과는 관련성이 없었으나, 연주통증과 우울 및 자살생각 간에는 통계적으로 유의한 관련성을 보였다. 우울 및 자살생각 여부에 대한 다중 로지스틱 회귀분석 결과에서는 연주통증(OR=2.51)이 우울에 영향을 미쳤고, 연주통증(OR=3.31) 및 우울(OR=8.24)이 자살생각에 영향을 미치는 요인이었다. 결론적으로 클래식 연주자 및 전공자들은 그들에게 특정된 연주불안과 같은 정신 건강 문제와 연주통증이나 연주관련 근골격계질환과 같은 신체적 건강 문제를 가지고 있다. 이에 이들에게 특화된 질병의 예방과 치료에 초점을 맞춘 상담 및 치료프로그램의 개발과 적용 방안이 필요하다 하겠다.;This study analyzed data from a survey that was conducted by the authors between April 4 and 15, 2016, on 198 South Korean college music students. A x2-test and a multiple regression model were used to examine the factors influencing their depression and suicidal ideation. Additionally, a literature review was conducted to investigate the physical and mental health problems of classical musicians. The results were as follows. First, over two thirds of the classical musicians had experienced physical health problems like music performance anxiety (MPA) and playing-related musculoskeletal disorders (PRMDs), and 16%-55% of those had experienced MPA as a mental health problem. Second, performance pain during their lifetime and current performance pain was reported by 34% and 24% of the college music students, respectively. Further, 47% of the study population had experienced depression, and 11% reported having had suicidal ideation during the last 12 months. Third, the findings showed that music performance pain was significantly related to both depression and suicidal ideation. Further, the multiple logistic regression analysis showed that music performance pain was a significant factor influencing depression, and music performance pain and depression were significant factors influencing suicidal ideation among the college music students. These findings suggest the need to not only develop consultation and treatment programs, but also to improve the music education curriculum to help instrumental musicians and music students cope with their mental and physical problems.

본 연구는 제1차(2005) 청소년 건강행태 온라인 조사 원시자료를 이용하여, 2004년 12월 30일 담배가격 인상 이후 흡연 청소년 6,458명의 흡연 행태가 어떻게 변화했는지를 분석하였다. 이들 중 15.2%(1,029명)가 흡연량을 줄였고, 31.1%(2,008명)가 금연하였으며, 금연 청소년 중 59.7%(1,222명)가 9개월 이후에도 금연을 지속하였다. 다중다항 로지스틱 회귀분석 결과, 흡연량 감소에 영향을 미치는 요인은 학년(낮을수록), 아버지의 학력(낮을수록), 학교성적(낮을수록), 흡연시도 시기(늦을수록), 고위험 음주(무), 흡입제 사용(무) 등이었고, 금연에 영향을 미치는 요인은 성(여자), 학년(낮을수록), 어머니 학력(높을수록), 학교성적(높을수록), 흡연시도 시기(늦을수록), 금연홍보노출(많을수록), 금연교육(유), 고위험 음주(무), 흡입제 사용(무), 우울(무) 등이었다. 금연지속 여부에 대한 다중 이분형 로지스틱 회귀분석 결과, 성(여자), 학년(높을수록), 흡연시도시기(이를수록), 고위험 음주(무), 흡입제 사용(무), 우울(무) 등이 유의한 요인으로 밝혀졌다. 담배가격 인상 이후 흡연자들의 금연에 영향을 미쳤던 금연홍보와 금연교육은 금연지속 수단으로서는 유의하지 않았으며, 우울과 더불어 고위험 음주, 흡입제 사용과 같은 중독관련 물질의 사용경험은 흡연량 감소나 금연뿐 아니라, 금연지속 저해 요인으로 나타났다. 따라서 학교 금연교육 시 음주, 흡입제 등 중독 관련 건강 위해 요인들에 대한 예방교육을 병행한다면 금연교육의 효과를 더욱 높일 수 있을 것으로 생각된다.;We analyzed data from the first Korea Youth Risk Behavior Web-based Survey to examine (1) a change in smoking behavior following a cigarette price increase on December 30, 2014 among 6,458 Korean adolescents who smoked at the time of the price increase, and also (2) factors associated with reduced cigarette consumption, smoking cessation, and continuous abstinence from smoking. Following the price increase, 15% of the study population reduced cigarette consumption, and 31% quit smoking. 60% of the quitters achieved nine months of continuous abstinence from smoking. Findings from a multinominal logistic regression model show that younger grades at school, parental education level, academic performance, smoking attempt at older ages, exposure to anti-smoking campaign, no high-risk drinking, and no inhalants use are positively associated with reduced cigarette consumption and also with smoking cessation. Furthermore, female, older grades at school, smoking attempt at younger ages, no high-risk drinking, no inhalants use, and no depression are significantly associated with continuous abstinence. Anti-smoking campaign and anti-smoking education are associated with cessation, but not with continuous abstinence. In addition, depression, high-risk drinking, use of inhalants appear to deter reduced cigarette consumption, cessation, and continuous abstinence. Therefore, we deduce that preventive education for risky behaviors such as risky drinking and inhalants use, along with anti-smoking education, at school would further potentially positive effects of anti-smoking education.
