생존에 심각한 위협을 가하는 암은 환자와 배우자의 부부적응에 까지 부정적 영향을 미칠 수 있다. 이는 암환자의 적응 및 삶의 질, 나아가 결혼안정성을 저해할 수 있기에 암환자 부부의 부부적응을 도모하기 위한 노력이 요구된다. 이에 본 연구는 암환자의 부부적응에 영향을 미치는 예측요인을 파악하여 암환자 부부를 돕는 심리사회적 서비스 및 프로그램을 마련하는데 함의를 제공하는 것을 목적으로 수행되었다. 이를 위해, 선행연구를 토대로 정서조절곤란이 부부적응에 영향을 미치는 연구모형을 구성하였고 암환자 부부 159쌍을 대상으로 검증하였다. 본 연구는 부부관계의 상호의존성과 상호역동성을 고려하여 커플자료 분석방법인 Actor-Partner Interdependence Model(APIM)을 구성하여 구조방정식 모형으로 분석하였다. 연구 결과, 첫째, 연구모형의 적합도는 측정모형이 χ²(df/P)=121.671(64/.000), TLI=.923, CFI=.946, RMSEA=.076, 구조모형은 χ²(df/P)=296.004(152/.000), TLI=.876, CFI=.916, RMSEA=.077로 수용 가능한 것으로 나타났다. 둘째, 환자와 배우자 모두 정서조절곤란이 자신의 부부적응에 유의한 부적 영향을 미치고 있는 것으로 나타났으며, 환자의 정서조절곤란은 배우자의 부부적응에 유의한 부적 영향을 미치는 것으로 나타나 환자의 상대방효과가 유의한 것으로 나타났다. 환자의 성별에 따른 집단을 구성하여 다집단 분석을 실시한 결과, 남녀 집단 간 차이가 없었으며 본 연구모형은 남녀집단 모두에게 적용 가능한 것으로 나타났다. 이러한 결과를 토대로 암환자 부부가 암으로 인한 충격과 심리적 고통에 대해 자신의 정서를 수용하고 명확하게 인식하여 이를 적절한 방식으로 표현할 수 있도록 하는 정서조절 프로그램 및 서비스를 마련할 필요성에 대한 실천적 함의를 도출하였다. 후속연구에 대한 제언으로, 다기관 연구를 통한 보다 많은 표본을 확보하는 것과 종단연구 및 질적 연구 등 다각적 연구 방법을 통한 심도 깊은 연구가 이루어져야할 필요성에 대해 제언하였다.;Cancer is a disease that may pose a serious threat to life and it can have a negative effect on marital adjustment in cancer patients and their spouses. Bad state of marital adjustment can have a negative effect on cancer patient’s attempts on adjusting to life and to the quality of life. Moreover, it can also harm the stability of marriage. Therefore efforts should be made to help the couple adjust to cancer. The purpose of this research is to figure out the factors that can affect the marital adjustment of cancer patient couples. This research has verified the effects of difficulties in emotion regulation has on marital adjustment which had been known to be a crucial point in personal mental health and interpersonal relationships. Considering the interdependence of cancer patient and his or her spouse, information from 159 couples were collected and Actor-Partner Interdependence Model(APIM) was used for analysis. Through the result of the research, it has been found out that difficulties in emotion regulation has significant negative effect on each of the couples in marital relationship. Cancer patient’s difficulties in emotion regulation is found out to have significant negative effect on the spouse’s marital adjustment. Based on this result, conclusion had been reached that emotional adjustment program and services in order to help the cancer patient’s spouses express the anguish and shock they have felt when the spouse fell ill with cancer by accepting one’s own emotion and recognize what they are going through. It had been concluded that clinically psychosocial support program and service needs to be in a form of counseling in pairs because couples do affect each other in marriage.

전 세계적으로 암 발생률과 사망률, 암관리비용이 증가하고 있다. 우리나라 또한 사회경제적 수준에 따라 암 치명률과 사망률의 격차를 보일 뿐 아니라, 오히려 사회경제적 취약계층 일수록 암 치료를 위한 의료이용에서 소외되는 건강불평등 양상이 나타나기 시작했다. 따라서 효과적이고 효율적인 의료재원 활용의 필요성이 제기되고 있다. 외국의 경우 우리보다 앞서 암 치료 소외계층이 의료 서비스 이용 과정에서 겪게 되는 물리적?경제적 장애요소에 적절히 개입하여 의료이용 접근성을 높이기 위한 ‘환자 네비게이션 프로그램’이 개발?적용되고 있으며 지속적으로 여러 연구를 통해 프로그램의 효과가 검증되고 있다. 미국 및 캐나다에서 진행된 200개 이상의 환자 네비게이션 프로그램을 통해 사회경제적 취약계층이 암 의료이용 시 겪게 되는 경제적 어려움, 물리적으로 낮은 이용 접근성, 문화적 차이와 언어적 제약, 분절된 서비스 전달체계, 인종에 따른 차별 등을 줄이고 암 의료이용에의 접근성을 높인 결과 암 생존율이 높아진 것으로 보고되었다. 이러한 검토에 따라 우리나라 암 치료 소외계층의 의료이용 형평성 제고를 위한 환자 네비게이션 프로그램 적용 가능성 탐색을 위해서는 우선 암 환자 및 가족 등 프로그램 대상자의 요구도 조사가 이뤄져야 하며 운영 주체를 통한 원활한 프로그램의 기틀 마련, 그리고 네비게이터의 발굴이 이뤄져야 한다. 한국형 환자 네비게이션 프로그램의 성공적인 적용을 위해서는 암 환자 의료이용 실태에 대한 체계적인 조사와 네비게이터 양성을 위한 교육 프로그램이 과제로 제시되었다.;The rate of cancer and the cost of treatment for cancer have increased exponentially worldwide. Given the social costs attributed to the rising rate of cancer-related mortality and death, medical inequality became an issue, and there is a recognized need for effective medical support for cancer patients. In several countries, the Patient Navigation Program, a systematic cancer prevention and intervention program designed to be accessible to Health care disparities in need of cancer treatment, has been reported to be effective. The focus of the study is to investigate the characteristics of Patient Navigation Program and to examine the feasibility of the program in South Korea. The navigation program train paraprofessionals to provide intervention services for cancer patients who experience barriers to medical services. Presently, there are over 200 Navigation Programs in the United States and Canada. As a result, the program has been found to be effective as increasing number of low-income patients have access to medical service which has reportedly decreased the rate of cancer-related mortality. In order to examine whether such program would be as effective in South Korea, our study examines the background and characteristics of Navigation Program use. There is a need for survey for data base for cancer patient service utility and develop navigator training program. The navigation program will decrease the rate of cancer related mortality and serve for medical inequality.

본 연구는 유방암 검진 수검률 향상을 위한 네비게이터 개입의 효과성을 검증하는것을 목적으로, 서울시 S구에 거주하면서 사회경제적으로 취약한 국가 암 검진 대상자 중 개입 당시 유방암 검진을 받지 않고 네비게이터 개입 이후 유방암 검진 수검 여부를 확인한 730명을 연구 대상으로 하였다. 연구 결과, 첫째, 유방암 검진 수검률은 69.9%로, 전국, 서울시, S구의 전년도 검진 수검률 보다 높았다. 둘째, 유방암 미검진 이유인 ‘시간적 여유 없음’(χ²=30.33, p;This study aims to investigate the effectiveness of patient navigation services to improve breast cancer screening rates. The study samples are 730 socioeconomically disadvantaged women who were identified from S public health center, and were possible to confirm if the cancer screening has been conducted after the first interview. First, the breast cancer screening rate was 69.9%, indicating that this rate was higher than those of nation, Seoul, and S public health center. Second, participants who responded ‘I don’t have time’ or ‘I worry that I have cancer’ were less likely to screen the breasts for cancer. Third, knowledge and beliefs about breast cancer and screening were significantly improved after activities of navigators. Fourth, logistic regression showed that patients who have barriers to screening were less likely to screen for breast cancer, and those who have experiences about navigation services were more likely to have breast cancer screening. This study contributes to the improvement in cancer screening rate and knowledge/beliefs about breast cancer screening, and suggests navigator role to improve breast cancer screening rates.
