초록

본 연구는 2010년 국민건강영양조사 원시자료를 이용하여 26세 이상 현재 흡연자 1,241명을 대상으로 하드코어 흡연자 비율을 산출하고, 하드코어 흡연에 관련된 요인을 파악한 것이다. 하드코어 흡연자는 만 26세 이상의 5년 이상 매일흡연자로서, 일 15개비 이상을 흡연하고, 지난 1년간 금연시도 경험이 없고, 향후 6개월 내 금연계획이 없는 자로 정의하였다. 연구 결과, 2010년 우리나라 하드코어 흡연자 비율은 25.5%(남자 27.7%, 여자 10.7%)이었다. 하드코어 흡연자에 영향을 미치는 요인을 분석한 단순 로지스틱 회귀분석(모형 1)에서는 성(남 vs. 여 OR=3.20), 고용상태(유 vs. 무 OR=1.45), 고위험음주(유 vs. 무 OR=1.47), 흡연시작연령(<19세 vs. ?26세 OR=2.40, 19~25세 vs. ?26세 OR=1.72), 흡연기간(10~29년 vs. <10년 OR=2.03, ?30년 vs. <10년 OR=2.62), 흡연량(20~29개비/일 vs. ?19개비/일 OR=6.49, ?30개비/일 vs. ?19개비/일 OR=9.22), 금연교육(무 vs. 유 OR=1.67)이 통계적으로 유의한 요인이었다. 반면, 다중 로지스틱 회귀분석(모형 2)에서는 흡연량(20~29개비/일 vs. ?19개비/일 OR=6.26, ?30개비/일 vs. ?19개비/일 OR=8.50)과 금연교육(무 vs. 유 OR=1.89)이 통계적으로 유의한 요인으로 나타났다. 결론적으로, 흡연시작 연령이 어릴수록, 흡연기간이 길수록, 흡연량이 많을수록, 그리고 금연교육을 받지 않을수록 하드코어 흡연자가 될 확률이 증가하므로, 흡연율을 낮추기 위해서는 흡연시작 자체를 예방하는 정책과 더불어 하드코어 흡연자를 대상으로 차별화된 금연정책을 강구할 필요가 있다.;Data from 1,241 Korean current smokers aged over 26 from the sample used in the 2010 Korea National Health and Nutrition Examination Survey were used in this study. Among the current smokers, 25.5% were hard core smokers (male 27.7%, female 10.7%). A simple logistic regression analysis showed the following factors to be significantly related to hardcore smoking: gender (male vs. female OR=3.20), employment status (yes vs. no OR=1.45), alcohol problem (yes vs. no OR=1.47), age of smoking initiation (<19 vs. ?26 OR=2.40, 19~25 vs. ?26 OR=1.72), duration of smoking (10~29 years vs. <10 OR=2.03, ?30 vs. <10 OR=2.62), amount of smoking (20~29 cigarettes/day vs. ?19 OR=6.49, ?30 vs. ?19 OR=9.22), and anti-smoking education (no vs. yes OR=1.67). However, a multiple logistic regression analysis showed that only the amount of smoking (20~29 cigarettes/day vs. ?19 OR=6.26, ?30 vs. ?19 OR=8.50) and anti-smoking education (no vs. yes OR=1.89) were significantly associated with hardcore smoking. In conclusion, hardcore smokers are more likely to be male, be employed, and have alcohol problems. They also are more likely to have started smoking at a younger age, have smoked for a longer time, and smoke more, and are less likely to report receiving anti-smoking education. Therefore, to reduce the smoking rate, the Korean government should not only prevent adolescents from smoking but also focus on policies for hardcore smokers.
