본 연구는 약물사용자의 치료서비스 이용 의도에 대한 영향 요인과 그 경로를 분석하는 데 연구목적이 있다. 분석틀로 Gelberg-Andersen의 취약계층행동모형을 사용하였다. 성인 약물사용자 714명이 연구에 참여하였으며, 다변량이상치를 제거한 694사례를 분석에 활용하였다. 분석방법은 구조방정식모형과 효과분해를 사용하였다. 분석결과 총효과가 통계적으로 유의한 요인은 전통영역의 성별과 취약영역의 과거치료경험, 약물사용년수, 약물남용심각도로 나타났다. 치료서비스 이용 의도는 성별에 따라 뚜렷한 차이가 나타났다. 과거치료경험은 치료서비스 이용 의도에 직접 영향을 미치기도 하지만 약물남용심각도를 매개로 하여 간접 영향을 미치기도 한다. 약물사용년수는 치료서비스 이용 의도에 직접 영향을 미치지 않았으나 약물사회망과 약물남용심각도를 매개로 하여 간접적으로 영향을 미쳤다. 약물남용심각도는 치료서비스 이용 의도에 직접 영향을 미치며, 다른 영향요인들의 효과를 완충하거나 매개하는 역할도 하였다. 약물중독 치료현장에서는 약물사용자의 자발적인 치료 참여 도모를 위하여 성인지적 관점 및 약물사용자의 취약특성을 고려한 개입 대안을 마련할 필요가 있다.;The purpose of this study was to investigate the factors and paths affecting Korean drug users’ intention to use treatment services. The Behavioral Model for Vulnerable Populations was used as the theoretical framework. The survey originally included 714 drug users, 694 of whom were analyzed after the removal of multivariate outliers. Data were analyzed using structural equation modeling and effect decomposition. Factors with a statistically significant effect were as follows: from the traditional domain - gender; from the vulnerable domain - previous treatment experience, years of drug use, and drug abuse severity. There was a distinct difference between genders regarding users" intention to use treatment services. Previous treatment experience influenced users" intention to use treatment services directly, as well as indirectly, via drug abuse severity. Years of drug use did not have a direct effect on intention to use treatment services, but it had an indirect effect via drug networks and drug abuse severity. Drug abuse severity was found to have a direct effect on intention to use treatment services and functioned as a buffering and mediating factor. There should be a careful consideration of gender and other vulnerable characteristics of drug users in treatment settings in order to encourage their voluntary use of treatment services.

본 연구는 우리나라 만 19세 이상 성인의 주관적 건강상태 궤적과 그 예측요인을 확인하고, 이러한 궤적 및 예측요인이 장애인과 비장애인 각 집단 내적으로, 그리고 집단 간에 차이가 있는지 살펴보았다. 이를 위해 한국복지패널 1차년도(2006년)부터 9차년도(2014년) 자료를 활용하였고, 최종분석대상은 14,613명이다. 주요 결과는 다음과 같다. (1) 무조건부 모형에 대한 장애인과 비장애인 집단의 다중집단분석결과 두 집단의 건강상태 궤적이 유의미한 차이를 보였고, 장애인의 건강상태 궤적이 더욱 나쁜 것으로 나타났다. (2) 주관적 건강상태 궤적과 예측요인의 관계를 분석한 조건부 모형의 다중집단분석 결과, 건강 불평등 예측요인 유형이 ①심화요인(장애인과 비장애인 모두 연령), ②지속요인(장애인: 교육, 취업, 음주, 주거환경, 사회적지지; 비장애인: 흡연, 사회적지지) ③완화요인(장애인: 소득, 외래진료 수; 비장애인: 소득, 교육, 결혼, 취업, 음주, 외래진료 수, 주거환경) ④무영향요인(장애인: 성별, 결혼, 흡연, 건강검진 수; 비장애인: 성별)으로 구분되었다. 이러한 결과는 장애인과 비장애인 집단별로 건강상태궤적이 불평등하고, 예측요인의 종류가 다름을 의미한다. 따라서 장애인과 비장애인 집단의 건강을 증진 및 예방하고 건강 불평등을 해결하기 위해서는 건강상태에 미치는 요인들이 시간의 흐름에 따라서, 그리고 장애유무에 따라서 다름을 고려하여 보다 세밀한 접근이 필요함을 시사한다.;This study was aimed at examining self-rated health trajectories of health and their associated factors among people aged 19 and over in Korea. Also, whether these estimated results differ according to people with and without disabilities was analyzed. For these purposes, we used latent growth curve modeling and multi-group analysis involving 14,613 individuals who participated in the Korean Welfare Panel Study from 2006 to 2014. Major findings are as follows. 1) Multi-group analysis revealed that health trajectories differed between people with and without disabilities. Specifically, health status was worse in people with disabilities’ than in people without disabilities. 2) Multi-group analysis of conditional model revealed that health trajectories and their associated factors differed between people with and without disabilities. These results can be divided into four types: ① inequality intensified (i.e., aged), ② inequality maintained (i.e., w/disabled: education, employment status, residential environment) ③ inequality alleviated (i.e., w/disabled: income, number of outpatient visits; w/o disabled: income, education), ④ non effected (i.e., w/disabled: marital status, smoking; w/o disabled: gender). These findings suggest that the health related interventions should be planned considering the differences between the disabled and non-disabled people.

본 연구는 한국의 비정규직 고용과 건강의 연관성을 다룬 기존 논문을 체계적으로 정리하고 향후 연구방향을 제시하고자 하였다. 이를 위해 국내외 웹 기반 학술데이터베이스를 활용해 비정규직 고용과 건강의 연관성을 검토한 문헌을 검색하여 2014년 12월 31일까지 출판된 학술논문 37편을 검토하였다. 본 연구의 결과는 다음과 같다. 첫째, 관련하여 총 37편의 논문이 출판되었으며 모두 양적 연구였고, 가장 많이 이용된 데이터는 한국노동패널(N=9)이었다. 둘째, 건강변수는 정신건강(N=17)과 자가평가건강수준(N=13)에 집중되어 있었다. 셋째, 전체 37편 중 31편의 연구에서 인구학적 혹은 사회경제적 정보를 잠재적 교란인자로 통제하여 다변량분석을 진행했다. 넷째, 전체 37편 중 35편의 연구결과에서 정규직보다 비정규직의 건강이 유의미하게 나빴음을 보여주었다. 이에 기반하여, 향후 연구는 다음과 같은 고려가 필요하다. 비정규직을 정의하고 범주화하는 구체적, 현실적 기준이 제시되어야 한다. 행정데이터나 의료기록 등의 자료를 활용하여 보다 다양한 건강관련 변수를 고려하고, 고용형태와 건강과의 연관성 분석에서 교란인자와 매개인자를 구분해서 분석해야 한다. 나아가, 고용형태와 건강에 대한 질적 연구나 정책적 개입 연구, 성별, 종사산업별, 연령별 차이에 주목한 연구가 필요하다.;The goal of this study is to systematically review the academic literature on the association between non-standard employment and health in South Korea. We searched online academic databases to find relevant papers in domestic and international journal by 31 December 2014 and this study reviewed a total of 37 articles. The followings are the findings of this study. First, all 37 studies have adopted quantitative methods and the most frequently used dataset was the Korean Labor & Income Panel Study (N=9). Second, the studies focused mainly on mental health (N=17) or self-rated health (N=13). Third, 31 studies controlled demographic or socio-economic indicators as potential confounders. Fourth, 35 studies reported that non-standard workers had poorer health compared to standard workers. This study suggests recommendations for future research. First, it is necessary to define non-standard employment with concrete criteria, reflecting labor market conditions in Korean society. Second, various health outcome should be examined using administrative data and medical records. Third, it is important to take account of relevant confounders in the data analysis. Finally, future research needs to consider how the health effect of non-standard employment could differ by worker’s gender, age, and industry division.
