Abstract

초록

본 연구의 목적은 다년간의 건강검진 및 문진 자료를 이용하여 고혈압 건강위험평가 모형을 개발하기 위한 것이다. 2010년 고혈압 미발생 수검자 11,632명을 대상으로 이들의 2009~2010년 건강행태, 신체계측 및 생화학검사 결과와 같은 건강위험요인 상태변화가 2011~2012년 고혈압 발생에 미치는 영향을 파악하기 위해 로지스틱 회귀분석으로서 고혈압 건강위험평가 모형을 개발하였다. 그 결과, 개발된 고혈압 건강위험 평가 모형을 통해 연령, 가족력, 음주습관 변화, 허리둘레 변화, 혈압 및 중성지방 수치의 변화가 주요 위험요인으로 선별되었다(C-통계량 =0.81[남=0.78, 여=0.87]). 특히 고혈압 전단계(120-139/80-89 mmHg) 수준을 유지하는 경우, 정상 수준을 유지하는 사람에 비해 3-4년 이내 고혈압 발생위험이 16.52배(95% CI, 9.36-29.16) 높았다. 건강행태, 신체계측, 생화학검사의 변화 정도를 위험 요인으로 구성한 고혈압 발생 예측 모형은 환자뿐만 아니라 임상의료인과 보건정책가에게도 유용하게 이용될 수 있을 것이며, 추후 외적 타당도 연구가 수행되어야 할 것이다.;This study aimed to develop a health risk appraisal model for predicting hypertension incidence by using measures based on multi-year (2008-2012) data of individuals who underwent a general health examination. A total of 11,632 subjects with a normal blood pressure in 2010 were enrolled to observe changes of health behavior, anthropometric measurements, and laboratory results from 2009 through 2010 affecting hypertension incidence in 2011-2012. A hypertension risk score model was developed using logistic regression. Also, the C-statistic (95% confidence interval) was used to develop the best-fitting prediction model. Hypertension risk model consisted of age, family history of hypertension, and changes in drinking behavior, visceral obesity, blood pressure level, and triglycerides level (C-statistic=0.81[male=0.78, female=0.87]). The incidence rate of hypertension was positively associated with change (Δ) in blood pressure level, especially sustaining level of prehypertension (odds ratio [95% CI], 16.52 [9.36-29.16]), independently from other risk factors. The health risk model will be helpful for clinicians and policy makers as well as individuals to prevent hypertension.

본 연구는 당뇨병 유병률의 지역 간 변이 요인을 규명하여 지역별 특성에 맞는 당뇨병 관리 사업을 지원하기 위한 기초자료를 제공하기 위해 수행되었다. 이를 위해 질병관리 본부의 2011년도 시군구 지역사회건강조사 230건 자료를 지역별 사회경제학 지표 등과 연계하여 생태학적 연구를 위한 자료를 구축한 후 단계적 회귀분석, 의사결정나무 등의 기법으로 분석하였다. 단계적 회귀분석 결과 인지된 고혈압 유병률과 경제활동 비율이 높고, 비만율이 낮을수록 당뇨병 유병률이 높아지는 것으로 나타났다. 지역 간에 상이한 변이 요인을 보다 구체적으로 알아보고자 의사결정나무 모형을 이용하여 지역 간 변이 요인을 규명한 결과 인지된 고혈압 유병률, 비만율, 고위험 음주율, 유배우자 비율, 인구밀도 등이 당뇨병 유병률의 주요 변이 요인으로 나타났다. 당뇨병 유병률의 지역 간변이 요인은 세부 지역별로 다양한 양상으로 나타났는데, 본 연구 결과는 지역별 맞춤형 당뇨병 관리 사업 계획을 수립하는데 기초자료로 활용될 수 있겠다.;We investigate how the regional prevalence of diabetes is affected by health-related and socioeconomic factors with a special emphasis on geographic variations. We focus on the likelihood of diabetes as function of various region-specific attributes. We construct a unique set of data at the level of 230 small administrative district collected from 2011 Annual Community Health Survey by Korea Centers for Disease Control and Prevention and other government agencies. To estimate, we use several methods including correlation analysis, multiple regression and decision tree model. We find that diabetes prevalence is more likely to be associated with hypertension prevalence, obesity and economic activity rate. Further findings using decision tree model suggest that hypertension prevalence, obesity, the rate of drinking and the density of population are more likely to affect the prevalence of diabetes than other regional attributes considered. More importantly, we find significant geographic variations in factors affecting diabetes prevalence across administrative districts. This topic are not just of academic interest, but have practical implications by helping policy makers to understand and identify important regional factors relating to the prevalence of diabetes by which they can implement more effective planning for promotion of health.

본 연구는 의사결정트리를 이용하여 성인의 미충족 치과의료 경험의 영향 요인을 파악하고, 미충족 치과의료 경험 가능성이 높은 위험집단 및 치과의료 미충족 이유에 따른 집단을 분류하여 그 특성을 분석하였다. 자료는 제6기(2013~2015년) 국민건강영양조사의 만 19세 이상 성인 15,918명이며, 연구에 이용된 변수는 인구사회학적 특성, 건강행태적 특성, 정신인지적 특성, 건강관련 특성, 구강건강 특성 등이다. 자료는 복합표본분석, 의사결정트리, 로지스틱회귀 분석을 실시하였다. 연구결과 미충족 치과의료에 미치는 영향 요인은 저작불편호소가 가장 중요한 요인이었으며 다음으로 가구소득수준, 스트레스, 연령, 경제활동 상태, 고할압 의사진단 등의 순으로 나타났다. 저작불편을 호소하고 가구소득 수준이 낮은 65세 이상의 보험급여 가입자 그룹의 미충족 치과의료 경험이 67.8%로 전체 미충족 치과의료 경험률 31.3% 대비 두 배 이상 높았다. 또한 미충족 치과의료의 이유를 의료접근성 모형의 단계에 따라 위험집단의 특성을 파악해 본 결과, 의료필요의 인지(12.5%), 의료추구(21.8%), 의료도달(27.0%), 의료이용(36.9%)의 단계에 이를수록 미충족 치과의료의 이유에 대한 분율이 높았다. 본 연구는 미충족 치과의료의 영향요인과 이유를 체계적으로 분석하기 위해 의료접근성 모형 및 의사결정트리를 이용하여 위험집단을 분류하고 이들의 특성을 파악하였다. 치과의료 접근성 향상을 위해서는 미충족 치과의료 위험집단의 특성별로 의료인지, 의료추구 단계에서부터 보건교육, 사회적 지지 등의 개입이 필요하다.;The purpose of this study was to analyze the factors affecting the unmet dental care of adults using decision trees and to classify them according to the risk group of unmet dental care and the group of reasons for unmet dental care. The data are 15,918 adults aged 19 and over in the National Health and Nutrition Examination Survey during the 6th period (2013 - 2015). The variables used in the study were socio-demographic characteristics, health behavior characteristics, mental health characteristics, oral health characteristics. Data were analyzed by composite sample, decision tree, and logistic regression analysis. The results of this study are as follows. Dental complaints such as mastication discomfort were the most important factors affecting unmet dental care, followed by household income level, stress, age, economic activity status. 67.8% of those who have a complaint of mastication discomfort, low income and medical aid-insured group 65 years old or older experienced unmet dental care. This is over two times compared with overall unmet dental care experience 31.3%. In addition, the reason for the unmet dental care was classified according to the stages of the access model to health care. The results were as follows: recognition of dental needs (12.5%), dental care pursuit (21.8%), dental reach (27.0%), and use of dental care (36.9%). This study analyzed systematically the factors and reasons of unmet dental care using a health care access model and decision tree analysis. In order to improve accessibility of dental care, interventions such as health education and social support are needed for each type of unmet dental health risk group, especially classified according to early stages of access to health care such as first stage of perception of needs and desire for care and second stage of health care seeking.
