우리나라의 생애의료비 분포 추정
Lifetime Distribution of Medical Cost in Korea

초록

본 연구는 개인들이 생애에 걸쳐 지출하게 되는 의료비 분포를 분석하고자 하였다. 이를 위하여 생명표 모형(life table model)을 기반으로 하여 남자, 여자 각각 100,000명의 가상코호트를 설정하고 국민건강보험공단의 진료비자료와 통계청의 사망자료를 활용하여 생애의료비를 추정하고자 하였다. 분석결과, 우리나라 국민의 1인당 생애의료비는 남자의 경우에는 약 7천 415만원, 그리고 여자의 경우에는 약 8천 787만원을 지출하는 것으로 분석되었다(2007년 기준의 현재가치). 남녀의 차이를 보면, 여자의 1인당생애의료비가 남자의 생애의료비보다 약 19% 정도 더 지출하는 것으로 나타났다. 여성의 생애의료비가 남성의 생애의료비를 초과하는 가장 주요한 요인 중의 하나는 평균수명의 차이에 기인한 것으로 파악된다. 남성의 경우 64세까지 생애의료비의 절반에 해당하는 액수가 지출되는데, 이는 곧 64세 이후에 전 생애의료비의 절반을 지출하게 되는 것을 의미한다. 그리고 여성의 경우에는 66세까지 생애의료비의 50%를 지출하고, 66세 이후 50%를 지출하는 것으로 추정된다. 즉, 의료비 지출이 고령시기에 집중되는 불균형을 확인할 수 있었다. 본 연구에서 분석한 결과에 따르면 생애의료비의 절반 이상이 65세 이후에 발생하는 것으로 나타났는데, 급격한 고령화 사회에 직면하고 있는 우리에게 주요한 정책적 시사점을 제시하고 있다. 현재와 같은 의료비 분포가 지속될 경우 고령화의 정도가 가속화되면서 폭발적인 의료비의 급증이 예견된다. 따라서, 급격한 의료비의 증가에 대응하고 의료재정의 지속성, 안정성, 건전성을 위한 제도의 착실한 계획과 구축이 절대적으로 필요할 것이다.;With the recognition that health care cost is highly correlated with age, this study aims at estimating the magnitude and distribution of lifetime medical costs. We employed a period life table model using data on single year’s medical expenditures from the Korea National Health Insurance Corporation, population data and a life table from Statistics Korea. To estimate lifetime medical costs for males and females, 100,000 hypothetical cohorts for each sex whose mortality rates come from the life table were constructed and assigned to medical costs at different ages and sexes. This procedure permits us to estimate lifetime medical costs at each age and for each sex. The method used here is to conceptually convert cross-sectional costs data into a longitudinal pattern of costs, generating profiles of medical costs from birth to death. The average member of the birth cohorts will spend KRW 74,150,000 (US$ 98,883 PPP) for men and KRW 87,868,000 (US$ 117,177 PPP) for women in 2007 over the course of his or her life. Total lifetime medical costs are 19 percent higher for females than males mainly due to differences in life expectancy. For the average life table member, half of all lifetime costs occur after about age 65. We also estimate the distribution of lifetime medical costs by phases of the life course. For males(females), 12.7(9.2) percent of a cohort’s expenditures occur from birth to age 19; 8.4(10.0) percent accrue during ages 20~39; 30.2(28.4) percent are expended during ages 40~64; 42.7(44.0%) percent are realized during ages 65~84; 5.9(8.4) percent accrue over age 85. The findings of the study are similar to the patterns of medical costs of other countries’ experiences provided by other literatures-high during infancy, low during childhood and young adulthood, then rising during middle-age and rapidly growing during the senior years.

2010년부터 베이비붐 세대가 은퇴연령에 도달하기 시작하면서 대규모 인구집단이 은퇴를 맞이하게 되어, 이에 대한 사회적, 경제적 측면의 대응뿐만 아니라, 이 세대가 은퇴 후 사회참여를 통해 활기차고 건강한 노년을 맞이할 수 있도록 국가 차원의 준비가 함께 필요하다. 일본의 경우 후생성으로 창구를 일원화하여 은퇴 후 일자리나 자원봉사와 같은 사회참여 정책을 지원하고 있다. 일본은 여가 및 자원봉사 등을 원하는 은퇴자들을 위해서는 커뮤니티를 기반으로 쉽게 사회참여가 가능한 노인클럽과 같은 프로그램을 활성화하였고, 일을 원하는 은퇴자들을 위해서는 실버인재센터를 시정촌별로 최소한 개씩 두어 언제든지 지역사회에서 일할 수 있도록 지원하는 체계를 구축하고 있다. 이와 같은 일본의 사회참여 지원정책이 우리나라에 주는 시사점은, 사회참여를 활성화함은 물론 커뮤니티를 기반으로 한 양방향의 사회참여 채널을 일원화하여 운용하고 있어 단편적이거나 중복적인 사업 지원의 소지를 최소화 했다는 데에 있다. 우리나라 베이비붐 세대의 본격적인 은퇴에 대비하여, 단기적으로는 부처별로 해당 영역에서 은퇴 후 중고령자의 사회참여를 보다 활성화시키고 중장기적으로는 효율적이고 체계적으로 지원할 수 있는 시스템을 구축하여 현재 부처별로 산재해 있는 노인 사회참여 지원정책 또한 일원화하여 시행할 필요가 있다.;A large population of Korean baby boomers has begun in 2010 to retire from the labor force. The social and economic response for the impacts due to retirement, through the community participation after retirement, has been required to prepare the vibrant and healthy old age at the national level. In Japan, the Ministry of Health and Welfare supports community involvement programs to induce employment or volunteer work among retirees. For retirees who want leisure or volunteer work, Japan has implemented such programs as Seniors’ Club, which allows easy access to community-based social participation, and local Silver Centers which support retirees who want to continue working. Through Japan’s social involvement policy, we learned that a unified management channel is conducive to reducing leakage and duplication in support. After retirement for the baby-boom generation, in order to participate by selecting in the program smoothly and any time, it is necessary for the Korean government to build more efficient and systematic support programs for the social participation of retirees, as well as a unified support policies channel.
