코로나19 유행기간 보건소 진료서비스 이용자의 보건소 서비스 중단・축소 경험에 관한 질적 연구
Experiences of Chronic Patients with the Interruption of Healthcare Services in Community Health Centers during the COVID-19 Pandemic

Abstract

Community health centers have served as a main institution in response to the COVID-19 pandemic. As their tasks increased with respect to COVID-19, such as screening tests, epidemiological investigations, and the management of quarantined people, public health centers during the COVID-19 pandemic had to reduce or discontinue their existing services for patients with chronic diseases. The objective of this study was to examine the experiences public health center users had with being unable to use treatment or medical examination services during the COVID-19 pandemic. A focus group interview was conducted with nine patients with chronic diseases who had received treatment at public health centers before the outbreak of COVID-19. We analyzed the interview using the thematic analysis method. Six themes and 10 sub-themes were derived: 1) Service use at public health centers prior to the COVID-19 pandemic; 2) the notice of suspension/change of public health center services; 3) finding a hospital or clinic to replace the medical service use of public health centers; 4) adapting to a new clinic or hospital; 5) understanding and complaints about discontinuation of health center treatment/test services; and 6) willingness to use public health centers after the end of the pandemic. The results suggest the need to support continued access to healthcare and provide alternative services, such as non-face-to-face service, so that disease management for chronic patients does not stop when public health center services are reduced or discontinued due to an epidemic of infectious disease.

보건소는 지역사회에서 코로나19 팬데믹 대응의 핵심 기관으로 선별검사소 운영, 역학조사, 격리대상자 관리 등의 방역업무가 증가하면서 지역사회 만성질환자 대상 진료서비스가 축소되거나 중단되었다. 이 연구의 목적은 보건소의 업무가 중단・축소된 기간 동안 진료서비스 이용이 불가능해진 보건소 이용자의 코로나19 팬데믹 기간의 경험을 파악하는 것이다. 코로나19 발생 전 보건소에서 진료를 받았던 수도권 거주 9명의 만성질환자가 초점그룹 인터뷰에 참여하였다. 인터뷰 내용을 주제 분석 방법에 따라 분석하였으며, ‘보건소 이용 경험’, ‘보건소 서비스 중단에 대한 정보 전달’, ‘보건소 서비스 중단에 대한 대응’, ‘민간 병의원에서의 경험’, ‘보건소 서비스 중단에 대한 이해와 불만’, ‘향후 보건소 이용 의사’의 6개 주제와 10개의 하위 주제가 도출되었다. 이러한 보건소 이용자들의 경험은 감염병 유행 등으로 보건의료기관 서비스가 중단・축소될 경우 만성질환자의 질환 관리가 중단되지 않도록 지속적인 의료 이용을 지원하고 비대면 진료 등 대체 서비스 제공이 필요하다는 것을 보여준다.

장기요양보험제도에서 요양보호사는 돌봄을 제공하는 주체로서 상당한 역할을 하고 있으나, 기존 연구들에서는 이들의 건강에 대해 제한적으로 다루고 있었다. 본 연구는 요양보호사의 건강 문제와 이를 완화하기 위한 개입 방안을 사회맥락적 관점에서 다루었다. 요양보호사를 직업으로 선택하는 이들은 대체로 중장년층 저학력 여성이고, 이들의 돌봄 노동은 가치를 제대로 인정받지 못하고 있었다. 업무 과정에서 과도한 신체활동이 요구되나 이러한 위험을 완충하기 어려운 열악한 근로 환경에 놓여있는 것으로 나타났다. 이들의 차별적인 건강결과는 위험요인으로 작용하는 열악한 근로환경뿐 아니라, 위험의 부정적 효과를 최소화할 자원에 대한 접근, 그리고 취업 시점부터의 사회적 층화에서 그 원인을 찾아볼 수 있었다. 요양보호사의 건강문제는 차등노출, 차등취약과 연결되어 있으며, 근본적으로는 자원의 불공정한 분배를 야기하는 사회구조로 인한 불형평임을 알 수 있다. 따라서 요양보호사의 건강문제를 보다 근본적으로 변화시키기 위해서는 건강결과가 나타나는 마지막 단계뿐 아니라 사회적 층화 수준에서의 개입이 동반되어야 할 것이다.;The paper explores fundamental causes of the LTC (long-term care) givers’ health risks and interventions to reduce adverse health outcomes. A realist review based on contextual model connecting from social determinants of health to health outcomes is conducted with 128 relevant articles systematically selected. Literatures show that LTC givers are exposed to multiple health risks: overwhelming load of work, sexual harassment, and job stress. In comparison, it is rarely available for them to have power to control work situation or resources to deal with health risks. These imbalances, excessive exposure to health risk vs. limited access to resources, are often caused by social stratification. These results reveal that social stratification (social context) has to be dealt with properly to reduce health risks of LTC givers.
