우리나라 기후보건 탄력성의 현황과 과제: 보건의료체계 관점의 국제비교를 중심으로
Climate Change and Health Resilience in South Korea: An International Comparison from a Health System Perspective

Abstract

Climate change is emerging as a significant global health risk. While previous studies have highlighted the health impacts of climate change, there remains a limited understanding of the adaptation strategies remains limited within the healthcare system in South Korea. This study, grounded in the World Health Organization’s framework for building climate health systems, evaluates South Korea's health resilience to climate change from an international perspective. Drawing insights from major countries like the UK, Germany, and Japan, we assessed how the South Korean health system responds to climate change and identified areas for improvement. Our findings indicate that the recognition of the link between climate change and health in South Korea is relatively low, and there is a lack of systematic response strategies. To address these challenges, we propose the following: enhance public education and promotional activities to raise awareness of the health implications of climate change; invigorate research to establish a systematic response strategy; incorporate diverse stakeholder opinions to develop a comprehensive response strategy; strengthen inter-departmental collaboration for efficient strategy implementation; and bolster international cooperation to integrate global standards and experiences into the strategy. Through these systematic responses, we anticipate that South Korea’s ability to address the health implications of climate change will be strengthened, thereby safeguarding the health of its citizens.

기후변화는 전 세계적으로 건강 위험을 높이는 주요 요인으로 부상하고 있다. 선행연구들은 기후변화의 건강 영향을 지적하였으나, 국내에서의 보건의료체계 적응 전략에 대한 깊은 이해는 아직 부족한 상황이다. 본 연구는 세계보건기구(WHO)의 기후보건시스템 구축 프레임워크를 기반으로, 우리나라의 기후보건 탄력성을 국제적 시각에서 평가하였다. 프레임워크는 6개의 보건의료체계 요소와 함께 기후보건 탄력성을 강화하기 위한 10가지 주요 구성요소를 통합하여 제시한 것으로 1) 리더십 및 거버넌스, 2) 건강정보시스템, 3) 취약성, 역량 및 적응 평가, 4) 통합된 위험 모니터링 및 조기경보, 5) 건강 및 기후연구, 6) 필수의료 제품 및 기술, 7) 서비스 제공, 8) 환경적 건강 결정요인 관리, 9) 기후 기반 건강 프로그램, 10) 비상대비 및 관리를 포함한다. 더불어 본 연구에서는 주요 국가들인 영국, 독일, 일본의 기후보건 탄력성 전략을 참조하여, 우리나라의 건강 시스템이 기후변화에 어떠한 대응을 하고 있는지, 그리고 어떤 개선 방향이 필요한지를 도출하였다. 결과적으로, 우리나라에서는 기후변화와 건강의 연결 인식이 상대적으로 낮으며, 체계적인 대응 전략이 부족함을 확인하였다. 이를 극복하기 위한 방향으로는 첫째, 대중의 기후변화와 건강 인식을 높이기 위한 교육 및 홍보 활동 강화가 필요하다. 둘째, 체계적인 대응 전략 마련을 위한 연구 활성화가 요구된다. 셋째, 다양한 이해관계자의 의견을 수렴하여 포괄적인 대응 전략을 구축해야 한다. 넷째, 다부처 간 협력을 통한 효율적인 대응 전략 실행이 중요하다. 마지막으로, 국제적 협력을 통해 세계적인 기준과 경험을 반영한 전략을 수립해야 한다. 이러한 체계적인 대응을 통해 우리나라의 기후변화에 대한 건강 대응 능력을 향상시키고, 국민의 건강을 지킬 수 있을 것으로 기대한다.

본 연구는 국내의 미숙아 지원정책 제언을 목적으로 OECD 주요 선진국의 미숙아 관련 정책을 비교보건의료제도론적 방법론을 활용하여 고찰하였다. 우리나라의 정책으로는 2000년 미숙아 의료비 지원정책, 2008년 신생아 집중치료 지역센터 설치·운영 지원사업, 2009년 지방자치단체의 미숙아 의료지원 정책, 2010년 미숙아 등록관리 정책, 2014년 고위험 산모·신생아 통합치료센터 설치 및 운영지원 사업, 2015년 고위험 임산부의 의료비 지원사업이 존재한다. 주요 선진국의 미숙아 정책은 크게 미숙아 보건의료 인프라 구축과 미숙아 가정을 위한 의료적·경제적 및 기타 지원이 이루어지고 있다. 외국과 비교해보았을 때 우리나라의 미숙아 지원정책을 단계적으로 확대해 나가고는 있으나 총괄적 차원에서의 미비점이 존재한다. 거버넌스 측면에서 해당 예산규모의 확대와 국가차원의 미숙아 관련 관리체계 및 전략 수립, 임신 전 단계 및 임신 기간 중의 미숙아의 예방 및 감소를 위한 방안 마련, 미숙아 지원을 위한 관련 R&D 및 관련 사업의 추가 확대, 미숙아 관련 기초 통계 자료 확보, 근거에 기반 한 미숙아 정책 마련, 지역의 주산기 관리체계 구축, 신생아중환자실에 대한 시설·장비 지원 확대, 인력 역량강화 지원이 필요하다. 미숙아 가정의 부담경감을 위한 지원으로는 의료적 차원에서 미숙아 관리 가이드라인, 가족의 케어 참여, 장기적 관점의 지원이 요구되며, 경제적 차원에서는 출산 후 입원 의료비, 호흡기 질환 예방접종, 퇴원 후 의료비, 재활치료 비용의 지원이 고려 가능하다. 그 밖에 미숙아 가정 지원방안으로 출산휴가 연장 및 특별휴가, 양육지원, 심리 정서적 지원 방안을 고려할 수 있다.;The aim of this study is for suggesting the support policy for prematurity in Republic of Korea using comparative analysis among some OECD countries. Although the Ministry of Health and Welfare has been expanding the scope of pre-term birth support system for pre-term birth since 2000, it is still insufficient in a broader context. Premature support policies in developed countries can be categorized by establishment of healthcare infrastructure and family support on medical, economical and others. From the governance perspective, it is needed to: increase national budget; establish national long-term strategy; provide prevention program during before and during pregnancy; increase R&D investment; produce basic statistics; establish evidence based policy; establish prenatal administration; and provide subsidy for the installation equipment in NICU and professional capacity building programs. Several ways can be considered for reducing the burden on families with pre-term children, such as development of guideline for pre-term birth care, encouragement of family participation, and long-term plan for premature babies as medical aspects, supporting the medical cost at NICU, subsidy for RSV vaccination, and supporting medical and rehabilitation cost after discharge from hospital as economical aspects. Lastly, prolonging the maternity leave or providing special leave for premature parents, aiding for child care, and psychological and emotional support can be also possible as prematurity support policy.
