고용형태별 및 성별에 따른 미충족 의료경험의 차이
Differences in Unmet Healthcare Needs by Employment Status and Gender

Abstract

In the Korean labor market, due to the proliferation of precarious employment and long working hours, there is a problem where workers do not have access to use medical care despite their need for medical treatment due to economic burden or lack of personal time. In particular, polarization and inequality of the labor market by gender is getting worse. This study aims to analyze the difference in unmet healthcare needs according to workers’ employment status and gender by reflecting the characteristics of the labor market in Korea. We used the Korea National Health and Nutrition Examination Surveys from 2015 and 2016 and targeted 6,174 workers aged 19 years or older. A logistic regression analysis was conducted to determine the effect of workers’ employment status on unmet healthcare needs, and a stratified analysis was performed to compare gender differences. As a result, men were found to have more unmet healthcare needs in non-regular workers (OR = 1.519, CI = 1.052-2.194) and self-employed and unpaid family workers (OR =1.499, CI = 1.015-2.214) than in regular workers, while women had fewer unmet healthcare needs in non-regular workers (OR = 0.653, CI = 0.477-0.893) than in regular workers. These results indicate that polarization by employment status and gender can lead to inequality in medical utilization. Therefore, it is necessary to establish a policy basis to improve unmet healthcare needs by employment status that varies according to gender and to devise a plan to increase workers’ access to medical care utilization.

우리나라는 불안정고용의 확산과 긴 노동시간 등 노동환경의 취약성으로 인해 근로자가 의학적 치료가 필요함에도 불구하고 의료이용을 할 수 없는 문제가 발생한다. 특히 여성 근로자의 노동시장 양극화와 불평등은 더욱 심화되고 있다. 이에 본 연구는 한국의 노동시장 특성을 반영하여 근로자의 고용형태 및 성별에 따른 미충족 의료경험 여부 차이를 분석하고자 하였다. 분석자료는 2015, 2016년도 국민건강영양조사를 이용하였으며, 만 19세 이상 근로자 총 6,174명을 대상으로 하였다. 근로자의 고용형태가 미충족 의료경험에 미치는 영향을 확인하기 위해 로지스틱 회귀분석을 실시하였으며, 성별에 따른 차이를 비교하기 위해 계층화분석을 수행하였다. 분석결과, 남성은 정규직에 비해 비정규직(OR=1.519, CI=1.052-2.194)과 자영업자・무급가족종사자(OR=1.499, CI=1.015-2.214)에서 미충족 의료경험이 증가한 반면, 여성은 정규직에 비해 비정규직(OR=0.653, CI=0.477-0.893)이 미충족 의료경험이 낮았다. 이러한 결과를 통해 고용형태와 성별에 따른 양극화가 의료이용의 불평등을 야기할 수 있음을 보여주었다. 따라서 성별에 따라 달라지는 고용형태별 미충족 의료 경험을 개선할 수 있는 정책적 기반을 마련하고, 근로자들의 의료 접근성을 높일 수 있는 방안을 강구하여야 할 것이다.

Abstract

This study investigates whether the wage effects of childbirth vary across the wage distribution. It is generally argued that women pay motherhood penalties and men earn fatherhood premiums when they become a parent. However, recent studies found that these parenthood effects differ across the wage distribution. To explore such variation across the income distribution, this study applies unconditional quantile regression and mixed effect model using 1-21 waves of the Korean Income Panel Study. The results show that both men and women pay the parenthood penalty in lower-wage groups while they earn parenthood premiums in higher-wage distribution. This means that the transition to parenthood aggravates income inequality. It implies that the transition to parenthood is deeply correlated with the social structure of inequalities.

본 연구는 임금 분포별로 부모로의 이행이 임금에 미치는 영향을 확인해보았다. 통상 여성은 어머니가 되면서 임금 불이익을 경험하며, 남성은 아버지가 되면서 임금 이익을 얻는 것으로 알려져 있다. 그러나 이러한 모성 패널티와 부성 프리미엄이 모든 임금 분포에서 동일하지 않은 점이 최근 서구 선행연구들에서 관측되고 있다. 이에 본 연구에서는 한국노동패널 1~21차 자료에 응답한 25~45세 남녀의 자료를 토대로 무조건부분위회귀분석과 고정효과모형을 적용하여 부모됨이 임금 분포별로 어떻게 다르게 영향을 미치는지를 확인하였다. 그 결과, 남성과 여성 모두 부모가 되거나, 자녀수가 늘어날 때 임금이 낮은 집단은 패널티를, 임금이 높은 집단은 프리미엄을 얻었다. 즉, 부모로의 이행이 남성과 여성 모두 임금 불평등을 가중시키는 방향으로 작동하는 것이다. 이러한 결과는 통상 임금이 높은 이들이 자녀를 낳는 경향에 더불어 자녀를 낳는 생애과정이 사회의 불평등 구조와 밀접하게 연결되어 있음을 시사한다.

Abstract

One of the phenomena to be noticed in Korean society is that individual demographic behavior is gradually spreading in an unequal way. The transition and timing of the life-course events such as dating, marriage and childbirth is determined by family background, leading to inequality in demographic behavior. In this context, this study analyzed how the demographic behavior pattern of marriage differs according to the socio-economic resources of the family, using data from the 1st to 19th year of the KLIPS. According to the results, it was found that the difference in the marriage rate between groups according to economic resources such as household income and financial assets of parents had a statistically significant effect on the marriage of unmarried men and women in Korean society. In particular, it was confirmed that the effect of family economic power is positive for the marriage in the marriageable age, and is greater in men than women. These results show that marriage in Korea society is becoming an inequality phenomenon determined by the socio-economic background of the family.

최근 한국 사회에서 발생하고 있는 여러 현상 중 주목해야 할 점은 개인의 인구학적 행위가 점차 불평등한 방식으로 확산되고 있다는 것이다. 연애와 결혼, 그리고 출산으로 이어지는 일련의 생애과정 이행 및 속도가 귀속 자원에 따라 결정되고 있으며, 이는 인구학적 행위의 불평등 현상으로 이어지고 있다. 이러한 맥락에서 본 연구는 가족의 사회경제적 자원에 따라 결혼이라는 인구학적 행위가 어떠한 차이를 갖는지 한국노동패널 1차년도부터 19차년도 자료를 활용해 분석해 보았다. 분석 결과에 따르면, 한국 사회 미혼남녀의 결혼 가능성은 부모의 가구소득 및 금융자산과 같은 경제 자원에 따라 집단 간 차이가 존재하고, 이는 통계적으로 매우 유의한 것으로 나타났다. 특히 부모의 경제력 효과는 결혼 적령기에 속한 미혼남녀의 결혼 이행에 긍정적인 영향을 미치고, 여성보다 남성에서 그 효과가 더욱 큰 것으로 확인됐다. 이 같은 결과는 한국 사회 결혼 현상이 불평등의 결과로 나타나고 있다는 것을 보여준다.
