Abstract

초록

본 연구는 개인의 행동변화가 외부의 자극 즉 문제인식으로부터 출발한다는 일반적인 이해 하에서, 고혈압이나 당뇨의 발생에 대한 인지가 흡연이나 음주행태에 영향을 미칠 것이라는 가정을 구체적으로 검증해 보고자 시도되었다. 여기에는 흡연과 음주가 우리나라에서 악성신생물(암)에 이어 사망률의 수위를 차지하고 있는 뇌·심혈관질환의 주요 위험요인으로서 또 다른 위험요인인 고혈압 및 당뇨와도 관련되어 있는 점, 이들 질환은 발병 초기에 자각증상을 거의 나타내지 않음에 따라 검진을 거치기까지 인지되지 못한 상태로 남아 있게 되는 점, 건강검진의 중요성에 대한 인식증대 속에서 고혈압과 당뇨에 대한 검진이 최근 비교적 활발히 이루어지고 있는 점, 건강생활실천사업과 지역보건사업을 통해 고혈압과 당뇨에 대한 흡연 및 음주의 위해성이 널리 홍보되고 있는 점, 그리고 마지막으로 기존의 흡연이나 음주행태에 관한 연구들이 고혈압과 당뇨 등 관련질병의 발생 후 인지 및 치료여부를 구분하지 않고 수행된 점 등이 바탕으로 자리하고 있다. 이에 2001년 국민건강·영양조사에서 건강면접조사, 보건의식행태조사, 검진조사를 완료한 20세이상 성인 5,367명 가운데 고혈압이나 당뇨를 지닌 1,506명(전체의 28.1%)의 자료를 분석한 결과, 고혈압이나 당뇨를 지니고 있으면서 그 사실을 인지하고 치료 중에 있는 경우는 남자 25.2%, 여자 40.7%로서 매우 낮은 가운데, 연령 및 개인의 사회경제적 특성 등을 상호 보정한 로지스틱 회귀분석으로부터 고혈압·당뇨의 발생을 인지하고 치료하는 경우의 흡연확률과 음주확률은 그렇지 않은 경우와 비교하여 유의하게 낮은 것으로 파악되었다. 연구결과는 주요 사망원인질병의 위험요인인 흡연 및 음주 관리에 있어서 건강검진의 중요성을 부각시키는 것으로, 그 중 고혈압·당뇨에 대한 검진은 소요비용이나 가용인프라 측면에서 타 질환 검진에 비해 접근성이 높으며 인지치료율 제고를 통해 흡연율이나 음주율 저하는 물론이고 그 자체로 뇌·심혈관질환의 발생이나 악화의 위험을 낮출 수 있는 점에서 우선적인 관심을 필요로 한다. 그밖에도 본 연구를 통해 흡연과 음주행태 간 상호 밀접한 관련성이 다시한번 입증되었으며, 이는 흡연율보다 높은 수준의 음주율 그리고 흡연율의 감소추세에 대비한 음주율의 상승추세 등과 더불어 향후 금연·금주/절주사업이 보다 연계적으로 추진되고 음주자중심에서 전개되어야 할 필요성을 강하게 시사하고 있다.;Smoking and alcohol drinking have been described as major preventable risk factors of cardiovascular diseases(CVD), a worldwide leading cause of death, and of hypertension and diabetes which also have strong association with CVD. Accordingly, policies to promote people’s health have priority to reduced rate of smoking and alcohol drinking and to early detection and treatment of those diseases. In Korea, there have been a clear reduction in smoking population among male aged 20 and over, and an increase in check-up for hypertension and diabetes since National Health Promotion Act of 1995. However, both the smoking population among female rather increase and the rate of awareness of his(her) own hypertension and diabetes stay at low level indicate the need to further development of program strategies. This study is designed to adapt a health promotion theory which describe the influence of cognitive factor on behavioral change, as against former studies on health behavior have mostly conducted regardless of the perceived prevalence of related diseases. Thus, it attempts to identify the fact that the awareness of hypertension/diabetes from regular check up affect smoking and alcohol drinking. Health interview and examination data for total of 1,506 persons who have hypertension/diabetes among aged 20 and over in 2001 National Health Interview and Examination Survey are employed. Descriptive and logistic regression analyses are conducted and produce meaningful results; only 25.2% of male and 40.7% of female who currently have hypertension/diabetes aware and treat it(them); the awaring and treating their hypertension/diabetes show significantly low rate of smoking and alcohol drinking compare to those neither aware nor treat; smoking increases the probability of alcohol drinking and vice versa. Based on the findings, it is recommended that health promotion programmes put more emphasis on health examination especially on regular check up for hypertension/diabetes with considerations of accessibility and significance. In addition, interrelationships among health behaviors suggest the need to link two programs of smoking and alcohol drinking together while aiming at abstaining from smoking of who drink with smoke.
