이 연구는 재난적 의료비 지출이 가구의 재정운영과 소비지출, 그리고 빈곤에 미치는 영향을 검증하는 것이 목적이다. 한국복지패널 3차(2008년)~8차(2013년)년도 조사에 지속적으로 참여한 5,155개 가구를 대상으로 기술분석과 패널분석을 실시하였다. 재난적 의료비는 2011년 이후 발생률이 지속적으로 증가하여, 2013년에는 19.3%였다. 패널분석 결과, 재난적 의료비 지출은 사적이전과 민간보험수령액과 유의한 양(+)의 연관성이 있는 반면, 저축액과는 음(-)의 연관성이 있었다. 가구 유형별로는 빈곤 계층은 사적이전을 이용하는 반면, 비빈곤 계층은 사적이전, 보험수령액, 저축액 등 다양한 대응을 선택하고 있었다. 재난적 의료비 발생 가구는 그렇지 않은 가구보다 비-의료서비스의 소비지출이 유의하게 낮았으나, 가구 유형별로는 비비곤 계층에서만 유의하게 낮았다. 또한 재난적 의료비 발생가구는 그렇지 않은 가구보다 빈곤 상태에 있을 확률이 높았다. 재난적 의료비 발생의 증가와 그로 인한 가구의 사적이전의 증가 및 소비지출 감소, 그리고 빈곤화는 한국의 의료보장제도가 사회안전망으로써 충분한 역할을 하지 못한다는 것을 의미한다. 또한 재난적 의료비에 대한 재정적 대응이 가구의 경제 수준별로 다른 양상을 보이고 있어, 재난적 의료비 발생을 줄이기 위한 정부의 의료보장제도의 개선뿐만 아니라 취약 계층을 고려한 정책을 추가적으로 마련 할 필요가 있다.;The objective of this paper was to examine the effects of CHE on financial coping and poverty of households in South Korea. Using panel data of 5,155 households that participated over a 6 year period (2008-2013) in the Korea Welfare Panel. The incidence of CHE in South Korea increased overall between 2011 and 2013. The results showed that private transfer income and private insurance benefits were significantly positively correlated with CHE, but savings was significantly negatively correlated with CHE. Especially, poor households were more likely to rely on private transfer income, while non-poor households were likely to finance CHE through health insurance, savings and private transfer income. Non-medical consumption expenditure was significantly lower in households with CHE than in those without CHE, but this result was statistically significant only in non-poor households. Households with CHE showed significantly higher risk of facing poverty than households without CHE. The high rates of CHE and impoverishment resulting from CHE can be implied that the medical safety net in South Korea was insufficient. It is necessary that a policy should be developed for the protection of vulnerable households, as well as the reinforcement of South Korea"s health security system to reduce the risk of catastrophic health expenditure.
