Abstract

초록

이 연구는 흡연으로 인한 기대여명 손실량을 추정함으로써, 흡연으로 인한 질병과 사망에 대한 대책을 마련하기 위한 기초자료를 제공하기 위한 것이다. Peto-Lopez Method로 흡연 관련 사망수를 추정하고, 통계청 생명표작성법에 따라 흡연에 따른 기대여명관련지표를 산출하였다. 분석을 위해 사용된 연구 자료는 American Cancer Society의 Cancer Prevention Study-Ⅱ의 Lung cancer mortality와 Relative Risk(RR), 한국 통계청의 2002년 생명표, 2002년 사망원인 통계, 2002년 주민등록 인구이다. 남성의 경우 흡연으로 인한 YLL(Years of Life Lost)은 398,592 인년이며, 여성은 128,516인년으로 남성의 YLL이 약 3.1배가량 더 크다. 남성의 YLL 이 가장 큰 연령대는 60대로 전체 YLL의 약 33.3%를 차지하였다. 사인별로는 남성의 경우 폐암(109,525년), 심혈관계 질환(90,882), 기타 사인(83,612)순이었고, 여성은 기타 사인(18,419), 심혈관계 질환(17,472), 만성하기도 질환(16,208)순이었다. 2002년 흡연으로 인한 사망이 제거된다면 40세 기대여명은 남성 38.84년, 여성 42.61년, 70세 기대여명은 각각 14.51년과 15.42년이 될 것으로 기대된다. 흡연으로 인한 40세 기대여명 손실은 남성의 경우 3.73년, 여성의 경우 1.00년이다. 연구의 한계는 미국(CPS-2)의 RR을 사용한 것, 폐암을 흡연 영향(SIR, Smoking Impact Ratio)의 단일지표로 사용한 것, 간접흡연, 대기오염 등으로 인해 비흡연자의 폐암 사망률이 미국과 다를 수 있음에도 CPS-2의 비흡연자 폐암 사망률을 그대로 사용하여 정확하지 못한 SIR을 사용한 것 등이다.;The aim of this study is to emphasize importance of accumulated hazards of smoking by providing estimates of smoking-attributable loss of life expectancy. Peto-Lopez’s indirect method was used to calculate smoking-attributable deaths in Korea. Life table functions were calculated as proposed by Korea National Statistical Office. Korean vital statistics are available on-line in KOSIS database, and, as Peto did in his original study, relative risks and lung cancer mortality of never-smokers were acquired from ACS CPS-Ⅱ. Smoking-attributable YLL was 398,592yrs for males and 128,516yrs for females. Ages 60-69 attributed 33.3% of male YLL being the most attributable age group. With respect to causes of death, lung cancer(109,525yrs), vascular diseases(90,882yrs), other medical causes(83,612yrs) attributed mostly for males, and other medical causes(18,419yrs), vascular diseases(17,472yrs), COPD(16,208yrs) for females. At age of 40, life expectancy with no smoking-attributable deaths is 38.95yrs(3.73yrs gained) for male and 42.69yrs(1.00yrs gained) for female. Limits of this study are as follows: first, absolute lung cancer mortality was used as a single indicator of maturity of smoking, and second, we assumed that lung cancer mortality of never smokers are same as in ACS CPS-Ⅱ.
