본 연구는 부산광역시 S노인요양시설에서 2013년 4월부터 2014년 1월까지 약 10개월에 걸쳐 SQI팀을 중심으로 수행된 리스크매니지먼트 시스템 구축 활동과 성과에 대한 사례연구이다. 본 연구를 시작하게 된 계기는 노인요양시설에 발생하는 다양한 사고를 사전에 예방하고 발생한 사고에 효과적으로 대응하기 위한 관리기법으로서 체계적인 위험관리 방법과 수행활동을 마련하기 위한 것이다. 특히, 본 연구는 시설 내 팀장들이 직접 참여하여 인적, 물적 위험요인들을 파악하고 스스로 문제해결을 위한 대응책을 제시해 나가는 Bottom-Up 방식의 SQI활동을 시도하였다는데 커다란 의의가 있다. 본 연구의 결과, 우선 노인요양시설에서 리스크매니지먼트 시스템이 시급히 구축되어야 할 것이며, 구축방법으로서 SQI팀을 중심으로 위험관리 과정에 따라 단계별로 구축해 나가는 것이 보다 용이하다고 사료된다. 또한, 리스크매니지먼트 시스템 구축을 위한 SQI 활동 결과에 대한 효과성을 파악하기 위해 전 직원에 t-test를 실시한 결과, 위험인식 수준과 기본자세, 위험사고 대응의 지식과 위험사고 대응의 절차, 그리고 위험관리 조직 및 기능 세 영역 모두에서 유의미한 변화(pqqqlt;.05)가 나타났다. 그리고 위험관리 과정 중 1단계인 위험의 인식과 확인이 가장 중요한 단계임을 알 수 있었고 가장 도움이 된 위험관리 기법은 인시던트 보고서와 위험노출도 작성, 4M-4E, SHELL 분석으로 나타났다. 따라서 노인요양시설에서 리스크매니지먼트 시스템 구축을 위해서는 먼저 SQI팀을 조직하여 위험관리에 대한 사전교육과 위험관리 기법에 의한 과학적인 위험분석과 체계적인 대응책을 마련한 후, 전 직원에게 위험관리 수행활동을 전개해 나가도록 하는 것이 보다 효과적임을 알 수 있었다. 또한 향후, 요양보호사들은 위험관리 민감성이라는 조직문화를 정착시켜 나가도록 하고, 정부는 법적, 제도적으로 노인요양시설에 리스크매니지먼트 시스템을 구축하도록 노력해야 할 것이다. This study is a case study of the activities and outcomes to establish the risk management system in an elderly care facility. It was conducted during 10 months from April in 2013 to January in 2014 in S elderly care facility in Busan metropolitan city. The motivation to conduct this study was to set up the systemic risk management practice and method to prevent and cope with various accidents that have been taking place at elderly care facilities. The results of this study suggest that the risk management system should be established ungently in every elderly care facility and that it be considered effective to follow the risk management procedure conducted mainly by the SQI(Service Quality Improvement) team as a establishment methode. According to the results, based on the results of the paired sample t-test by first and second survey of all staffs to explore the effectiveness of the SQI team activities for the establishment of the risk management system, there are significant changes and rising awareness in all three areas of risk management. In addition, the most significant step of risk management process is every staff’s risk recognition and identification as the first step of risk management procedure and the most technical methods are incident report, 4M-4E, and, SHELL analysis. Over all, this study concludes that in order to establish the risk management system in elderly care facilities, it is effective that the SQI team be firstly organized and come up with scientific risk analysis and systemic solutions, and then make every staff practice newly developed risk management performing activities. Also, for the future, the sensitivity for the risk management should be fostered among elderly care workers as a organization culture and governmental policies to establish the risk management system in elderly care facilities should be enacted and enforced.
