본 연구는 65세 이상 노인의 의료 서비스 이용, 의료비 지출 및 건강성과의 발달궤적, 발달궤적의 상호관계 그리고 발달궤적에 영향을 미치는 예측요인을 파악하고 이들 변수에 대한 연령차를 확인하기 위하여 Andersen의 행동모형에 근거하여 한국의료패널의 2009년부터 2013년 까지 5년 연속으로 설문조사에 참여한 2,267명의 자료를 분석하였다. 다변량 잠재성장모형(multivariate latent growth model)을 이용하여 각 변인의 변화와 관련성을 추정하고, 다중집단분석(multi-group analysis)을 통하여 초기노인(young-old)과 후기노인(old-old)집단의 차이를 분석하였다. 분석결과, 우리나라 노인은 연령이 증가함에 따라 입원서비스 이용과 의료비 지출은 선형으로 증가하고 주관적 건강상태 또한 선형으로 나빠지는 것으로 나타났고, 시간이 지나면서 점차 개인 간 격차는 줄어 평준화되는 경향이 나타났다. 입원서비스 이용, 의료비 지출 및 주관적 건강상태의 발달궤적에 가장 많은 영향을 미치는 요인은 만성질환, 장애유무, 그리고 통증이나 불편감과 같은 필요요인으로 나타났다. 연령별 차이를 살펴보면, 초기노인은 시간이 지남에 따라 입원서비스 이용과 의료비 지출, 주관적 건강상태에 있어 개인 간의 차이가 점차 줄어들어 평준화 경향이 발견되었으나, 후기노인은 입원서비스 이용과 주관적 건강상태의 개인 간 차이가 점차 확대되었다. This study was to investigate the developmental trajectories of medical service utilization, medical expenses and health outcome of elderly people over 65, to identify the longitudinal relationship, to examine the determinants which influence to the developmental trajectories, and to investigate the differences of these three factors and their relationships young-old and old-old age groups. Data were drawn from the Korea Health Panel and the study sample consisted of 2,276 subjects over 65 who participated in the survey from 2009 to 2013. Latent Growth Model (LGM) and Multi-Group Analysis was performed. The utilization of inpatient services and medical expenditure increased and the perceived health status declined over time. The major determinants on the developmental trajectories of inpatient service utilization, medical expenditure and health status were the needs factors of Andersen’s health behavior model, which were disabled, chronic disease and pain and/or discomfort. In the young-old group, the individuals on the high score of intercept in inpatient service utilization and perceived health status had lower rate of incremental changes of these factors, which mean the gaps of these factors between individuals had been lessened over time. However, the gaps were greater for the old-old age group.
