연구목적: 본 연구의 목적은 장애와 기능상태의 평가에 사용되고 있는 국제기능장애건강분류(ICF)를 국내 정신질환자들에게 적용하여 활용가능성을 알아보고자 한 것이다. 정신기능18개, 활동과 참여 25개, 환경요인 16개 등 총 59개 코드를 평가 대상 ICF코드로 선정하고, 충청북도에 소재한 일개 정신건강의학과 병원의 입원환자 100명을 대상으로 평가값의 조사자간, 조사-재조사간 신뢰도를 평가하였다. 신뢰도 평가는 단순카파계수(simple kappa coefficient)와PABAK (Prevalence Adjusted Bias Adjusted Kappa)를 사용하였다. 신뢰도 평가 결과, 정신기능에서는 의식기능과 지남력기능이 0.72∼0.98로 신뢰도가 높았지만, 자아와 시간의경험기능 등에서는 신뢰도가 낮았다. 활동과 참여에서는 보기 0.98∼1, 듣기 0.82∼1, 보행0.98∼1, 대소변 처리 0.88∼1, 먹기 0.82∼0.92, 마시기 0.96∼1) 등이 신뢰도가 높은 반면, 대인관계 영역에서 매우 낮은 신뢰도를 보였다. 환경요인에서는 자연환경과 인위적 환경변화 0.76∼0.98, 지원과 관계 0.62∼0.94로 신뢰도가 높았던 반면, 태도영역에서는 신뢰도가낮았다. 전체적으로는 평가값의 신뢰도가 높지 않아 정신질환자를 대상으로 ICF를 적용하기 위해서는 평가값에 대한 구체적인 정의와 교육을 통해서 조사자의 숙련도를 높일 필요가있다. Purpose: We used the International Classification of Functioning, Disability, and Health (ICF)to assess the disability and functional status of Korean patients with mental illness. Weselected 59 ICF codes including 18 codes in mental function, 25 codes in activities and participation, and 16 codes in environmental factors. Inter-rater and test-retest reliabilities of the ICF code qualifiers were assessed in 100 patients admitted to a mental health hospital in Chungbuk province using a simple kappa coefficient and PABAK (Prevalence Adjusted Bias Adjusted Kappa). In mental function category, the reliabilities for the consciousness functions and orientation functions were high, ranging from 0.72 to 0.98, whereas those for global psychosocial functions and the experience of self and time functions were low. In the activities and participation category, the reliabilities were high for watching (0.98-1), hearing (0.82-1), mobility (0.98–1), toileting (0.88–1), eating (0.82–0.92), and drinking (0.96–1).However, the reliabilities for the interpersonal interactions and relationships items were significantly lower. In environmental factors category, the reliabilities ranged from 0.76 to 0.98 for natural environment and human-made changes to environment items and from 0.62 to 0.94 for the support and relationship items. However, the reliabilities for attitude items were low. The overall reliability was unsatisfactory. To apply ICF to the patients with mental illness, it is necessary to specify the definition of qualifiers and increase investigators' skills in ICF evaluation.
