The Development of an Evaluation Model for Korea's Community Nutrition Program - With Special Reference to Experiences in Selected Countries -

ꏚ Objectives
The objectives of this study were to examine the current evaluation system for the community nutrition programs in Korea's National Health Promotion Project and to develop a new and comprehensive model. The study was focused on a number of issues concerning community nutrition services, including workforce, facilities and activities for the existing community nutrition programs and evaluation scheme in public health centers and local governments.
ꏚ Method
The data were collected from various sources. First, three mail surveys were conducted for health centers (comprehensive survey), workers at health center nutrition divisions (basic survey), and nutrition service workers (attitude survey). In addition, information was collected from health centers, provincial governments(Si-Do), and the Ministry of Health and Welfare, on their evaluation of community nutrition programs. Second, interviews were conducted on the focus groups of professors, professionals, and health center nutrition workers, to get their opinions on the models, indicators, and application of nutrition programs. Third, information was collected on the development of community nutrition evaluation programs in the U.S. and Japan, including CNE(Community Nutrition Education) Logic Model and 「Health 21 Japan」.
ꏚ Results
◦ Community Nutrition Programs Evaluation in Korea
Out of 246 health centers across the country, 151 health centers responded to the comprehensive survey; 309 health center nutrition program workers to the personnel survey; and 270 nutrition service workers to the attitude questionnaire.
Each health center had an average of 2.4 nutrition workers, 54% of whom were nutritionists;
many of the others were licensed nurses. Only half of their working hours were allocated for the nutrition-related tasks and the remainder for other works such as obesity programs, administrative affairs, and etc.
An average number of 7.7 nutrition programs were being conducted with a limited workforce.
The main programs were for the infants and children. Most of nutrition programs were settled in a short period, focusing on individual level, not on social structure and social policy level.
Some works required for planning and evaluation, such as diagnosing the community, matching the objectives and evaluation indexes, and applying a theoretical model to practices, were negatively rated.
Required programs and selective programs recommended by the Ministry of Health were positively evaluated by many respondents, but negatively evaluated by many others as well. The current local governments' evaluation system, which applied rating scheme on execution required programs or not, was not preferred by the nutrition service workers.
The main complaints for the current evaluation method were unclear evaluation criteria,quantity-based, inflexibility, and so on.
◦ Community Nutrition Programs Evaluation in the U.S. and Japan
CNE(Community Nutrition Education) Logic Model, which was applied for Food Stamp Program in the U.S.A., was considered to be a bench-marking model for development of community nutrition evaluation program in Korea, because it focused on outcome-based and objective-related evaluation, which included not only individual level but also organization level and social structure/policy level. CNE also covered various kinds of programs in different regions.
「Health 21 Japan」 was considered as another bench-marking model, because Japan had public health system similar to that of Korea, and its health promotion program was conducted through a process similar to the Korean system. The examining of their evaluation system gave us ideas; their long-term evaluation for some indicators and their flexible application for the evaluation should be considered as valuable implications for our system.
ꏚ Conclusion:
The objectives of community nutrition programs should conform to those of 「Health Promotion 2010, Korea」so that the outcomes of the programs including the evaluation could be effective and productive.
The results of this study can be used to help improve the nutrition evaluation system in Korea with its analysis of the current system of the evaluation and suggestions of conceptual models,modes of application, and a long-term nutrition monitoring system.
