Health Monitoring Systems for Disaster Victims in the US and Japan and Their Policy Implications

A health monitoring system for disaster victims, currently lacking in Korea, would enable the identification of the long- and short-term health impacts of disasters, as well as the extent and patterns of these impacts on different population groups. This article examines cases from the US and Japan where national-level efforts, utilizing registries of victims or community resident cohorts, have been made to identify the physical and mental health impacts of disasters. Both the US and Japan have implemented support and policy measures based on findings from their monitoring of disaster victims. The fact that these findings are shared with policymakers, researchers, and the general public, for both academic and policymaking purposes, has significant implications for Korea.

재난 피해자에 대한 건강 모니터링 체계가 구축되면 재난의 장단기적인 건강 영향을 파악할 수 있을 뿐만 아니라 인구집단별 재난 피해 양상을 보다 구체적으로 파악할 수 있지만, 우리나라는 아직 재난 피해자에 대한 체계적인 모니터링이 부족한 실정이다. 이 글에서는 대규모 재난 발생 이후 국가 단위에서 피해자 레지스트리나 지역 주민 코호트를 구축하여 재난으로 인한 신체적, 정신적 건강 영향을 확인하고 있는 미국과 일본의 사례를 소개한다. 미국과 일본은 재난 피해자 모니터링 결과를 바탕으로 피해자에게 필요한 지원과 정책을 마련하고 있으며, 더 나아가 이를 정책 입안자, 연구자, 대중과 공유하여 정책적･학술적으로 활용하고 있다는 점에서 우리나라에 시사하는 바가 크다.

To ensure healthy pregnancies and childbirths, it is important that interventions are made early on, building on an understanding of risk factors that may start having effects on pregnancy even before conception. As the importance of preconceptual health has been increasingly emphasized at the international level, many countries have developed guidelines and policies for preconception care. China and Japan are examples of countries where preconception care services are provided at both national and local levels in a comprehensive range, encompassing education, health checkups, and consultations. This article examines these preconception care services and discusses their implications for policymaking in Korea.

건강한 임신과 출산을 위해서는 임신 전부터 임신에 영향을 미칠 수 있는 위험요인을 인지하고 개입하는 것이 중요하다. 국제적으로 임신 전 건강관리의 중요성이 강조됨에 따라 여러 국가들에서 임신 전 건강관리를 위한 지침이나 가이드라인을 개발하고 정책을 마련하고 있다. 특히 중국과 일본은 국가 및 지자체 차원에서 제공하는 임신 전 건강관리 서비스를 통해 임신 전 건강관리에 대한 교육과 건강검진, 상담에 이르기까지 포괄적인 지원을 하고 있다. 이 글에서는 중국과 일본의 임신 전 건강관리 지원 사례를 중심으로 국제 동향을 살펴보고, 한국의 임신 전 건강관리 지원 정책에 대한 시사점을 도출하고자 한다.
