At the forefront of international health discourse is the perspective that health should be viewed less as a byproduct of economic growth and more as an objective that economies should actively pursue—a foundation for sustainable development. The World Health Organization (WHO) adopted the Health Economics for All strategy (2026-2030) at the 79th World Health Assembly earlier this year, building on the rationale set out in Health for All: Transforming Economies to Deliver What Matters, a 2023 report by the WHO Council on the Economics of Health for All, which was established in 2020. The strategy, in which health is defined not as a cost but as an investment, proposes reshaping economic policies to prioritize health and sets out five strategic directions, along with the corresponding roles of Member States and the WHO Secretariat. Reviewing the main arguments of the Council’s aforementioned report, which serves as a foundation for the strategy, as well as key elements of the strategy, including its vision and eight guiding principles, this article synthesizes the positions that Member States held at the Executive Board and the World Health Assembly and considers the implications for Korea.
건강을 경제성장의 부산물로 보던 관점에서 벗어나 건강을 경제가 지향해야 할 목표이자 지속가능한 발전의 토대로 보는 시각이 국제 보건 논의의 중심으로 떠오르고 있다. 세계보건기구(WHO)는 2020년 설치한 ‘모두를 위한 보건경제위원회’가 2023년 발간한 ‘모두를 위한 건강: 중요한 것을 실현하는 경제로의 전환’ 보고서를 토대로 2026년 제79차 세계보건총회에서 ‘모두를 위한 보건경제 전략(2026-2030)’을 채택하였다. 이 전략은 건강을 비용이 아니라 투자로 규정하고, 건강을 우선하는 방향으로 경제정책을 재설계할 것을 제안하며 다섯 가지 전략적 방향, 회원국 및 사무국의 행동을 담고 있다. 이 글에서는 전략의 토대인 위원회 최종 보고서의 핵심 논지와 전략의 비전, 8대 핵심 원칙 등 주요 내용을 검토한 뒤 집행이사회와 세계보건총회에서 이루어진 회원국의 입장을 종합하고 한국에 주는 시사점을 짚어 본다.
The UK Department for Education launched the “Giving Every Child the Best Start in Life” strategy in July 2025, with the aim of closing early developmental gaps and ensuring a more equal start to school. This strategy seeks to provide integrated support across family services, health care, childcare, and early education from pregnancy through to school entry, with the goal of increasing the proportion of 5-year-olds who achieve a Good Level of Development (GLD) from 68 percent in 2025 to 75 percent by 2028. The strategy also aims to establish up to 1,000 Best Start Family Hubs and as many as 2,000 connected services by 2028, provide eligible parents with 30 hours of funded childcare per week for children aged 9 months and older, and strengthen health care support through Healthy Babies services. Under the initiative, priority has been given to disadvantaged areas in the establishment of Family Hubs, which are open to all families, to improve access to services, providing continuous support and integrating local-level performance management with digital and face-to-face services. These integrated early-years support policies in the UK have useful implications for the development of a child support policy framework in Korea.
영국 교육부는 2025년 7월 아동의 발달 격차를 줄이고, 평등한 학교 출발선을 목표로 ‘모든 아이에게 최고의 출발을(Giving Every Child the Best Start in Life)’ 전략을 발표하였다. 이 전략은 임신기부터 만 5세까지 가족서비스, 보건의료, 영유아 보육 및 교육을 통합 지원하고, 2028년까지 만 5세 아동의 적정발달수준(GLD: Good Level of Development) 달성률을 현재 약 68%에서 75%로 높이는 것을 목표로 한다. 정부는 2028년까지 베스트 스타트 패밀리 허브(BSFH: Best Start Family Hubs) 최대 1000곳과 2000개의 네트워크 거점을 조성하고, 생후 9개월부터 주 30시간의 영아 무상보육 확대, 헬시 베이비(Healthy Babies) 서비스를 통한 보건의료 지원 강화를 추진한다. 또한, 모든 가정이 이용할 수 있는 허브를 취약지역에 우선 배치해 접근성을 높였으며, 임신기부터 취학기까지 지원을 연속적으로 제공하고, 지역 단위 성과관리 및 디지털·대면 서비스의 결합을 함께 시도하고 있다. 이러한 영국의 생애 초기 통합 지원 정책은 한국의 아동지원정책 체계 구축에도 유의미한 시사점을 제공할 것이다.
This article examines the key findings of the evaluation report on Germany’s basic pension supplement (Grundrentenzuschlag), introduced in 2021, and draws implications for Korean’s pension reform. According to the evaluation report, as of 2023, 12.29% of old-age pension recipients qualify for the supplement; however, the income test disqualifies nearly half of all eligible recipients (49.5%) from actually receiving it, and the average supplement among recipients ranges from 67 to 103 euros per month, depending on region and gender. The gender gap in receipt of the basic pension supplement is pronounced: while 18.20% of female old-age pension recipients and 4.57% of male recipients qualify for the supplement, the income test works to women's particular disadvantage — only 46.6% of eligible women actually receive the supplement, versus 70.9% of eligible men. The German basic pension supplement has demonstrated that it is institutionally feasible to support long-term, low-wage contributors through a dedicated supplement mechanism even within an earnings-related pension system; nevertheless, the operational evaluation makes clear that considerably more refined design is needed.
이 글은 독일에서 2021년 도입한 기초연금가산금제도의 시행 결과 평가 보고서의 주요 내용을 살펴보고, 시사점을 찾는 것을 목적으로 한다. 평가에 따르면 2023년 기준 노령연금 수급자의 12.29%가 기초연금가산금 수급 자격을 갖추고 있으나 소득조사로 인해 수급 자격자 중 절반(49.5%)이 가산금을 받지 못한다. 수급자의 평균 기초연금가산금은 지역·성별에 따라 67∼103유로 수준이었다. 기초연금가산금 수급에서 성별 격차는 뚜렷하였다. 노령연금 수급 여성 중 18.20%, 남성 중 4.57%가 수급 자격을 갖췄지만, 소득 심사가 여성에게 불리하게 작용하여 수급 자격자 중 수급 비율은 여성(46.6%)이 남성(70.9%)보다 현저히 낮았다. 독일 기초연금가산금제도는 소득비례 방식 연금에서 가산금 장치를 통해 저소득 장기 가입자를 지원하는 것이 제도적으로 가능함을 보여 주었지만, 운영 평가 결과 소득감액 방식에서 보다 정교한 설계가 필요함을 알려준다.
This study compares the social rights granted to immigrants in France by residency status and examines the issues raised by recent policy changes, including the 2024 immigration reform. France has long maintained a universal framework of social rights grounded not in nationality but in residency and human dignity, through the 1946 Constitution and the post-war social security system. Recent legislative and administrative measures, however, have increasingly linked access to social rights to legal residence and social contributions. The study reviews the protection of social rights across different categories of migrants—refugees, legally residing workers, international students, and irregular migrants. It concludes by discussing the implications of the French experience for South Korea, where social rights remain largely nationality-based, underscoring the relevance of a universal, dignity-based approach.
이 글에서는 프랑스의 체류 지위에 따른 이주민 사회권 보장 내용을 비교하고, 2024년 이민법 개정을 포함한 최근의 정책 변화가 이주민 사회권에 제기하는 쟁점을 분석한다. 프랑스는 전후 헌법과 사회보장제도를 통해 ‘국적’이 아니라 ‘거주’와 인간의 ‘존엄’에 기초한 보편적 사회권을 확립해 왔다. 그러나 최근 입법과 행정 조치는 사회권을 합법적 체류와 사회적 기여에 연동시키는 방향으로 재편되고 있다. 이에 따라 난민, 합법 체류 노동자, 유학생, 불규칙 이주민 범주별 사회권 보장 양상을 검토하였다. 이를 바탕으로 국적 중심 사회권 체계를 유지해 온 한국에 인간의 존엄에 기초한 보편적 사회권 보장의 함의를 제시하였다.
This article examines the manner in which Japan’s social assistance applies to the growing population of foreign nationals and draws policy implications for Korea. The principal issue identified is that livelihood protection, which under the spirit of Article 25 of the Constitution ought to function as a minimum guarantee, has not been recognized as a legal right but has instead been applied only mutatis mutandis to foreign-born residents. To provide background, this article contextualizes the terminology used to classify foreigners into different categories, changes in the scale and composition of the foreign-born population, and the various types of residence status. The article also examines how foreign-national residents, depending on whether or not they have registered residence, may have differing levels of access to social security in general and may be granted exceptions to rules excluding foreigners from livelihood protection. It provides an overview of the basic principles and operation of livelihood protection and presents the current criteria for its quasi-application to foreigners, linked to their residence qualifications. It also traces the formation of, and subsequent changes in, the Livelihood Protection Act, outlines the contours of relevant debates, and considers policy implications from the perspective of asking who constitutes the community of shared life on which social assistance ought to be based.
이 글은 일본의 외국인 체류가 확대되는 가운데 공공부조가 외국인을 어떻게 포괄하는지에 주목하여 일본의 외국인 생활보호 적용 체계를 현황, 역사, 쟁점으로 나누어 정리하고 한국에 대한 시사점을 도출하는 것을 목적으로 한다. 특히 생활보호가 헌법 제25조 이념에 근거한 최저생활 보장 제도임에도 불구하고 외국인에게는 권리가 아니라 행정상 ‘준용’으로 운영되어 왔다는 점을 핵심적인 문제로 설정한다. 그 배경을 이해하기 위해 외국인 관련 용어, 규모 변화, 체류 자격을 중심으로 현황을 정리하고, 사회보장제도 전반에서 주민등록 여부에 따른 접근 차이와 생활보호의 예외성을 검토한다. 이어 생활보호의 기본 원리와 운영을 개괄하고, 외국인 준용의 현행 기준을 체류 자격과 연계하여 제시한다. 또한 「생활보호법」의 형성 및 변형 과정을 추적한다. 끝으로 최근 논쟁 지형을 검토하고, 공공부조가 누구를 생활공동체의 구성원으로 상정하는가라는 관점에서 정책적 함의를 논의한다.
In this article, I examine the UK’s social assistance system as it applies to migrants and explore its implications for Korea. Migrants’ eligibility for social welfare benefits in the UK is primarily determined by two principles: the No Recourse to Public Funds (NRPF) condition and the Habitual Residence Test (HRT). Notably, since Brexit, the extension of the points-based immigration system to EU and EEA citizens has further intensified welfare exclusion linked to immigration status. Even within the framework of Universal Credit—introduced under the Welfare Reform Act 2012—barriers to benefit access have become more entrenched for migrants. Drawing upon UK government statistics on Universal Credit claims by immigration status, released for the first time in 2025, this article provides an empirical analysis of benefit receipt among migrant populations. With respect to policy implications for Korea, I suggest: (1) fostering public deliberation on eligibility criteria while streamlining administrative procedures; (2) developing a database on welfare benefit receipt by migrants to enable evidence-based policymaking; and (3) incorporating humanitarian exemption clauses.
이 글에서는 영국의 이주민 대상 공공부조 제도를 검토하고, 한국에 대한 시사점을 도출하였다. 영국에서 이주민의 복지 수급 자격은 공공기금 청구 금지(NRPF)와 상시 거주 요건(HRT)이라는 두 가지 원칙으로 규율된다. 특히 브렉시트 이후 유럽연합(EU)·유럽경제지역(EEA) 출신 시민에게도 포인트 기반 이민 제도(point-based system)가 적용되면서 체류 자격에 따른 복지 사각지대 문제가 더욱 심화되고 있다. 2012년 복지개혁법 이후 도입된 유니버설 크레딧(UC) 체계 내에서도 이주민의 수급 진입 장벽은 한층 공고해졌다. 영국 정부가 2025년 최초로 공개한 체류 자격별 유니버설 크레딧 수급 통계를 활용하여 이주민의 수급 실태를 실증적으로 분석하였다. 끝으로, 한국 사회에 대한 시사점으로 ① 수급 자격 기준의 공론화 및 제도의 효율적 운영, ② 이주민 복지 수급 실태 파악을 위한 정책 근거 데이터 구축, ③ 인도주의적 예외 규정의 도입이라는 세 가지를 제언하였다.
This article provides a cross-national comparison of social assistance as applied to migrants. The cases, including Korea, are classified into four types of welfare regime, social democratic, conservative, liberal, and East Asian, with three countries assigned to each category. The focus is on the extent to which social protection in each country is inclusive or exclusive of migrants. Differences across the welfare regimes are stark. The social democratic regime is the most inclusive, conditioning eligibility primarily on residence. The conservative type imposes more restrictive conditions, usually requiring long-term residence of five years or more. What distinguishes the liberal welfare regime is that social assistance coverage is limited to select migrant groups, such as permanent residents and recognized refugees. Welfare regimes of the East Asian type are marked by a “nationals-only” principle that largely excludes migrants from eligibility. Korea, in particular, exemplifies a model exclusive of almost all foreign-born residents, granting social assistance only to asylum seekers and a segment of marriage migrants. Given the growing trend of settlement migration, Korea will need to consider gradually extending social assistance coverage to non-national residents.
이 글에서는 이주민 대상 공공부조 제도의 국제 비교 분석을 시도했다. 한국을 포함한 12개국을 사회민주주의, 보수주의, 자유주의, 동아시아 4개 복지체제로 세 나라씩 분류하여 분석했다. 분석의 초점은 ‘공공부조에서 이주민의 제도적 포괄·배제 여부’다. 분석 결과 체제별 차이가 명확했다. 사회민주주의 체제는 ‘거주’를 기반으로 가장 포용적이었다. 보수주의 체제는 대체로 ‘5년 이상 장기체류’를 요구하는 등 엄격한 거주 조건을 부과했다. 자유주의 체제는 영주권자나 난민 등 특정 자격자만 포괄하는 배제적인 특성을 보였다. 동아시아 체제는 혈연을 중심으로 하는 ‘국민’ 원칙에 기반하여 이주민을 원칙적으로 배제했다. 특히 한국은 난민과 일부 결혼이주 여성을 제외한 모든 유형의 체류 외국인을 공공부조에서 제외하여 가장 배타적인 유형이었다. 한국에서 정주형 이주민이 늘어나는 경향을 고려하면 현재의 포괄성에 대한 점진적인 개선이 필요하다.
Old-age income protection systems constitute the foundation of the welfare state, of which the quintessential component is the public pension system. However, amid shifts in the economic structure coupled with population aging, traditional pension policies have proven insufficient to achieve the goal of old-age income protection. In this context, public pension reform has been a staple of the US political agenda for well over a decade. In the process, the US has developed a wide variety of old-age income security programs, including private pension plans and tax-based welfare programs. This article discusses in depth the three pillars of the US old-age income protection system—Social Security, occupational pensions, and individual pension plans. Exploring at length the dynamics between these pillars and tax programs, this article examines old-age income security programs in the US and their characteristics that define the American welfare state as it is today, and draws implications for policymaking in Korea.
노후소득보장제도는 복지국가의 근간을 이룬다. 그 대표적인 정책이 공적연금이다. 하지만 경제구조의 변화와 고령화가 맞물려 기존의 전통적 정책으로는 노후소득보장이라는 정책 목표를 달성하는 데 한계에 봉착하게 되었다. 이런 배경에서 공적연금 개혁이라는 과제는 지난 십수 년간 미 정치권의 주요 의제로 자리매김하였다. 특히 사적연금제도와 조세복지제도가 발달한 미국에서는 다양한 노후소득보장 정책이 발달하여 왔다. 이 글에서는 미국 노후 소득보장제도의 세 축이라 할 수 있는 공적연금, 기업퇴직연금, 개인퇴직연금의 내용을 구체적으로 살펴본다. 특히 이들 제도와 조세정책의 역학관계에 천착하여 노후소득보장제도에 녹아 있는 미국 복지의 독특한 성격이 무엇인지 알아보고 한국 사회에 주는 시사점을 짚어 본다.
