In an effort to address its declining birth rate, Japan carried out a broad reform of the child allowance scheme in 2023 that abolished income limits, expanded eligibility, and increased benefit levels. This article analyzes how the decision to abolish income limits was debated and made. The reform was debated mainly in the Diet and at expert meetings. While political considerations prevailed in the Diet, expert meetings called for a careful assessment of the effects of abolishing income limits. Ultimately, the government decided to abolish the income limits, largely driven by political logic. The reform came into effect in October 2024, and contributions to a new Child and Childcare Support Fund, levied on top of public health insurance premiums, have been collected since April 2026 to finance the expansion. For Korea, where the child allowance has been a universal benefit since 2019, the implications of the Japanese case lie not in the abolition of income limits per se, but in the process through which benefit levels were increased without rigorous evaluation of the effects, as well as in the financing structure that relies on quasi-tax contributions collected through the social insurance system.
일본에서는 2023년 ‘이차원적 저출산 대책’의 일환으로 아동수당의 소득제한 철폐, 대상 연령 확대, 수당 금액 증액 등 제도를 대폭 개편하였다. 개편된 제도는 2024년 10월부터 시행되고 있다. 이 글에서는 소득제한 철폐가 일본에서 어떤 논의를 거쳐 결정되었는지에 대해 분석하였다. 아동수당제도 개편 논의는 국회의 관련 법 개정 과정과 전문가 회의에서 주로 이루어졌다. 국회에서는 여야의 정치적 논리가 제도 자체에 대한 논의보다 우선하였다. 전문가 회의에서는 소득제한 철폐에 따른 편익 등 제도 자체에 대한 검토가 면밀하게 이루어져야 한다고 주장하였으나, 결국 정부 방침대로 소득제한 철폐가 결정되었다. 이미 보편급여로 전환된 한국의 아동수당에 대해 일본 사례가 주는 함의는 소득제한 철폐라는 결정보다 급여 확대가 효과성 검증 없이 이루어진 의사결정 과정과 재원 조달 구조에 있다.
Germany has incorporated full-day care for primary school-aged children into a federally guaranteed legal framework and has further supplemented the means of realizing this entitlement during school holidays. As a result, care that was previously provided in a fragmented manner across schools and the child-and-youth welfare sector has been restructured into a legally guaranteed system of public support. This article examines the types and current conditions of full-day care in Germany and finds that regional disparities, unmet demand, and constraints in financial and human resources remain significant challenges in practice. The German case demonstrates that care policy for primary school-aged children is not merely a matter of expanding services, but a complex policy field that requires careful consideration of legal entitlement, the coherence of delivery structures, the stability of operational foundations, and the maintenance of quality standards.
독일은 초등 학령기 아동에 대한 전일제 지원을 연방법상 권리보장 체계로 편입하고, 최근에는 방학 기간 중 권리 실현의 방식도 보완하였다. 이에 따라 학교와 아동·청소년 복지 영역에서 분산적으로 운영되던 돌봄은 법적으로 보장되는 공적 지원체계로 재구성되었다. 이 글은 독일의 초등 학령기 아동에 대한 전일제 돌봄의 제공 유형과 운영 현황을 검토하고, 실제 운영에서 지역별 편차와 미충족 수요, 재정 및 인력 확보의 한계가 여전히 중요한 과제로 남아 있음을 확인한다. 독일 사례는 초등 학령기 아동에 대한 돌봄 정책이 단순한 서비스 확대의 문제가 아니라 권리 보장과 제공 체계의 정합성, 운영 기반의 안정성, 질적 기준의 확보를 함께 고려해야 하는 복합적 제도 영역임을 보여 준다.
In the UK, with the multiple-birth rate soaring in the 1980s, concerns about maternal and infant health, along with rising public health expenditure, came to the forefront, prompting the government to implement various policy initiatives by the early 2000s to manage multifetal pregnancies. The National Institute for Clinical Excellence (NICE) adopted the Twin and Triplet Pregnancy Guideline and later introduced quality standards for pre- and postnatal care for multifetal pregnancies. Through its single-embryo transfer policy, the Human Fertilisation and Embryology Authority (HFEA) helped reduce the post-IVF multifetal pregnancy rate from 28.5 percent in 1991 to 3.4 percent in 2023. The National Health Service (NHS) implemented the Multiple Births Midwife program, which over the years has brought improvements to maternal and child health service delivery. Non-profit organizations, such as Twins Trust, have worked in collaboration with the NHS to enhance the management of maternal and child health through improved guidelines, education, and awareness campaigns. These efforts have enabled the UK to reduce multifetal pregnancies while maintaining an appropriate total fertility rate, making them a valuable source of insights and implications for Korea as it grapples with a high rate of multiple births.
영국은 1980년대 다태아 출산율이 급증하며 산모와 신생아의 건강 문제, 공공보건 의료비의 증가 등의 문제가 대두되었다. 이에 영국은 2000년대 초반부터 다태임신 관리를 위한 다양한 정책적 대응을 시행하였다. 국립보건임상연구원(NICE)은 다태아 임상 지침과 품질 향상 기준을 도입하여 다태임신 산전・산후 관리의 표준 지침을 마련하고, 인간생식 및 배아관리청(HFEA)은 단일배아 이식 정책을 통해 체외수정(IVF) 평균 다태임신율을 1991년 28.5%에서 2023년 3.4%까지 감소시켰다. 또한 국민보건서비스(NHS)는 다태임신전문조산사(MBM) 제도를 운영하며 산모 및 신생아의 건강 관리를 개선하고 있다. 다태아협회(Twins Trust) 등 비영리단체도 NHS와 협력하여 관련 지침 개선 및 교육, 캠페인을 실시하는 등 다태임신 관리의 질 향상에 기여하고 있다. 이를 통해 영국은 다태임신을 관리하면서도 출산율은 적정 수준을 유지하고 있다. 이러한 영국의 경험은 다태아 출산율이 높은 한국에도 중요한 시사점을 제공한다.
This article examines the UK quality management system for medical institutions practicing assisted reproductive technology (ART) procedures and discusses its implications for policymaking in Korea. The survey of the system centers around the Human Fertilisation and Embryology Authority (HFEA), an independent entity responsible for managing the quality of procedures and treatments involving ART. Anchored in the Human Fertilisation and Embryology Act, the HFEA was established to regulate the full gamut of fertility treatment, gamete donation, and embryo research. Complementing the HFEA are the National Institute for Health and Care Excellence (NICE) and the Medicines and Healthcare Products Regulatory Agency (MHRA), responsible for clinical guidelines and the safety of medicines and medical devices, respectively, to form an integrated quality management system. The HFEA specifies statutory provisions, licensing conditions, and mandatory requirements through its Code of Practice, and enhances transparency and accountability by collecting and publishing treatment, outcome, and safety data through its register. This UK model, which links legal and regulatory structures with practical guidance and a registry, offers a useful reference framework for developing Korea’s quality management system for ART.
이 글은 영국의 보조생식술(난임시술) 의료기관 질 관리체계를 분석하고 이를 바탕으로 한국의 제도 설계에 대한 시사점을 도출하는 것을 목적으로 한다. 이를 위해 ‘인간 수정 및 배아 관리청(HFEA)’을 중심으로 영국의 보조생식술 의료기관 질 관리체계를 검토하였다. 영국은 「인간 수정 및 배아 생성에 관한 법률(HFE Act)」에 근거해 HFEA를 설립하고, 난임 치료·기증·배아 연구 전 과정을 규제하고 있다. 아울러 HFEA, 국립보건임상연구소(NICE), 의약품·의료제품 규제기관(MHRA)이 각각 보조생식술 규제, 임상지침 및 의약품·의료기기 안전을 담당함으로써 통합적인 질 관리체계를 상호보완적으로 운영하고 있다. 특히 HFEA는 실무지침을 통해 관련 법 조항과 인가 조건, 의무 요구사항을 구체화하고, HFEA register를 통해 시술·성과·안전 관련 데이터를 관리·공개하여 기관의 투명성과 책임성을 강화하고 있다. 이러한 법·규제 구조와 실무지침, 레지스트리가 연계된 영국의 모델은 한국의 보조생식술 질 관리체계 구축에 중요한 참고 틀을 제공한다.
The US approach to managing the quality of public medical institutions that practice assisted reproductive technology (ART) relies not on direct government assessment but on a national surveillance system that, by making ART outcomes publicly available, prompts providers to improve their services voluntarily. The system also discloses whether each institution holds laboratory accreditation from a private professional association, creating further incentives for quality improvement. Intended to enhance social trust and encourage provider cooperation through data disclosure, the US quality management system differs significantly from Korea’s centralized approach. The US system, which respects clinical autonomy while providing users with transparent information, offers a meaningful model for Korea, where information available to users when choosing providers is limited and where cooperation from the medical field is greatly needed. In light of these considerations, Korea should actively consider expanding the scope of standardized data collection, disclosure, and utilization at the level of individual fertility treatment institutions.
미국은 정부가 보조생식술(ART) 의료기관의 질을 직접 평가하기보다 국가 감시 시스템을 통해 시술 결과와 성공률을 투명하게 공개하여 자율적인 질 관리를 유도한다. 보조생식술 의료기관별 민간 전문 단체의 실험실 인증을 획득했는지도 함께 공개하여 의료기관들이 자발적으로 서비스의 질을 개선하도록 촉진한다. 시술 데이터에 대한 사회적 신뢰를 제고하고, 공급자의 협조를 이끌어 내는 미국의 질 관리 체계는 중앙집권적인 한국의 방식과 차이가 있지만 시사하는 바가 작지 않다. 전문가의 임상적 자율성을 존중하면서도 소비자에게 필요한 정보를 투명하게 제공하는 미국의 사례는 시술 기관 선택 정보가 부족하고 전문가의 협조가 절실한 한국에 중요한 실마리를 제공한다. 이를 위해 한국도 시술 기관별로 표준화된 정보의 수집·공개 및 활용 범위를 확대하는 방안을 적극 논의할 필요가 있다.
The Netherlands’ policies on assisted reproductive technology are quite distinct from those of other European countries. Following the birth of its first IVF baby in 1983, the Netherlands officially introduced ART in 1985. IVF Planning Decree was enacted to regulate the establishment of IVF centers, laying the foundation for ensuring that related medical practices and procedures remained lawful. Subsequent measures include guidelines, quality assurance indicators, and accreditation programs, all aimed at enhancing safety and quality in the operation of embryo laboratories. The Dutch approach to promoting user safety and service quality in IVF procedures, with ART policies designed to strengthen procedural regulation while keeping the medical profession autonomous, offers valuable insights for policymaking in Korea.
네덜란드의 보조생식술 정책은 여타의 유럽 국가들과 다른 특징을 보인다. 1983년 첫 시험관 시술이 시작된 이후 1985년 보조생식술을 본격적으로 도입하였다. 이와 동시에 IVF 센터 허가를 위한 법 마련으로 시술의 무분별한 확장을 제한하고, 합법적 시술의 근간을 마련하였다. 또한 난임시술에 대한 임상 가이드라인을 만들고 질 지표를 개발하여 난임시술 의료기관 인증은 물론 배아 연구실의 안전과 질 관리를 위한 중요한 도구로 활용하고 있다. 네덜란드의 보조생식술 정책은 의료계의 자율권을 유지하면서 절차적 규정을 강조하는 정책상을 반영하고, 시술의 질과 환자 안전 확보를 위한 실질적인 정책 실행을 위해 노력하고 있다는 점에서 시사점이 크다.
This article examines how, since it was introduced, France’s PACS has evolved, extending beyond the scope of marriage-centric family policies to include diverse forms of families under its institutional umbrella. First established in 1999 as a system aimed at protecting the rights of same-sex couples, the PACS has taken root as a form of partnership as legally and socially approved as marriage and common-law marriage. Amid rising rates of out-of-wedlock births and increasing family diversity, the PACS has helped lay the institutional groundwork for supporting childbirth and childcare in non-marital families, helping shift family policies from a focus on marital status to prioritizing the rights of the child. As Korea continues to record the world’s lowest fertility rate, France’s PACS suggests that Korea should move away from a marriage-focused policy framework toward building institutional mechanisms that genuinely support individuals in non-marital partnerships and their childrearing.
이 글에서는 프랑스 시민연대계약(PACS) 제도의 도입과 발전이 혼인 중심 가족정책의 한계를 넘어 다양한 가족 형태를 제도적으로 수용하고 저출산 대응에 기여한 과정을 살펴본다. 1999년 도입된 PACS는 동성 커플의 권리 보장을 출발점으로 했으나, 현재는 이성 커플이 주된 이용자이고 결혼 및 사실혼과 함께 법적・사회적으로 인정되는 파트너십 형태로 자리잡았다. PACS는 혼외출산 증가와 가족 형태의 다변화 속에서 비혼 가정의 출산과 양육의 제도적 기반을 마련하였고, 프랑스의 가족정책이 혼인 여부가 아니라 아동의 권리를 중심으로 발전하는 데 기여하였다. 한국이 세계 최저 수준의 출산율을 기록하고 있는 현 상황에서 프랑스의 PACS 사례는 혼인 중심 정책 프레임을 넘어 비혼 관계와 자녀 양육을 실질적으로 지원하는 제도적 장치 마련의 필요성을 시사한다.
