Peer support workers are people who, having experienced mental health problems themselves, draw on their lived experiences to support the treatment and recovery of others experiencing similar difficulties. In Korea, recent efforts to establish a legal basis for peer support workers have increased policy interest in their role. In England, peer support workers have been incorporated into mental health care reform and recognized as a distinct workforce role within the national mental health workforce plan. England has also developed a competency framework and curriculum to define the core competencies, training programs, and career development support required for peer support workers. England’s experience suggests that multifaceted policy efforts are needed to embed peer support worker roles in mental health services.
동료지원인은 정신건강서비스 내에서 정신건강 문제를 경험한 당사자가 개인의 치료 및 회복 경험을 바탕으로 유사한 어려움을 겪는 사람을 지원하는 인력이다. 국내에서도 최근 동료지원인의 법적 기반이 마련되면서 본격적인 제도화가 추진되고 있다. 영국은 정신건강서비스 개혁의 일환으로 동료지원인 제도를 도입하였으며, 정신건강서비스 인력 확대 전략으로 동료지원인을 포함한 새로운 직군을 적극 확대해 왔다. 또한 동료지원인에게 필요한 역량 체계와 교육과정을 개발하여 역할의 표준화를 시도하였다. 영국의 사례는 정신건강서비스 내 동료지원인 제도의 정착 및 활성화를 위해 양적 확대 이상의 다각적인 정책적 노력이 필요함을 시사한다.
Australia recognizes the lived experiences of mental health consumers and their families as essential and unique expertise, formally defining them as a “distinct professional discipline” separate from clinical practice (Roennfeldt & Byrne, 2021). Through the National Mental Health Workforce Strategy (2022–2032), Australia presents a medium- to long-term roadmap that includes qualification standards and career pathways. This is supported by a standardized curriculum under the national qualification framework (Certificate IV in Mental Health Peer Work) and regulatory mechanisms to ensure fair working conditions, which together strengthen both the professional competency of peer workers and a foundation for more stable employment. Such an approach is significant in that it positions peer workers not as mere auxiliary staff, but as core agents in recovery-oriented services. The Australian case suggests that South Korea should redefine peer workers as “lived experience professionals” and establish a robust institutional foundation for their roles. This includes developing a professionalization system that links education with employment and creating a corresponding compensation structure, enabling peer workers to serve as active partners in the mental health field.
호주는 정신건강 당사자와 가족의 회복 경험을 현장의 필수적인 고유 역량으로 인정하며 이들을 임상 인력과 차별화된 독립적 전문 직역으로 공식 규정하고 있다. 국가 차원의 인력 전략(2022-2032)을 통해 자격 기준과 경력 경로를 포함한 중장기 로드맵을 제시하고 있으며, 국가 자격 체계에 따른 표준 교육과정(Certificate IV)과 법적 처우 보장 체계를 통해 동료지원인의 전문성과 안정적 고용 기반을 함께 강화하고 있다. 이러한 접근은 동료지원인을 단순 보조 인력이 아니라 회복 중심 서비스의 핵심 주체로 자리매김하게 했다는 점에서 의의가 있다. 한국 역시 동료지원인을 경험 기반 전문가로 재정립하고, 교육과 고용이 연계된 전문화 체계와 이에 상응하는 보상 구조를 마련함으로써 현장에서 실질적인 파트너로 기능할 수 있는 제도적 기반을 구축할 필요가 있다.
This paper examines Japan’s policies on mental health peer support workers and aims to draw policy implications for South Korea. Along with the establishment of a comprehensive care system, Japan has recognized peer support workers as a core component of recovery-oriented practice. By introducing the Peer Support Training Project, as well as add-on payments for service provision, Japan has provided financial incentives to promote the stable employment of peer support workers. Surveys have found that most of these workers have lived experience of mental illness. They actively participate across the spectrum of service delivery, with face-to-face peer counseling as their primary role. Furthermore, Japan has established multifaceted support frameworks, including collaborative training with professionals, mandatory two-person team placements, and the direct involvement of peers in training and professional development. These findings offer important policy implications for South Korea, which amended its legislation on peer support workers earlier in 2026, including securing a stable financial structure, establishing requirements for professional-peer partnerships, and systematizing a peer-centered training curriculum.
이 글은 일본의 정신장애인 동료지원인 정책을 고찰하고 한국에 대한 시사점을 제시하고자 하였다. 일본은 포괄케어시스템 구축과 더불어 동료지원인을 회복 중심 실천의 핵심 요소로 인정하여 피어 서포트 연수 사업, 체제 및 실시 가산 제도를 신설함으로써 동료지원인의 안정적인 고용을 유도하는 재정적 인센티브를 마련했다. 실태조사에 따르면 장애인 동료지원인의 과반수가 정신장애인으로, 대면 동료 상담을 핵심 역할로 수행하면서 서비스 제공 전반에 참여한다. 전문가와의 협동 교육, 2인 1조 배치 의무화, 당사자 양성과정 참여 등 다각적 지원체계를 구축했다. 이는 2026년 동료지원인 관련 개정 법률 시행을 맞이한 한국에 안정적 재정 구조 구축, 전문가·동료지원인 간 파트너십 요건 마련, 당사자 중심의 양성 과정 체계화 등 정책적 함의를 제공한다.
The UK Department for Education launched the “Giving Every Child the Best Start in Life” strategy in July 2025, with the aim of closing early developmental gaps and ensuring a more equal start to school. This strategy seeks to provide integrated support across family services, health care, childcare, and early education from pregnancy through to school entry, with the goal of increasing the proportion of 5-year-olds who achieve a Good Level of Development (GLD) from 68 percent in 2025 to 75 percent by 2028. The strategy also aims to establish up to 1,000 Best Start Family Hubs and as many as 2,000 connected services by 2028, provide eligible parents with 30 hours of funded childcare per week for children aged 9 months and older, and strengthen health care support through Healthy Babies services. Under the initiative, priority has been given to disadvantaged areas in the establishment of Family Hubs, which are open to all families, to improve access to services, providing continuous support and integrating local-level performance management with digital and face-to-face services. These integrated early-years support policies in the UK have useful implications for the development of a child support policy framework in Korea.
영국 교육부는 2025년 7월 아동의 발달 격차를 줄이고, 평등한 학교 출발선을 목표로 ‘모든 아이에게 최고의 출발을(Giving Every Child the Best Start in Life)’ 전략을 발표하였다. 이 전략은 임신기부터 만 5세까지 가족서비스, 보건의료, 영유아 보육 및 교육을 통합 지원하고, 2028년까지 만 5세 아동의 적정발달수준(GLD: Good Level of Development) 달성률을 현재 약 68%에서 75%로 높이는 것을 목표로 한다. 정부는 2028년까지 베스트 스타트 패밀리 허브(BSFH: Best Start Family Hubs) 최대 1000곳과 2000개의 네트워크 거점을 조성하고, 생후 9개월부터 주 30시간의 영아 무상보육 확대, 헬시 베이비(Healthy Babies) 서비스를 통한 보건의료 지원 강화를 추진한다. 또한, 모든 가정이 이용할 수 있는 허브를 취약지역에 우선 배치해 접근성을 높였으며, 임신기부터 취학기까지 지원을 연속적으로 제공하고, 지역 단위 성과관리 및 디지털·대면 서비스의 결합을 함께 시도하고 있다. 이러한 영국의 생애 초기 통합 지원 정책은 한국의 아동지원정책 체계 구축에도 유의미한 시사점을 제공할 것이다.
Germany’s Wohngeld is a supplementary housing subsidy that provides financial support to low-income households that are not eligible for basic income support. The Wohngeld-Plus reform, implemented in 2023, marked the most significant change to the housing subsidy scheme since its introduction in 1965. Against the backdrop of rising rents, the energy crisis, and inflation, the reform both broadened eligibility and increased benefit levels. In particular, the reform introduced new heating and climate components into the calculation of the housing subsidy, thereby taking into account energy costs and other expenses associated with the transition to a climate-neutral economy. In 2025, the German government increased the housing subsidy further as part of the regular biennial adjustment. While these changes have strengthened the role of the housing subsidy as a social protection instrument, the expansion of the scheme has also raised issues that warrant continued review, including questions concerning fiscal sustainability, administrative burden, actual take-up rates, benefit adequacy, and the extent to which regular adjustments alleviate housing-cost burdens.
독일 주거수당(Wohngeld)은 기초보장급여에 진입하기 이전 단계의 저소득 가구를 지원하는 보충적 주거비 급여이다. 2023년 시행된 주거수당 플러스(Wohngeld-Plus) 개혁은 1965년 주거수당 제도 도입 이후 이루어진 가장 중대한 제도 개편으로, 임대료 상승, 에너지 위기, 물가 상승을 배경으로 수급 대상과 급여 수준을 확대하였다. 특히 주거수당 산정의 새로운 구성요소로 난방비 구성요소와 기후 구성요소를 도입하여 에너지 비용과 기후전환 비용을 반영하였다. 이후 2025년에는 2년 주기의 정기 조정에 따라 주거수당이 추가 인상되었다. 이를 통해 독일 주거수당의 사회보장 기능은 강화되었지만, 제도 확대 이후 재정 지속가능성, 행정 처리 부담, 실제 수급률 제고, 급여 수준의 적정성, 정기 조정의 주거비 부담 완화 효과 등은 지속적으로 검토되어야 할 과제로 남아 있다.
In an effort to address its declining birth rate, Japan carried out a broad reform of the child allowance scheme in 2023 that abolished income limits, expanded eligibility, and increased benefit levels. This article analyzes how the decision to abolish income limits was debated and made. The reform was debated mainly in the Diet and at expert meetings. While political considerations prevailed in the Diet, expert meetings called for a careful assessment of the effects of abolishing income limits. Ultimately, the government decided to abolish the income limits, largely driven by political logic. The reform came into effect in October 2024, and contributions to a new Child and Childcare Support Fund, levied on top of public health insurance premiums, have been collected since April 2026 to finance the expansion. For Korea, where the child allowance has been a universal benefit since 2019, the implications of the Japanese case lie not in the abolition of income limits per se, but in the process through which benefit levels were increased without rigorous evaluation of the effects, as well as in the financing structure that relies on quasi-tax contributions collected through the social insurance system.
일본에서는 2023년 ‘이차원적 저출산 대책’의 일환으로 아동수당의 소득제한 철폐, 대상 연령 확대, 수당 금액 증액 등 제도를 대폭 개편하였다. 개편된 제도는 2024년 10월부터 시행되고 있다. 이 글에서는 소득제한 철폐가 일본에서 어떤 논의를 거쳐 결정되었는지에 대해 분석하였다. 아동수당제도 개편 논의는 국회의 관련 법 개정 과정과 전문가 회의에서 주로 이루어졌다. 국회에서는 여야의 정치적 논리가 제도 자체에 대한 논의보다 우선하였다. 전문가 회의에서는 소득제한 철폐에 따른 편익 등 제도 자체에 대한 검토가 면밀하게 이루어져야 한다고 주장하였으나, 결국 정부 방침대로 소득제한 철폐가 결정되었다. 이미 보편급여로 전환된 한국의 아동수당에 대해 일본 사례가 주는 함의는 소득제한 철폐라는 결정보다 급여 확대가 효과성 검증 없이 이루어진 의사결정 과정과 재원 조달 구조에 있다.
At the forefront of international health discourse is the perspective that health should be viewed less as a byproduct of economic growth and more as an objective that economies should actively pursue—a foundation for sustainable development. The World Health Organization (WHO) adopted the Health Economics for All strategy (2026-2030) at the 79th World Health Assembly earlier this year, building on the rationale set out in Health for All: Transforming Economies to Deliver What Matters, a 2023 report by the WHO Council on the Economics of Health for All, which was established in 2020. The strategy, in which health is defined not as a cost but as an investment, proposes reshaping economic policies to prioritize health and sets out five strategic directions, along with the corresponding roles of Member States and the WHO Secretariat. Reviewing the main arguments of the Council’s aforementioned report, which serves as a foundation for the strategy, as well as key elements of the strategy, including its vision and eight guiding principles, this article synthesizes the positions that Member States held at the Executive Board and the World Health Assembly and considers the implications for Korea.
건강을 경제성장의 부산물로 보던 관점에서 벗어나 건강을 경제가 지향해야 할 목표이자 지속가능한 발전의 토대로 보는 시각이 국제 보건 논의의 중심으로 떠오르고 있다. 세계보건기구(WHO)는 2020년 설치한 ‘모두를 위한 보건경제위원회’가 2023년 발간한 ‘모두를 위한 건강: 중요한 것을 실현하는 경제로의 전환’ 보고서를 토대로 2026년 제79차 세계보건총회에서 ‘모두를 위한 보건경제 전략(2026-2030)’을 채택하였다. 이 전략은 건강을 비용이 아니라 투자로 규정하고, 건강을 우선하는 방향으로 경제정책을 재설계할 것을 제안하며 다섯 가지 전략적 방향, 회원국 및 사무국의 행동을 담고 있다. 이 글에서는 전략의 토대인 위원회 최종 보고서의 핵심 논지와 전략의 비전, 8대 핵심 원칙 등 주요 내용을 검토한 뒤 집행이사회와 세계보건총회에서 이루어진 회원국의 입장을 종합하고 한국에 주는 시사점을 짚어 본다.
