Abstract

At the forefront of international health discourse is the perspective that health should be viewed less as a byproduct of economic growth and more as an objective that economies should actively pursue—a foundation for sustainable development. The World Health Organization (WHO) adopted the Health Economics for All strategy (2026-2030) at the 79th World Health Assembly earlier this year, building on the rationale set out in Health for All: Transforming Economies to Deliver What Matters, a 2023 report by the WHO Council on the Economics of Health for All, which was established in 2020. The strategy, in which health is defined not as a cost but as an investment, proposes reshaping economic policies to prioritize health and sets out five strategic directions, along with the corresponding roles of Member States and the WHO Secretariat. Reviewing the main arguments of the Council’s aforementioned report, which serves as a foundation for the strategy, as well as key elements of the strategy, including its vision and eight guiding principles, this article synthesizes the positions that Member States held at the Executive Board and the World Health Assembly and considers the implications for Korea.

건강을 경제성장의 부산물로 보던 관점에서 벗어나 건강을 경제가 지향해야 할 목표이자 지속가능한 발전의 토대로 보는 시각이 국제 보건 논의의 중심으로 떠오르고 있다. 세계보건기구(WHO)는 2020년 설치한 ‘모두를 위한 보건경제위원회’가 2023년 발간한 ‘모두를 위한 건강: 중요한 것을 실현하는 경제로의 전환’ 보고서를 토대로 2026년 제79차 세계보건총회에서 ‘모두를 위한 보건경제 전략(2026-2030)’을 채택하였다. 이 전략은 건강을 비용이 아니라 투자로 규정하고, 건강을 우선하는 방향으로 경제정책을 재설계할 것을 제안하며 다섯 가지 전략적 방향, 회원국 및 사무국의 행동을 담고 있다. 이 글에서는 전략의 토대인 위원회 최종 보고서의 핵심 논지와 전략의 비전, 8대 핵심 원칙 등 주요 내용을 검토한 뒤 집행이사회와 세계보건총회에서 이루어진 회원국의 입장을 종합하고 한국에 주는 시사점을 짚어 본다.

Abstract

This article examines how the U.S. Medicaid program, expanded under the Affordable Care Act (ACA), is being restructured through the Trump administration’s second-term budget legislation (the One Big, Beautiful Bill Act (OBBBA)). While large literature reports that ACA Medicaid expansion significantly improved access to care and health outcomes, OBBBA introduces stricter work requirements and eligibility rules that are expected to reduce coverage. As a result, the number of uninsured individuals is projected to increase substantially, alongside rising uncompensated care and greater strain on local health systems. This article reviews these developments to outline recent shifts in U.S. Medicaid policy.

이 글에서는 오바마케어 이후 확대된 미국 메디케이드 제도가 트럼프 2기 행정부(2025∼2029)의 예산법안(OBBBA)을 통해 어떻게 재편될지를 살펴본다. 오바마케어하에서 확대된 메디케이드는 의료 접근성과 건강 성과를 개선하였으나, OBBBA는 근로요건 강화와 자격 요건 조정을 통해 제도의 포괄성을 축소하는 방향으로 설계되었다. 이로 인해 향후 10년간 무보험자가 1000만 명 증가하고, 미보상 진료비와 지역 의료체계의 부담 또한 크게 늘어날 것으로 전망된다. 이러한 변화를 정책 효과의 관점에서 정리함으로써 메디케이드 제도 변화 양상을 개괄적으로 제시하고자 한다.

Abstract

In Japan, the legal framework for donations has been taking shape over time in step with the steady growth of donations. Dormant bank accounts partially fund private-sector activities serving the public interest. The “hometown tax” system offers donors tax credits, while local governments as recipients use the contributions to supplement funding and promote regional development. Both initiatives help address social welfare shortfalls that local governments cannot cover with their official budgets alone. However, the downside of both is that they often fail to reflect donors’ specific intentions regarding how their contributions should be used. To ensure that donations effectively complement social welfare as intended, continuous improvements to these systems are necessary. Such improvements are likely to occur together with increasing public awareness of the limitations of official social welfare.

일본에서는 기부 규모의 지속적 증가 추세와 궤를 같이하여 기부 관련 법제도가 정비되어 가고 있다. 휴면예금은 민간의 공익활동 단체를 지원하기 위한 자금으로 활용되고 있다. 고향납세제도를 통하여 기부자에게 세액공제 등 이 제공되고, 기부받은 지자체는 재원 확보, 지역 활성화 등으로 활용하고 있다. 휴면예금 활용 제도와 고향납세제도 모두 기존 행정기관의 재정 한계로 대응이 어려운 영역에 대한 사회보장 보완 역할을 하고 있다는 공통적 장점을 가지고 있다. 반면 자금 제공자, 기부자의 의사 반영이 미약하다는 단점 또한 공통적으로 지니고 있다. 기부가 본래의 취지대로 사회보장을 보완하는 방식으로 기능하려면 지속적인 제도 개선이 필요하다. 이러한 개선은 행정적 사회보장의 한계에 대한 국민 인식 변화와 함께 이뤄질 것이다.

Abstract

Tax policies in the modern welfare state are inextricably linked with social security policies. This is because tax policies not only provide the financial resources essential for implementing social security policies but also serve as a key mechanism for reducing market inequality. In any welfare capitalist society, however, a tax increase is a sensitive issue about which most political forces tend to remain discreet, especially in the US, where support for the market system is pervasive and firm. In the wake of the COVID-19 pandemic, however, US society and its political circles have begun to consider the prospect of a tax increase in earnest and in a concrete manner, in the context of which a buzzword on the rise is ‘wealth tax’. In this article, I examine the ongoing discussion in the US regarding the introduction of a wealth tax and consider how it will unfold in the months leading up to the November election. To do this, I revisit the concept of a wealth tax as it emerged from US policy history and offer an account of the socioeconomic background that has brought the introduction of a wealth tax into public discourse. I also discuss some of the main features of the wealth tax as proposed in various bills and consider whether, and with what significance, the wealth tax might be implemented.

현대 복지국가에서 조세정책은 사회보장정책과 불가분의 관계를 지닌다. 사회보장정책을 집행하기 위하여 필연적으로 수반되는 재원의 출처가 조세제도이기도 하거니와 조세정책은 그 자체로서 재분배를 통하여 시장의 불평등을 완화하는 주요 기재로 작동하기 때문이다. 하지만 복지자본주의에서 증세라는 이슈는 대부분의 정치세력에게 조심스러울 수밖에 없는 영역이고, 특히 시장체제에 대한 확고한 지지가 사회 저변에 자리 잡은 미국에서는 더더욱 그러하다. 그런데 코로나19 팬데믹을 거치며 미국 사회와 정치권에서는 증세에 대한 구체적인 논의들이 활발히 이루어지고 있다. 이런 맥락에서 특히 ‘부유세(wealth tax)’가 정책적 화두로 떠오르고 있는데 이 글에서는 미국 부유세 도입 논의의 핵심 내용을 살펴보고 11월 대선과 맞물려 어떠한 방향으로 전개될지 예단해 보고자 한다. 이를 위하여 먼저 부유세의 개념과 미국의 정책사를 되짚어 보고, 현재 부유세 도입 논의가 촉발된 사회경제적 배경을 알아본다. 아울러 현재 법안으로 구체화된 부유세 정책의 골자를 소개하고 향후 전망과 의미를 살펴본다.

Abstract

In a move questioning the sustainability of the growth-centered capitalist mode of production, New Zealand officially named its national budget the Wellbeing Budget, which marks the first instance of any nation taking such action. This article examines New Zealand’s Wellbeing Budget in terms of framework, wellbeing indicators, the goals and items that it consists of, what changes it has brought to government spending, the values it imparts to social welfare, and the outlook it has for future budgeting. The naming in itself, an official manifestation, in a way, of the notion that the national budget in its essence and purpose is less about pursuing growth and more about making people happier, is an achievement enough to merit high regard for having made fundamental perspectival changes to approaches to national budgeting. It is time to assess how significantly the Wellbeing Budget has changed the lives of people in New Zealand. New Zealand's actions may provide implications for the Korean welfare state, which lacks a link between fiscal policy and quality of life indicators.

경제성장에 경도된 자본주의 생산양식의 지속가능성에 대한 문제 제기의 일환으로, 뉴질랜드 정부는 현대 국가 중에서 최초로 국가의 공식 예산을 ‘웰빙 예산(Wellbeing Budget)’으로 명명하였다. 이 글에서는 뉴질랜드 웰빙 예산의 프레임워크와 웰빙 지표, 내용(목표와 예산 항목) 그리고 사회복지 가치, 예산 정책 수립, 재정 전망 차원에서의 의의를 살펴본다. 예산의 본질이자 목표는 성장보다 행복 추구라는 국가의 공식 선언만으로도, 예산 접근에 대한 근본적 시각 변화를 이끌었다는 점에서 높이 평가받을 만하다. 이제 웰빙 예산이 뉴질랜드인의 삶을 의미있게 변화시켰는지를 평가해 볼 때이다. 뉴질랜드의 행보는 재정 정책과 삶의 질 지표 간 연계가 미흡한 한국 복지국가에 함의를 제공해 줄 수 있다.

Abstract

France, the UK, and Japan, four countries where population aging has been underway since early on, suggests that as their population aging progressed, public social expenditures increased significantly and economic growth declined, thereby deteriorating the national fiscal condition. While different countries may adopt distinct strategies for public social expenditures, it is common to them all that they must finance these expenditures through tax revenues, social insurance contributions, and deficit spending. The four countries examined trended toward increased tax revenues coinciding with decreased social insurance contributions, or vice versa. It is noteworthy that a substantial government debt could slow economic growth and, furthermore, erode the tax base and social insurance financing. As Korea anticipates its 65-and-older population reaching 20 percent of the national population in just two years, now is the time for the country to start in earnest with the development of strategic approaches to social expenditures and determine an optimal mix of financial sources.

인구고령화를 앞서 경험한 스웨덴, 프랑스, 영국, 일본 등 4개국 사례를 보면, 고령화가 진행됨에 따라 공공사회복지지출은 급증하는 반면 경제성장률은 하락해 재정여건이 취약해지는 것을 알 수 있다. 공공사회복지지출에 대한 전략은 국가별로 다를 수 있지만, 어느 경우든 조세와 사회보험료, 정부부채를 통한 재원 확보가 불가피하다. 4개국들의 사례를 보면, 조세와 사회보험의 비율은 국가간 수렴하는 양상이고, 정부부채 부담이 큰 경우 경제성장을 약화시켜 재원 기반이 취약해지는 것으로 나타난다. 한국은 앞으로 2년 후 초고령사회로 진입하며 공공사회복지지출의 증가세가 본격화될 것으로 예상된다. 이에 대비하여 지출 전략의 수립과 함께 최적의 재원 확보 전략을 마련할 때이다.

Abstract

With its social security spending growing rapidly and the thinning home care workforce becoming a serious issue, the Japanese Ministry of Health, Labor, and Welfare implemented LIFE, a long-term care information system, in 2021 with the aim of making in-home care services effective so as to promote self-reliance and prevent severe conditions from developing among older adults. LIFE, with a growing amount of data gathered from across the country on in-home care facilities and offices and service users and the status of their service receipt, is now able to provide evidence-based feedback information to providers and users alike. The Japanese government anticipates that the information system will help foster an encompassing virtuous cycle of less social security spending for the government, increased trust and work efficiency for providers, and enhanced quality of life and increased health life years (increased years of independent living) for older adults. The Japanese experience can have valuable implications for Korea.

일본은 인구의 초고령화로 인한 사회보장비의 급증과 개호 인력 문제가 심각해지면서, 후생노동성에서 자립 지원과 중증화 방지를 목적으로 효과적인 개호 서비스를 실현하기 위해 2021년 대규모 데이터베이스 과학적 개호 정보시스템(LIFE)을 도입했다. LIFE에 전국의 개호 시설·사업소 이용자의 상태와 케어 상황 등의 데이터가 축적·분석됨에 따라 시설·사업소와 이용자에게 근거(evidence)에 기반한 피드백을 제시할 수 있게 되었고, 개인의 삶의 질 및 사업의 효율성 향상 등 전체적인 선순환 구조를 만들어 내고 있다. 국가 차원에서 사회보장비 억제 효과, 고령자 개인 차원에서 건강수명 연장(자립생활 연장) 효과를 기대하고 있는 만큼 한국에 시사하는 바가 크다고 할 수 있다.

Abstract

The policy of Housing First is a dramatic shift from the previous “staircase” approach in that it is aimed at providing housing upfront, without going through preparatory steps, to homeless people with alcohol dependence or mental issues, for whom housing access or independent living was deemed unfit in the past. This article introduces the background of Housing First policies in European countries, the achievements of these policies, and the Housing First Europe Hub, a coalition of organizations responsible for Housing First policies in Europe. This article goes on to outline the special features of the European Housing First policies and draw policy implications for Korea.

하우징 퍼스트(Housing First)는 알코올 문제나 정신장애 등으로 주거 접근이나 주거생활 유지가 어렵다고 간주되는 취약 노숙인을 대상으로 준비 과정 없이 곧바로 주거 제공부터 시작한다는 점에서 과거 단계적 주거지원 방식을 획기적으로 전환한 정책 방향이다. 이 글에서는 유럽의 하우징 퍼스트 도입 배경과 관련 허브 조직인 하우징 퍼스트 유럽 허브(Housing First Europe Hub)를 소개하고 유럽 주요 국가의 하우징 퍼스트 관점에 기반한 노숙인 정책과 성과의 주요 내용을 서술한다. 이를 바탕으로 유럽연합(EU) 국가들의 하우징 퍼스트 정책의 특징을 정리하고 한국 사회에 하우징 퍼스트 관점의 정책을 도입할 필요성과 필요조건을 제시한다.

초록

금융위기 이후 보수정권이 긴축정책을 추진한 지 10년여가 지났지만 영국의 사회정책을 이렇다 저렇다 판단하기는 여전히 어렵다. 오늘날 영국의 긴축정책이 과거 영국의 긴축정책 혹은 다른 나라의 긴축정책과 다른 것인 지도 불분명하다. 영국이 GDP 대비 사회지출을 크게 축소한 것은 의심의 여지가 없으나 총사회지출은 늘어났고 높은 수준의 재정적자는 꿈쩍하질 않는다. 경기침체의 파급효과는 임금 인상 억제와 불안정한 일자리의 증가로 나타났다. 역사적, 비교적 관점에서 복지 긴축이 영국의 빈곤, 불평등에 미친 영향에 대한 평가는 매우 상반된다. 한편 정책에 따른 ‘고통’의 정도에 대해서는 논쟁의 여지가 있으나 영국의 긴축정책이 국가 부채 감소에는 효과가 없는 것으로 보인다.
