저소득층의 공공 암검진 수검률의 증가 추세에도 불구하고 소득 수준에 따른 수검률의 차이가 여전히 존재하며, 민간 암검진이 고소득층을 중심으로 이뤄지고 있어 전체 암검진 수검에 소득에 따른 불평등이 존재할 것으로 예측되고 있다. 이에 본 연구는 소득에 따른 암검진 수검의 불평등 양상을 국민건강영양조사 2010-2012 자료를 이용하여 집중지수(CI)를 산출하고 이에 기여하는 요인들을 집중지수 분해 방법으로 밝히고자 하였다. 연구 결과, 암검진 수검에 있어 고소득층에게 유리한 불평등이 존재하고 있었다(CI=0.054, SE=0.004). 본인부담금이 있는 암검진의 경우 고소득층 편향의 이용패턴을 보였으나(CI=0.143, SE=0.011), 본인 부담금이 없는 무료 암검진의 경우 저소득층의 이용이 집중되어 있었다(CI=-0.118, SE=0.007). 분해방법 결과, 이러한 불평등에 기여하는 주요한 요인은 민간보험 가입 여부와 소득 수준이었다. 한편, 수평적 형평지수(HI) 값에서도 고소득층에 유리한 불평등을 재확인하였다(HI=0.056). 본 연구결과, 암검진의 형평관련 정책이 지속적으로 이루어질 필요가 있으며, 특히 본 연구에서 밝혀진 불평등에 기여하는 요인들을 고려하여 정책이 수립될 필요가 있다.;Cancer is one of the heaviest burdens of disease in Korea. For early detection and timely treatment, regular cancer screening is suggested for all individuals over aged 40. Although the rate of cancer screening is continuously increasing in Korea, existing literature suggest that income-related inequalities in cancer screening exist regardless of type of health care system. This study aimed to assess income-related inequalities in cancer screening using a nationally representative survey data. In addition, this study quantified socio-demographic factors contributing income-related inequalities in cancer screening in the Korean population. For our analysis, Korea National Health and Nutrition Examination Survey (KNHANES) 2010-2012 were used. To measure income-related inequalities, we employed concentration index (CI) and decomposition of the CI method. Our results showed that income-related inequalities in cancer screening exist indicating a pro-rich pattern along with a pro-rich pattern of horizontal inequity index (HI). The main contributors to the existing inequities were private insurance and income levels. Our study suggests that the identified inequities and main contributors in relation to cancer screening should be considered for policy interventions. Further longitudinal studies are needed to understand impact of changes in co-payment for cancer screening in Korea.
